L’auteure, photographe et photo-thérapeute, essaie d’élaborer une nouvelle médiation thérapeutique basée sur l’acte photographique.
Cet article a pour objectif d’interroger les processus psychiques ...mis en œuvre dans la dynamique de la prise de vue photographique et d’explorer ce qui peut être mis au travail avec le support photographique dans une pratique clinique.
Il s’agira d’écouter les œuvres photographiques du célèbre photographe japonais Hiroshi Sugimoto afin de voir ce qu’elles peuvent apporter à un travail clinique en photothérapie.
Pendant sa pratique artistique, Hiroshi Sugimoto convoque des traces mnésiques liées à sa propre histoire et à celles de ses ancêtres et les réactualise dans ses photos. Il vit toutes sortes de sensations corporelles et d’hallucinations. Ses photos sont déjà en lui sous forme de visions intérieures avant même de les avoir exprimées matériellement. La transposition élaborative de l’image intérieure, devenue préconsciente, se fait dans un matériau bien précis : son antique appareil photo moyen format et ses éclairages particuliers viennent non seulement servir à créer une ambiance, mais aussi à le faire voyager dans l’histoire de l’humanité et dans sa propre histoire intérieure. Il en arrive à faire corps avec son appareil photo, ce qui contribue à le plonger dans un état de connivence avec l’objet photographié.
Pendant son processus créateur, le photographe passe par des phases similaires à celles que traverse le bébé lors de son développement et ce, afin de tenter d’arriver à une (ré)actualisation du moi : une alternance entre des phases d’intégration et de non-intégration du moi avec des phénomènes régressifs plus ou moins chaotiques, une certaine identification comportant un état monadique entre le photographe et l’appareil photo et entre le photographiant et l’objet photographié, la mobilisation de sensations corporelles et un retour à un état de fluidité dans le regard et dans le geste.
En photo-thérapie, cette réactualisation du moi est recherchée et le processus créateur en photographie semble y contribuer dans une certaine mesure. Les états régressifs accompagnés de sensations corporelles, c’est-à-dire, cet état de pathique qui traverse le patient photographiant, peut redonner, dans l’après-coup, du mouvement dans la psyché et en réactualiser certains aspects qui n’avaient pas été (suffisamment) figurés. Les images intérieures de sa psyché s’expriment sous la forme de photos extérieures matérielles mais cette fixation permet de retravailler, en boucle rétroactive, l’image intérieure initiale et donc de remodeler la psyché, en passant par le corps. L’appareil photo, qui a la particularité d’être à la fois l’outil mais aussi la surface d’inscription, s’intègre au corps du patient photographiant. Le transfert est donc diffracté entre l’analyste et le médium photographique. Issue de la subjectivité du sujet photographiant et de celle de l’objet photographié, se crée une troisième subjectivité, une sorte de « tiers analytique » : la photo chargée de sens, analysable en thérapie.
The author, a photographer and photo-therapist, tries to develop a new therapeutic approach based on the photographic act.
This article aims to examine the psychic processes involved in the dynamics of taking photos and to explore what can be done with the photographic medium in clinical practice.
We will listen to the photographic works of the famous Japanese photographer Hiroshi Sugimoto to see what they say about clinical work involving photo-therapy.
During his artistic practice, Hiroshi Sugimoto summons up memory traces related to his own history and that of his ancestors and updates them in his photos. He experiences all kinds of bodily sensations and hallucinations. His photos are already within him in the form of inner visions, even before he has expressed them materially. The elaborative transposition of the inner image, which has become pre-conscious, takes place in a very specific material: his antique medium-format camera and his special lighting not only serve to create an atmosphere, but also to take him on a journey through the history of humanity and his own inner history. He comes to identify completely with his camera, which helps to plunge him into a state of fusion with the object being photographed.
During his creative process, the photographer goes through phases similar to those that the baby goes through during his development, in an attempt to achieve a (re)actualization of the self: alternating phases of integration and non-integration of the self with more or less chaotic regressive phenomena, a primary identification with a quasi-fusional state between the photographer and the camera and between the photographer and the photographed object, the mobilization of bodily sensations and a return to a state of fluidity in the gaze and gesture.
In photo-therapy, this re-actualization of the self is sought and the creative process in photography seems to contribute to this to a certain extent. The regressive states accompanied by bodily sensations, that is to say, this state of pathos that runs through the photographing patient, can, in the aftermath, restore movement in the psyche and bring back to life certain aspects that had not been (sufficiently) represented. The inner images of the psyche are expressed in the form of material external photos but this fixation allows the initial inner image to be reworked in a retroactive loop, and thus to reshape the psyche, through the body. The camera, which has the particularity of being both the tool and the surface of inscription, becomes part of the body of the photographer-patient. The transference is thus diffracted between the analyst and the photographic medium. From the subjectivity of the photographing subject and that of the photographed object, a third subjectivity is created, a kind of “analytic third:” the photo is charged with meaning that can be analyzed in therapy.

S’appuyant sur les fondements théoriques de la psychanalyse, cet article pose la problématique clinique qui a été à l’origine de la conception et la mise en place d’un dispositif de médiation ...thérapeutique par le son, la musique et la vibration proposé à des patients adolescents souffrant d’anorexie mentale. Après une description du dispositif, les résultats d’une expérience pilote réalisée au département de psychiatrie de l’adolescent et du jeune adulte de l’Institut Mutualiste Montsouris sont présentés. Ils ouvrent des perspectives nouvelles pour comprendre la singularité et la pertinence de ce dispositif face aux difficultés rencontrées dans le travail clinique avec ces jeunes patients.
La désaffectation du corps et le refus des liens dans le rapport à l’objet en deçà de la relation à l’autre se présentent comme des modalités de défense psychique face aux menaces du vécu de débordement pulsionnel réactivé par les métamorphoses de la puberté. Le cadre de la thérapie fondée sur la parole et l’associativité, étayée sur la relation transférentielle, vient souvent renforcer ces angoisses et, par conséquent, les défenses chez ces patients organisés autour des symptômes anorexiques. Dans le but de favoriser le désamorçage progressif des défenses et la mise en marche des mouvements psychiques par l’associativité, un dispositif clinique innovant de médiation thérapeutique par le contact corporel avec les vibrations liées au son et à la musique a été conçu et proposé. Huit adolescents, dont un garçon et sept filles, âgés de 14 à 19 ans, hospitalisés pour anorexie mentale ont participé à cette expérience clinique. Ainsi, pendant cinq séances d’atelier, les patients ont exploré leurs sensations corporelles en présence de deux psychologues cliniciennes. Les sensations corporelles étaient suscitées par le jeu avec des séquences sonores et musicales perçues à travers le contact avec quatre objets médiateurs sonovibrotactiles : une table, un ballon, un plaid et un coussin repose-nuque.
Le discours, les gestes et les actions des patients pendant les séances d’atelier ont été révélateurs des manières dont ils ont investi psychiquement les objets sensoriels du dispositif. Les séquences sonores des pulsations cardiaques et des événements aquatiques associés à la mer (dont plusieurs représentations ont été explorées), avec leurs vibrations, ont été particulièrement investies. Les objets médiateurs ont été explorés avec une diversité de postures corporelles. Les sensations suscitées par les rapports psychocorporels à ces objets ont fait émerger des représentations et des affects liés au vécu que les patients ont exprimé dans la prise de parole avec les psychologues. L’expression verbale et non verbale des patients nous a permis de repérer trois grandes catégories d’analyse : l’expression de différents types de difficultés à jouer avec le dispositif ; l’expression des sensations corporelles suscitées et le rapport des patients au plaisir ou au déplaisir associé à ces sensations ; l’expression de contenus liés au vécu de la maladie, à la famille, l’inconnu, la solitude, la peur et l’angoisse, la mort, et l’affect entre autres. Les objets d’expérience ont rempli en ce sens une fonction projective, d’embrayeurs d’une dimension associative et d’un travail clinique possible.
Ce dispositif clinique de médiation, par ses objets sonovibrotactiles, est attractif pour les adolescents. Il introduit cependant chez le sujet une expérience sensorielle surprenante et peu familière, potentiellement troublante, faisant surgir l’inquiétante étrangeté de l’intime pulsionnel qui échappe au sujet. Le sujet récupère par cet intermédiaire des sensations corporelles intenses qui réactivent des expériences primaires enfouies. En ce sens, l’intérêt mais également le risque de ce dispositif, c’est qu’il touche aux zones profondes du corps, pouvant ainsi déjouer les barrières des mécanismes défensifs et faisant ressortir des contenus psychiques ayant une grande charge affective. C’est pour cela que ce dispositif ne peut pas être mis en place sans le clinicien, qui soutient par sa présence la capacité de porter une attention aux effets psychiques provoqués par ces sensations corporelles et de les reconnaître. Cet étayage permet également de ne pas laisser le sujet livré à lui-même.
La singularité de ce dispositif consiste en ce qu’il propose au sujet une constellation d’objets malléables de différents types comme vecteurs de son rapport à ses propres objets pulsionnels. Ce dispositif est en ce sens pensé comme une médiation à visée thérapeutique pouvant favoriser, selon les possibilités psychiques des patients, le réinvestissement libidinal des zones corporelles désaffectées dans les anorexies mentales. Les éprouvés corporels suscités et réactivés par le contact avec les différents types d’objet actualisent des traces mnésiques qu’il est fondamental de reconnaître dans le cadre du lien transférentiel entre le clinicien et le patient, notamment dans l’échange discursif.
Based on the theoretical foundations of psychoanalysis, this article outlines the clinical problem at the origin of the conception and the implementation of a therapeutic mediation platform using sound, music, and vibration for adolescent patients with anorexia nervosa. After a description of the platform, the results of a pilot experience carried out in the adolescent and young adult psychiatry department of the Institut Mutualiste Montsouris are presented. They open up new perspectives for understanding the singularity and the relevance of this platform with respect to the difficulties encountered in clinical work with these young patients.
The disaffectation of the body and the refusal of links in the relation to the object behind the relation to the other present themselves as modalities of psychic defense responding to threats coming from the experience of instinctual overflow, reactivated by the metamorphoses of puberty. The framework of therapy based on speech and associativity, supported by the transference, often reinforces the anxieties and, consequently, the defenses of patients organized around anorexic symptoms. In order to promote the progressive defusing of defenses and the activation of psychic movements through associativity, an innovative clinical platform for therapeutic mediation through bodily contact with vibrations linked to sound and music has been designed and proposed. Eight adolescents, a boy and seven girls, aged 14 to 19, hospitalized for anorexia nervosa participated in this clinical experience. During five workshop sessions, the patients explored their bodily sensations in the presence of two clinical psychologists. The bodily sensations were provoked by the play of sound and musical sequences perceived through contact with four audiovibrotactile objects : a table, a balloon, a blanket, and a neck pillow.
The speech, gestures, and actions of the patients during the workshop sessions revealed the ways in which they psychically invested the sensory objects of the platform. The sound sequences of heartbeats and aquatic events associated with the sea (several representations of which have been explored), with their vibrations, were the site of special investments. The objects were explored through a variety of body postures. The sensations called up by the psycho-corporeal relationships with these objects brought to light representations and affects linked to life experiences that patients had expressed in speaking with psychologists. The patients’ verbal and non-verbal expression allowed us to identify three main categories of analysis: the expression of different types of difficulty in engaging with the platform; the expression of the bodily sensations aroused and the relationship of patients to the pleasure or displeasure associated with these sensations; the expression of contents related to their experience of disease, to the family, to the unknown, to loneliness, fear and anxiety, death, and affect, among others. In this sense, the experimental objects fulfilled a projective function, as shifters of an associative dimension and possible clinical work.
This clinical mediation platform, through its audiovibrotactile objects, is attractive to adolescents. However, it introduces into the subject a surprising and unfamiliar, potentially disturbing sensory experience, bringing out the uncanniness of the intimate drive that subject cannot control. The subject experiences intense bodily sensations that can reactivate buried primary experiences. In this sense, the interest but also the risk of this platform is that it touches the deepest areas of the body, thus allowing it to overcome the barriers of the defensive mechanisms and to bring up psychic contents with a strong emotional charge. This is why this platform cannot be put in place without the clinician, who by his/her presence supports the ability to pay attention to the psychic effects provoked by these bodily sensations and to recognize them. The clinician's anaclitic function also means that patients are not left to their own devices.
The singularity of this platform consists in that it offers the subject a constellation of malleable objects of different types as vectors of her/his relationship to own instinctual objects. In this sense, this platform is thought of as mediation with a therapeutic aim which can encourage, depending on the psychic possibilities of the patients, the libidinal reinvestment of disaffected body areas in anorexia nervosa. The bodily experiences aroused and reactivated by contact with the different types of object update memory traces that it is essential to recognize in the context of the transferential link between the clinician and the patient, especially in discursive exchange.
Ce travail vise à examiner le rapport subjectif à la voix et à l’autre chez la personne autiste. Plus précisément, il vient interroger les possibilités d’expression de l’objet pulsionnel voix dans ...l’autisme, principalement à travers le geste dansé.
À l’oralisation et à la sonorisation, les auteurs proposent un troisième mode de traitement de la voix : la visualisation. Cette hypothèse fut mise à l’épreuve à partir d’un projet de médiation par la danse avec un enfant autiste mis en place au sein d’une institution.
Au fil des séances et à partir de l’exploration de mouvements orientés et adressés, ce dispositif a permis l’ébauche d’une grammaire gestuelle commune ouvrant la voie à la danse.
En faisant l’hypothèse que la pulsion invocante peut se traduire dans le registre gestuel/visuel, impliquant la substitution d’une boucle pulsionnelle bouche-oreille par une boucle corps-œil, l’analyse des différents extraits cliniques présentés et l’identification des modalités de bouclage de la pulsion invocante a permis de témoigner d’une visualisation de la voix.
La médiation par la danse ou par le mouvement pour la personne autiste pourrait favoriser l’établissement d’un lien à l’autre et rendrait possible l’expression de la voix comme objet pulsionnel.
This work aims to examine the subjective relationship, for autistic people, to the voice and to the other. More specifically, it questions the possibility of expression of the object-voice in autism, mainly through dance.
In addition to oral expression and voicing, the authors propose a third mode of processing the voice through visualization. This hypothesis was tested through a therapeutic dance project with an autistic child within an institutional setting.
Over the course of the sessions, by exploring directed and addressed movements, this project allowed for the sketching out of a common gestural grammar leading to expression through dance.
By making the hypothesis that the invoking drive can be transposed in movement through a passage from the mouth-ear drive loop to the body-eye loop, the analysis of the different clinical extracts presented and the identification of the invoking drive loop allow us to witness a visualization of the voice.
Mediation through dance or movement for the autistic person could facilitate the establishment of interpersonal relationships and could make possible the expression of the libidinal voice.

L’organisation d’un hôpital peut être considérée comme un système complexe sur le plan managérial. De nombreux facteurs expliquent l’émergence de conflits qui correspondent à une situation de perte ...de sens ou une menace de rupture dans l’organisation. Avant la création du système de médiation dans les établissements publics de santé et médico-sociaux par le décret du 28 août 2019, la gestion de ces conflits était assurée par le Centre national de gestion (CNG), notamment lorsque des praticiens étaient impliqués. En août 2017, la ministre de la Santé confie à Édouard Couty une mission pour préfigurer un service de médiation pour les personnels des établissements de santé et médico-sociaux publics. L’objectif principal du présent article est de proposer une analyse des missions de médiations effectuées pendant la phase de préfiguration entre 2017 et 2020, ainsi que celles effectuées en 2021 et 2022, et ce, à partir des rapports effectués par le service de médiation du ministère de la Santé et des Solidarités. L’objectif secondaire est de préciser les caractéristiques de la médiation, ses avantages et ses limites dans le cadre de la résolution des conflits. Pendant la phase de préfiguration (2017–2019), 236 dossiers ont été enregistrés avec un taux de résolution de 52 % par un processus de médiation (136 médiations). Les accords issus de la médiation sont plus durables s’ils intègrent les besoins, intérêts, ressentis et ressources des personnes concernées ; ils sont ressentis comme plus équitables si les parties contribuent conjointement à leur élaboration.
The organisation of French hospital can be considered as complex system from a managerial point of view. Many factors explain the development of the emergence of conflicts that correspond to a situation of loss of meaning, or a threat of breakdown in the organisation. Prior to the creation of the mediation system in public health and medico-social establishments by the decree of 28th August 2019, the management of these conflicts was handled by the Centre national de gestion (CNG), particularly when practitioners were involved. In August 2017, the Minister for Health entrusted Édouard Couty with the task of prefiguring a mediation service for the staff of public health and medico-social establishments. The main objective of this article is to provide an analysis of the mediation missions carried out during the prefiguration phase between 2017 and 2020, as well as those carried out in 2021 and 2022, based on the reports produced by the mediation service of the Ministry of Health and Solidarity. The secondary objective is to specify the characteristics of mediation, its advantages and its limits in the context of conflict resolution. During the pre-configuration phase (2017–2019), 236 cases were registered, with a mediation resolution rate of 52% (136 mediations). Mediated agreements are more sustainable if they take into account the needs, interests, emotions and resources of the people involved, and are perceived as fairer if the parties contribute jointly to their development.
Largement exploitée dans différents domaines d’étude et de pratique en sciences de l’information et de la communication et discutée dans les recherches liées au monde culturel (musées, bibliothèques ...et centres culturels), la notion de médiation n’a pas cessé de s’étendre, malgré la diversité conceptuelle, sur les autres sphères des sciences humaines et sociales. Considéré comme véhicule du processus de la sémiose, cette notion s’interpose au cœur de la réflexion sémiotique comme instance chargée de faire passer le sens. L’espace de la ville, ses composants et les pratiques qui s’y déroulent sont sollicités dans cette étude, comme point d’articulation de notre problématique qui consiste à savoir comment le pont en tant qu’élément architectural dans l’espace d’une ville, par son statut, les pratiques qui s’y passent et les valeurs qu’il porte, pourra fonctionner comme médiateur sémiotique. Nous tenterons de montrer à travers l’étude de notre corpus, le pont Khadjou à Ispahan, ville iranienne, comment ce rôle médiateur s’implante sur le niveau cognitif autant que passionnel des sujets qui le côtoient.
Over the past thirty years, there has been a dramatic shift in the way the legal system approaches and resolves family disputes. Traditionally, family law dispute resolution was based on an ..."adversary" system: two parties and their advocates stood before a judge who determined which party was at fault in a divorce and who would be awarded the rights in a custody dispute. Now, many family courts are opting for a "problem-solving" model in which courts attempt to resolve both legal and non-legal issues.
At the same time, American families have changed dramatically. Divorce rates have leveled off and begun to drop, while the number of children born and raised outside of marriage has increased sharply. Fathers are more likely to seek an active role in their children's lives. While this enhanced paternal involvement benefits children, it also increases the likelihood of disputes between parents. As a result, the families who seek legal dispute resolution have become more diverse and their legal situations more complex.
InDivorced from Reality, Jane C. Murphy and Jana B. Singer argue that the current "problem solving" model fails to address the realities of today's families.
