    Pregnant women with antiphospholipid syndrome (APS) have increased risk for venous or arterial thrombosis and obstetric complications threatening mother and fetus. The main cause of maternal ... mortality in western countries is venousthrombosis complicated with pulmonary embolism. Treatment of vascular complications in pregnancy differs from treatment in other patients. The live birth rate in non-treated pregnant women with APS is only 10 to 20%, however it rises up to 80% in treated pregnancies. Subcutaneous low-molecular-weight heparin and low dose aspirin is current standard therapy for management of thrombosis and prevention of obstetric complications in pregnant women with APS. Recommendatians for prevention and treatment of obstetric and vascular complications in pregnant women with APS do not meet criteria for the evidence-based medicine, in contrary, the recommendations mostly follow clinical experiences and retrospective studies.
