    Background. Pulmonary emphysema (PE), chronic bronchitis (CB) and small airways disease (SAD) are nowadays included in chronic obstructive pulmonary disase (COPD), although many other similar ... diseases with obstructive disturbances are added to COPD by lifferent authors. In spite of high incidence of COPD, the diagnostic value of the biopthic specimens in patients with COPD is limited. Methods. The most common bronchial/transbronchial bioptic tissue specimens, obtained with fiberbrounchoscopy, are very small, often tranumatized, and display many artifacts. Such material is usually unsatisfactory for the diagnosis of PE, and the evaluation of its type and size is impossible. Clinical data are the most relevant for the diagnosis of CB. Small airways are often present in bioptic specimens. Thus, frequent nonspecific and seldomspecific changes may be observed in SAD and many other disases, including asthma, which may also cause airways obstruction; they should be considered in differential diagnosis of COPD. Conclusions. Better diagnostic yield of clinical functional, laboratory etc. methods in COPD demands thorough indication for the biopsy to be ethically justified and professionally rewarding. Ethical limits for biopsies in patients with COPD have also restrictive influence on research of etiopathogenesis and pathology with modern methodologies in human bioptic specimens.
