  • Rezultati raziskave "Euroheart failure" v Sloveniji: zdravljenje bolnikov s srčnim popuščanjem
    Lainščak, Mitja ...
Background and aim of the study. In years 2000-1 European Society of Cardiology performed a "EuroHeart Failure" Survey in 24 European countries, including Slovenia. The aim of the survey was to ... determine the diagnostic and therapeutic ma-nagement of patients, hospitalised for heart failure. Methods. In the prospective survey medical records of all patients admitted during six weeks in April and May 200o to the University Medical Centre, Hospital of Internal Medicine, and Departments of Internal Medicine in General Hospitals Murska Sobota and Slovenj Gradec were screened. Only patients with diagnosis of heart failure on discharge were included and the data on drug treatment were obtained from the discharge letter. Results. We included 318 patietns (53% males), aged 72 12 years. They were discharged with 6 drugs (median value, range 2 to 13) drugs. Furosemide was prescribed to 79%, digitalis to 50%, angiotensin converting enzyme (ACE) inhibitors to 63% , beta-adrenergic blockers to 15% in spironolactone to 26% of patients. The combination of two or three neurohormonal drugs were prescribed only to 32% of patients. The daily dosages of ACE inhibitors and beta blockers reached 60% and 38% of target doses. The presence of left ventricular systolic dysfunction (ejection fraction below 40%) was not associated with higher use of neurohormonal drugs.Conclusions. Our results show inadequate implementation of heart failuretreatment guidelines in clinical practice. This holds both for usage ofACE inhibitors as well as beta blockers and prescribed daily dosages of these drugs. The presence of left ventricular systolic dysfunction did not significantly influence the pharmacological therapy.
