Aggressive periodontitis is characterized by considerable attachment loss overa relatively short period of time. The disease may develop as a consequence of highly aggressive pathogens or a highly ...susceptible host. The diagnosis is based on the following primary features: non-contributory medicalhistory, rapid attachment loss and bone destruction, and familial aggregation of cases. Secondary features present in most patients include: sparse microbial deposits inconsistent with the severity of periodontal tissuedestruction, elevated proportions of Actinobacillus actinomycetemcomifans and Porphyromonas gingivalis, phagocyte abnormalities, and a hyper-responsive macrophage phenotype associated with elevated production of prostaglandin E2 and interleukin 1 beta in response to bacterialendotoxins. A localized and a generalized form of the disease have been recognized on the basis of specific clinical and laboratory features. Thetreatment consists oral hygiene measures, periodontal surgery, supportive therapy, full-mouth disinfection and antimicrobials.
