  • Načela dermatološkega zdravljenja mešanih razjed na spodnjih udih = Principles of dermatological treatment of mixed ulcers of the lower extremities
    Košiček, Metoda
The frequency of chronic leg ulcers and the percentage of mixed leg ulcers, their definition, diagnosis and differential diagnosis are described. Mixed leg ulcers with lesser deterioration of the ... arteries are emphasized. The damage to the arteries can be evaluated by calculating the ankle brachial pressure index (ABPI values between 0.8 and 0.5 mean lesser deterioration and those lower than 0.5 greater deterioration of leg arteries). The therapy of mixed leg ulcers can be divided into five phases. Phase 1: Compression therapywith short-stretch bandages (elasticity less than 70%) is an important part of treatment of mixed leg ulcers with lesser damage to the arteries. Phase 2: Local symptomatic therapy of the ulcers is focused on debridement andelimination of necrotic tissue and formation of a wound matrix that will promote epithelization. Phase 3: Various vasoactive medicaments are an important additional therapy of mixed leg ulcers. Phase 4: Revascularization by endovascular or surgical means is the first step in most cases of mixed legulcer treatment. Phase 5: Patient education is vital for obtaining the besttreatment results.
