Creation of sizable subintima during intervention for chronic total occlusions (CTO) could lead to the key selection preference of metallic stents rather than bioresorbable vascular scaffolds (BVS) ...and then possibly deviate the outcome comparisons in real-world studies. By including recanalized CTO with true lumen tracking, we tested if any selection preference remained and compared the outcomes between everolimus-eluting stent (EES) and BVS implantation.Among 211 consecutive CTO interventions with true lumen tracking from August 2014 to April 2018 when BVS was available, we compared the clinical and interventional features between 28 patients with BVS and 77 patients with EES implantation. With propensity score matching and a median follow-up of 50.5 (37.3-60.3) months, we further assessed 25 patients with BVS and 25 with EES for target vessel failure (TVF: cardiac death, target vessel myocardial infarction, and target lesion revascularization).Multivariate analyses showed that BVS was still favored in the presence of LAD CTO (odds ratio (OR) = 3.4, 95% confidence interval (CI) = 1.0-11.7) and an average scaffold/stent size ≥ 3 mm (OR = 10.5, 95% CI = 3.0-37.3). EES was preferred for lesions with a J-CTO score ≥ 3 (OR = 19.3, 95% CI = 3.4-110.8) and multivessel intervention necessary at index procedure (OR = 11.3, 95% CI = 1.9-67.3). With matched comparisons, the TVF-free survival of EES was better than that of BVS for CTO recanalization (P = 0.049 by log-rank test) at long-term follow-up.Even with true lumen tracking techniques, selection bias remained substantial when determining either device for CTO implantation. The matched comparison of outcomes suggested the unfavorable longer-term impacts of the first generation of BVS on CTO lesions.
Several studies reported the efficacy of orthokeratology for myopia control. Somehow, there is limited publication with follow-up longer than 3 years. This study aims to research whether overnight ...orthokeratology influences the progression rate of the manifest refractive error of myopic children in a longer follow-up period (up to 12 years). And if changes in progression rate are found, to investigate the relationship between refractive changes and different baseline factors, including refraction error, wearing age and lens replacement frequency. In addition, this study collects long-term safety profile of overnight orthokeratology.
This is a retrospective study of sixty-six school-age children who received overnight orthokeratology correction between January 1998 and December 2013. Thirty-six subjects whose baseline age and refractive error matched with those in the orthokeratology group were selected to form control group. These subjects were followed up at least for 12 months. Manifest refractions, cycloplegic refractions, uncorrected and best-corrected visual acuities, power vector of astigmatism, corneal curvature, and lens replacement frequency were obtained for analysis.
Data of 203 eyes were derived from 66 orthokeratology subjects (31 males and 35 females) and 36 control subjects (22 males and 14 females) enrolled in this study. Their wearing ages ranged from 7 years to 16 years (mean ± SE, 11.72 ± 0.18 years). The follow-up time ranged from 1 year to 13 years (mean ± SE, 6.32 ± 0.15 years). At baseline, their myopia ranged from -0.5 D to -8.0 D (mean ± SE, -3.70 ± 0.12 D), and astigmatism ranged from 0 D to -3.0 D (mean ± SE, -0.55 ± 0.05 D). Comparing with control group, orthokeratology group had a significantly (p < 0.001) lower trend of refractive error change during the follow-up periods. According to the analysis results of GEE model, greater power of astigmatism was found to be associated with increased change of refractive error during follow-up years.
Overnight orthokeratology was effective in slowing myopia progression over a twelve-year follow-up period and demonstrated a clinically acceptable safety profile. Initial higher astigmatism power was found to be associated with increased change of refractive error during follow-up years.
The structures and absolute configurations of two organic compounds, excoecoumarin A and excoecoumarin B, were studied by recording their electronic circular dichroism (ECD) spectra and performing ...quantum‐chemistry computations. The optical rotations and the nuclear magnetic resonance (NMR), ECD, vibrational circular dichroism (VCD), and ultraviolet (UV) spectra of candidate structures were computed using the B3LYP approach of the density functional theory. The structures and absolute configurations of excoecoumarins A and B are inferred from the agreement between the experimental and theoretical NMR, ECD, and UV spectra. Although a specific configuration can be assigned to excoecoumarin A, three structures are possible for excoecoumarin B. The computations indicated that the absolute configuration of excoecoumarin B can be further identified by acquiring the VCD spectra. In any case, the present study indicated that the structures reported previously should be revised.

CONTEXT The occupational health literature has long been dominated by stress‐related topics. A more contemporary perspective suggests using a positive approach in ...the form of a health model focused on what is right with people, such as feelings of well‐being and satisfaction.
OBJECTIVES Using a positive perspective and multi‐source data collection, this study investigated the inter‐relationships among emotional intelligence (EI), patient satisfaction, doctor burnout and job satisfaction.
METHODS In this observational study, 110 internists and 2872 out‐patients were surveyed in face‐to‐face interviews.
RESULTS Higher self‐rated EI was significantly associated with less burnout (p < 0.001) and higher job satisfaction (p < 0.001). Higher patient satisfaction was correlated with less burnout (p < 0.01). Less burnout was found to be associated with higher job satisfaction (p < 0.001).
CONCLUSIONS This study identified EI as a factor in understanding doctors’ work‐related issues. Given the multi‐dimensional nature of EI, refinement of the definition of EI and the construct validity of EI as rated by others require further examination.
Speech enhancement (SE) aims to reduce noise in speech signals. Most SE techniques focus only on addressing audio information. In this paper, inspired by multimodal learning, which utilizes data from ...different modalities, and the recent success of convolutional neural networks (CNNs) in SE, we propose an audio-visual deep CNNs (AVDCNN) SE model, which incorporates audio and visual streams into a unified network model. We also propose a multitask learning framework for reconstructing audio and visual signals at the output layer. Precisely speaking, the proposed AVDCNN model is structured as an audio-visual encoder-decoder network, in which audio and visual data are first processed using individual CNNs, and then fused into a joint network to generate enhanced speech (the primary task) and reconstructed images (the secondary task) at the output layer. The model is trained in an end-to-end manner, and parameters are jointly learned through back propagation. We evaluate enhanced speech using five instrumental criteria. Results show that the AVDCNN model yields a notably superior performance compared with an audio-only CNN-based SE model and two conventional SE approaches, confirming the effectiveness of integrating visual information into the SE process. In addition, the AVDCNN model also outperforms an existing audio-visual SE model, confirming its capability of effectively combining audio and visual information in SE.
Dysregulation of histone demethylase Jumonji-C domain-containing protein 5 (JMJD5) has been identified as a great effect on tumorigenesis. Silibinin is a commonly used anti-hepatotoxic drug and ...exhibits anticancer effect in various cancers. However, the antitumor mechanism between silibinin and JMJD5 in oral squamous cell carcinoma (OSCC) remains unclear. In this study, the clinical significance of JMJD5 on OSCC patients was assessed through tissue microarray. Furthermore, mice bearing patient-derived tumor xenografts (PDTXs) and tongue cancer cell lines were treated with silibinin and evaluated for tumor growth and JMJD5 expression. High expression of JMJD5 in oral cancer was significantly associated with tumor size (P = 0.0241), cervical node metastasis (P = 0.0001) and clinical stage (P = 0.0002), was associated with worse survival rate compared with that of the total cohort (P = 0.0002). Collectively the data indicate that JMJD5 expression may be suitable for detection of unfavorable prognosis in OSCC patients, based in part on its apparent role as a marker of metastasis. In addition, silibinin inhibits cancer growth in vitro and in PDTX models. Furthermore, metastasis-associated protein 1 (MTA1) could regulate the expression for JMJD5 and had a positive correlation with JMJD5. Moreover, silibinin could downregulate JMJD5 and MTA1 in oral cancer. Present study thus identifies that JMJD5 might be an essential prognostic indicator and therapeutic target against OSCC progression. In addition, silibinin is a potential candidate among novel chemotherapeutic agents or adjuvants for modulating JMJD5 in OSCC, through a mechanism likely involving MTA1/JMJD5 axis.
Maintaining internal osmolality constancy is essential for life. Release of arginine vasopressin (AVP) in response to hyperosmolality is critical. Current hypotheses for osmolality sensors in ...circumventricular organs (CVOs) of the brain focus on mechanosensitive membrane proteins. The present study demonstrated that intracellular protein kinase WNK1 was involved. Focusing on vascular-organ-of-lamina-terminalis (OVLT) nuclei, we showed that WNK1 kinase was activated by water restriction. Neuron-specific conditional KO (cKO) of Wnk1 caused polyuria with decreased urine osmolality that persisted in water restriction and blunted water restriction-induced AVP release. Wnk1 cKO also blunted mannitol-induced AVP release but had no effect on osmotic thirst response. The role of WNK1 in the osmosensory neurons in CVOs was supported by neuronal pathway tracing. Hyperosmolality-induced increases in action potential firing in OVLT neurons was blunted by Wnk1 deletion or pharmacological WNK inhibitors. Knockdown of Kv3.1 channel in OVLT by shRNA reproduced the phenotypes. Thus, WNK1 in osmosensory neurons in CVOs detects extracellular hypertonicity and mediates the increase in AVP release by activating Kv3.1 and increasing action potential firing from osmosensory neurons.
We study a novel task, Video Question-Answer Generation (VQAG), for challenging Video Question Answering (Video QA) task in multimedia. Due to expensive data annotation costs, many widely used, ...large-scale Video QA datasets such as Video-QA, MSVD-QA and MSRVTT-QA are automatically annotated using Caption Question Generation (CapQG) which inputs captions instead of the video itself. As captions neither fully represent a video, nor are they always practically available, it is crucial to generate question-answer pairs based on a video via Video Question-Answer Generation (VQAG). Existing video-to-text (V2T) approaches, despite taking a video as the input, only generate a question alone. In this work, we propose a novel model Generator-Pretester Network that focuses on two components: (1) The Joint Question-Answer Generator (JQAG) which generates a question with its corresponding answer to allow Video Question "Answering" training. (2) The Pretester (PT) verifies a generated question by trying to answer it and checks the pretested answer with both the model's proposed answer and the ground truth answer. We evaluate our system with the only two available large-scale human-annotated Video QA datasets and achieves state-of-the-art question generation performances. Furthermore, using our generated QA pairs only on the Video QA task, we can surpass some supervised baselines. As a pre-training strategy, we outperform both CapQG and transfer learning approaches when employing semi-supervised (20%) or fully supervised learning with annotated data. These experimental results suggest the novel perspectives for Video QA training.
A metal nanoparticle composite, namely TPNT1, which contains Au-NP (1 ppm), Ag-NP (5 ppm), ZnO-NP (60 ppm) and ClO
(42.5 ppm) in aqueous solution was prepared and characterized by spectroscopy, ...transmission electron microscopy, dynamic light scattering analysis and potentiometric titration. Based on the in vitro cell-based assay, TPNT1 inhibited six major clades of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) with effective concentration within the range to be used as food additives. TPNT1 was shown to block viral entry by inhibiting the binding of SARS-CoV-2 spike proteins to the angiotensin-converting enzyme 2 (ACE2) receptor and to interfere with the syncytium formation. In addition, TPNT1 also effectively reduced the cytopathic effects induced by human (H1N1) and avian (H5N1) influenza viruses, including the wild-type and oseltamivir-resistant virus isolates. Together with previously demonstrated efficacy as antimicrobials, TPNT1 can block viral entry and inhibit or prevent viral infection to provide prophylactic effects against both SARS-CoV-2 and opportunistic infections.
