In this article, we approach strategic technology management from the perspective of the quintuple innovation helix with embedded AI modalities as it applies to public interest technologies (PIT) and ...we focus on the use cases of semiconductors, fusion, and AI per se. We have named this methodology the Quintuple Innovation Helix plus AI (QIH+AI). The need for an increasingly sophisticated and dynamic balancing act between techno-centric and human-centric design philosophies. These paradigms include the Industry 5.0 (more techno-centric) and the Society 5.0 (more human-centric) paradigm as well as the emerging Industry 6.0 and Society 6.0 paradigms where the presence, role and influence of AI modalities are rather substantive and even omnipresent. In this article, we develop and propose the QIH+AI methodology as a powerful tool for strategic technology road mapping. This article provides a comprehensive framework for all stakeholders to address the question of "how to" effectively navigate the complexities of strategic technology management in an uncertain environment.
The concept of industry 4.0 (i4.0) encompasses the integration of different technologies into an autonomous, knowledge- and sensor-based, self-regulating production system. Our objective is to ...synthesize which are the challenges and opportunities of adopting i4.0 from the perspective of technology provider companies. A single-case research was conducted with ten companies at the Portuguese Production Technologies Cluster. Based on i4.0 technologies – Augmented reality; Additive Manufacturing; Big Data; Cloud Computing; Cyber-Physical Systems; Cybersecurity; Smart Robotics; Simulation; and System Integration – interviewees mentioned that the main adoption challenges are the analysis of data generated, integration of new technologies with available equipment and workforce, and computational limitations. The main opportunities are improvements in: efficiency; flexibility; productivity; cybersecurity; quality of products and services; and decision process due to data analysis. Interviewees have also foreseen changes in company's business model through the integration of internal resources with complementary activities of their partners and other cluster companies.
Calcitonin gene-related peptide monoclonal antibodies (CGRP mAbs) are novel high-cost treatments for the prevention of migraine. This study presents data on utilization, expenditure, and treatment ...patterns with CGRP mAbs available under a managed access protocol in Ireland, to a cohort of treatment refractory patients (failed 3 or more previous treatments) with chronic migraine.
Data were extracted from the Primary Care Reimbursement Service High Tech claims database and special drug request online system and analyzed using Microsoft Excel and SAS. Treatment persistence was evaluated by refill patterns, and adherence was evaluated using the proportion of days covered method. Expenditure data were extracted directly from the database.
Between September 1, 2021 and April 30, 2023, 1517 applications for reimbursement approval for a CGRP mAb were received; 1458 (96.1%) were approved for reimbursement. Total expenditure on CGRP mAbs in year 1 (September 1, 2021 to August 31, 2022) was €3.2 million. The majority of patients initiated treatment with fremanezumab (60.8%) or erenumab (37.1%). Almost 90% of patients were considered adherent, and treatment persistence was high, with more than 75% of patients receiving more than 12 months of treatment in our 18-month study time frame.
This study demonstrates the importance of active health technology management, after reimbursement, in enabling cost-effective use of high-cost treatments while providing budget certainty for the healthcare payer. High levels of adherence and persistence suggest that treatment is successfully targeted in situations which unmet clinical need is greatest.
