  • Antagonisti aldosteronskih receptorjev in srčno popuščanje = Aldosterone receptor antagonists in chronic heart failure
    Ružič-Medvešček, Nadja ; Pitamic, Samo, 1962-
Chronic heart failure, a syndrome with increasing incidence and prevalence, represents the main cause of cardiovascular morbidity and mortality. It is a complication of spectrum of etiologically ... various diseases which, in syndrome progression, include complex pathophysiological mechanisms. Newer data indicate that aldosterone plays important and partly from kidney independent role in heart failure progression. Binding to mineralocorticoide receptors aldosterone causes direct harmful effects to myocardium and vasculature. Aldosterone is coresponsible for cardiac hypertrophy, necrosis, fibrosis, cardiac remodeling and endothelial dysfunction, factors known to contribute tothe pathophysiology of development and progression of heart failure. Clinical and detailed mechanistic studies performed by the end of millennium, have proved mineralocorticoid receptor antagonism to be a key component of CH F treatment regimen. It seems reasonable to expect aldosterone receptor antagonists to become a part of a new therapeutic paradigm along the whole range of cardiovascular diseases, together with already established ACE inhibitors, angiotensin II receptor blockers and beta blockers.
