    Complex dental plaque with opportunistic pathogenic bacteria found on tooth surfaces develops al so on the surface of dental implants. The bacteria give rise to periimplant mucositis, an inflammatory ... response in the surrounding mucosa that is similar to gingivitis. Mucositis may eventually progress to periimplantitis, which resembles periodontitis. Periimplant mucositis developsaround more than 70 per cent of implants, and periimplantitis is seen around 10 to 20 per cent of implants. Both complications can be prevented by proper cleaning, early detection and simple maintenance procedures. The condition of the mucosa should be monitored by visual inspection, measurement of probing depth, and detection of bleeding on probing. A reliable sign of periimplantitis is a bony crater visi ble on x-ray. The first step in the treatment of periimplantitis is instruction and motivation for proper cleaning. This is followed by removal of soft and hard deposits from the implant surface, use of a chlorhexidine rinse or gel, antibiotic treatment, and finally surgery. During a flap operation, the walls of the bony crater areremoved and the implant surface smoothed, or a regeneration and osseointegration procedure is applied.
