To evaluate biostatistics in scientific articles published in the European Annals of Otorhinolaryngology Head & Neck Diseases.
Retrospective analysis of 76 scientific articles published in 2018 and ...2019. The main goal was to describe the percentage of articles using biostatistics and to explore for potential associations with the article's main characteristics. Secondary goals were, within the group of articles using statistical tests, to describe the type of statistics used, the assessment of normality in case of comparison of mean values, the p-value threshold for significance, the use of confidence intervals, and power analysis.
Statistics were used in 73.7% of articles, without any significant association with main characteristics. Within the group of articles using statistics, the tests used were parametric, non-parametric and not specified in 77.7%, 51.4% and 1.8% of cases, respectively. Normality was checked in 14.2% of article using parametric tests to evaluate mean values. The p-value significance threshold was set at .05, .01, .005 levels and not defined in 60.7%, 1.8%, 1.8% and 35.7% of articles, respectively, while confidence intervals and power analyses were documented in 10.7% and 5.3%, respectively.
This article underlines the need for better use of statistics in the European Annals of Otorhinolaryngology Head & Neck Diseases, to improve the quality of scientific articles published in the organ of written expression of the French and International French-speaking Societies of Otorhinolaryngology, and to support the ongoing move toward better medical science.
Les cancers anaplasiques de la thyroïde (1–2 % des cancers de la thyroïde) sont de pronostic très défavorable. Au contraire, les cancers différenciés (≥80 % des cancers thyroïdiens) ont un pronostic ...favorable avec une chirurgie suivie ou non d’une irathérapie et la place de la radiothérapie y est anecdotique. Les cancers insulaires et médullaires sont de pronostic intermédiaire et sont rarement irradiés. Nous proposons des recommandations de radiothérapie dans ces différentes situations cliniques et ce à la lumière, notamment, des nouvelles techniques d’irradiation. Une recherche de la littérature française et anglaise a été réalisée avec les termes : thyroïde, cancer, anaplasique, différencié, insulaire, médullaire, radiothérapie, chirurgie. Une chirurgie non mutilante suivie systématiquement d’une radiothérapie est le standard pour les cancers anaplasiques (hyperfractionnée-accélérée avec doxorubicine et/ou non cisplatine si possible). La durée médiane de survie restant au plus de dix mois, une chimiothérapie néoadjuvante et/ou adjuvante peut aussi être discutée. La probabilité de survie à dix ans des patients atteints de cancer différencié dépasse 75 %. Les rares indications d’irradiation sont une résection incomplète pour maladie massive ne fixant pas l’iode. En plus d’un âge avancé (45 ou 60ans), certains variants histologiques (dont ceux à cellules hautes) et les cancers insulaires semblent être des indications croissantes de radiothérapie. Pour une maladie massive extracapsulaire, notamment ganglionnaire, des cancers médullaires peuvent justifier une radiothérapie. La morbidité de la radiothérapie a été longtemps un facteur limitant, mais la radiothérapie avec modulation d’intensité (RCMI) offre des avantages dosimétriques qui se traduisent par une probabilité de toxicité tardive de moins de 5 %. Le rôle de la radiothérapie évolue dans les cancers anaplasiques par l’utilisation de stratégies multimodalités utilisant de nouvelles molécules et combinaisons, tandis que des critères mineurs apparaissent comme devant être intégrés dans les arbres décisionnels pour la radiothérapie des cancers différenciés, avec la RCMI devenant un standard.
Anaplastic thyroid cancers represent 1–2% of all thyroid tumours and are of very poor prognosis even with multimodality treatment including external beam radiation therapy. Conversely, differentiated thyroid carcinomas (at least 80% of thyroid cancers) hamper good prognosis with surgery with or without radioiodine and there is hardly any room for external beam radiation therapy. Insular and medullar carcinomas have intermediary prognosis and are rarely irradiated. We aimed to update recommendations for external beam irradiation in these different clinical situations and put in light the benefits of new irradiations techniques. A search of the French and English literature was performed using the following keywords: thyroid carcinoma, anaplastic, chemoradiation, radiation therapy, surgery, histology and prognostic. Non-mutilating surgery (often limited to debulking) followed by systematic external beam radiation therapy is the standard of care in anaplastic thyroid cancers (hyperfractionated-accelerated radiation therapy with low-dose weekly doxorubicin with or without cisplatin if possible). Given anaplastic thyroid cancers’ median survival of 10 months or less, neoadjuvant and adjuvant chemotherapy may also be discussed. Ten-year survival rates for patients with papillary, follicular and Hürthle-cell carcinomas are 93%, 85%, and 76%, respectively. Massive primary incompletely resected iodine-negative disease indicates external beam radiation therapy. Older age (45 or 60-year-old), poor-prognosis histological variants (including tall cell cancers) and insular cancers are increasingly reported as criteria for external beam radiation therapy. Massive extracapsular incompletely resected nodal medullary disease suggests external beam radiation therapy. Radiation therapy morbidity has been an important limitation. However, intensity modulated radiation therapy (IMRT) offers clear dosimetric advantages on tumour coverage and organ sparing, reducing late toxicities to less than 5%. The role of radiation therapy is evolving for anaplastic thyroid cancers using multimodal strategies and new chemotherapy molecules, and for differentiated cancers using minor criteria, such as histological variants, with IMRT becoming a standard of care.
L’adénome pléomorphe parotidien est la tumeur bénigne la plus fréquente des glandes salivaires. La difficulté de sa prise en charge est liée à un risque de récidive élevé. Les complications de ...celles-ci sont, soit iatrogènes en cas de nécessité de reprises chirurgicales (paralysie faciale), soit intrinsèques, le risque de transformation maligne augmentant avec le nombre de rechutes locales. Cet article reprend les critères histologiques et radiologiques des récidives et revient sur les modalités chirurgicales de prise en charge. Pour améliorer le taux de contrôle local, la radiothérapie adjuvante (de première intention ou en situation de récidive) peut être délivrée mais reste à l’heure actuelle controversée : il s’agit d’une tumeur bénigne, dans une population jeune, et le risque de cancer radio-induit est redouté. Nous avons recensé les études où l’irradiation adjuvante était proposée afin d’essayer de mieux définir sa place dans les stratégies thérapeutiques ainsi que ses modalités. Des facteurs pronostiques ont été identifiés par certains auteurs. Les incertitudes demeurent, d’autres études sont nécessaires.
Parotid pleomorphic adenoma is the most frequent tumor of salivary glands. The prognosis depends on the recurrences because they could lead to iatrogenic events (facial paralysis). Moreover the risk of malignant transformation increases with the number of local relapses. This article aims at reviewing histological and radiological criteria and the surgical techniques. To improve local control, adjuvant irradiation (in first intention or after recurrence) may be useful but is still controversial for benign tumors in young patients with a risk of radio-induced cancer. We listed studies in which adjuvant radiotherapy was used so as to define its place in the treatment strategy. Prognostic factors were found by some authors. Other studies have to be done before strong evidence-based recommendations are issued.
L’objectif de cette revue de la littérature était d’évaluer les pratiques en cas de métastases cervicales ganglionnaires sans primitif retrouvé, en termes d’irradiation des muqueuses et ...ganglionnaires et de les discuter afin de définir une stratégie personnalisée de sélection des volumes à irradier. Une recherche de la littérature a été réalisée avec les mots clés suivants : adénopathie cervicale, primitif inconnu, cancer voies aérodigestives supérieures, radiothérapie, curage, carcinome épidermoïde. Les adénopathies cervicales (carcinomes épidermoïdes dans plus des deux tiers des cas) de primitif inconnu après confirmation diagnostique font l’objet d’un bilan étiologique, une scanographie, une TEP-scanographie au (18F)-fluorodésoxyglucose (FDG), une panendoscopie et une amygdalectomie. Dans un certain nombre de situations cliniques, les métastases cervicales ganglionnaires sans primitif retrouvé sont traitées par curage et irradiation ganglionnaire et muqueuse non sélective étendue du nasopharynx à l’hypopharynx afin de contrôler la maladie ganglionnaire et de prévenir l’émergence d’un cancer primitif métachrone à partir d’un cancer infraclinique lors du diagnostic d’adénopathie. Il pourrait être justifié du fait de la morbidité majeure de proposer une sélection des volumes cibles ganglionnaires et muqueux par radiothérapie conformationnelle avec modulation d’intensité plutôt qu’une irradiation étendue systématique sur la base de quatre critères décisionnels essentiels : le niveau ganglionnaire, le stade ganglionnaire, le statut selon les papillomavirus humains (HPV) et le virus d’Epstein-Barr (EBV), les données immuno-histochimiques avec les variants histologiques. Il ne s’agit pas de proposer une recommandation en l’absence de niveau de preuve suffisant mais de repositionner le problème de ces irradiations pan-muqueuses, associées à une importante toxicité, et en conséquence de proposer une redéfinition sélective personnalisée des volumes cibles. Ces critères de décision devront être évalués prospectivement au regard de l’indice thérapeutique obtenu.
The treatment of carcinomas of unknown primary revealed by cervical lymphadenopathy is based on neck dissection and nodal and pan-mucosal irradiation to control the neck and avoid the emergence of a metachronous primary. The aim of this review was to assess diagnostic and therapeutic approaches and criteria that may be used for a customized selective approach to avoid severe toxicities of pan-mucosal irradiation. A literature search was performed with the following keywords: cervical lymphadenopathy, unknown primary, upper aerodigestive tract, cancer, radiotherapy, squamous cell carcinoma, variants. The diagnostic workup includes a head and neck scanner or MRI, (18F)-FDG PET CT, a panendoscopy and tonsillectomy. Squamous cell carcinoma represents over two thirds of cases. The number of metastatic cervical nodes, nodal level, and histological variant (associated with HPV/EBV status) may determine the primary site origin and might be weighted for the determination of radiation target volumes on a multidisciplinary basis. A selective customized approach is relevant to decrease radiation toxicity only if neck and mucosal control is not impaired. Although no recommendation can yet be made in the absence of sufficient level of evidence, the relevance of systematic pan-mucosal irradiation appears questionable in a number of clinical situations. Accordingly, a customized selective redefinition of target volumes may be discussed and be prospectively evaluated in relation to the therapeutic index obtained.

A retrospective analysis was made of reviewers’ comments on ...1,133 scientific articles (700 original articles, 96 literature reviews, and 337 case reports), originating from 69 countries, consecutively submitted on-line between January 1st, 2020 and December 31st, 2021. The main objective was to document the acceptance rate and decision time. Accessory objectives were to synthesize the main comments and to screen for correlations between acceptance and the main characteristics of first authors, articles and reviewers’ comments.
In total, 4.1% of submitted articles were accepted. Median decision time differed significantly (P<0.0001), at 1 month in case of refusal and 4 months in case of acceptance. Reviewers mentioned failure to adhere to the journal's authors’ guide, to use the appropriate EQUATOR guidelines and to adopt the recommended P<0.005 significance threshold in 94.8%, 54.2%, and 39.9% of cases, respectively. On multivariate analysis, 3 variables significantly impacted acceptance, which increased from 1.3% to 44.6% (P<0.0001) when an appropriate EQUATOR guideline was used and from 0.3% to 57.4% (P<0.0001) when the significance threshold was set at P<0.005, and decreased from 10.5% to 1.1% (P=0.0001) when the article did not originate from a French-speaking country (member of the Francophonie organization).
Adhesion to modern scientific medical writing rules increased acceptance rates for articles in the European Annals of Otorhinolaryngology Head & Neck Diseases. Teaching modern scientific medical writing needs to be enhanced in otorhinolaryngology.
