Low back pain and pelvic girth pain are common in pregnancy and women commonly utilize complementary manual therapies such as massage, spinal manipulation, chiropractic, and osteopathy to manage ...their symptoms.
The aim of this systematically review was to critically appraise and synthesize the best available evidence regarding the effectiveness of manual therapies for managing pregnancy-related low back and pelvic pain.
Seven databases were searched from their inception until April 2015 for randomized controlled trials. Studies investigating the effectiveness of massage and chiropractic and osteopathic therapies were included. The study population was pregnant women of any age and at any time during the antenatal period. Study selection, data extraction, and assessment of risk of bias were conducted by 2 reviewers independently, using the Cochrane tool. Separate meta-analyses were conducted to compare manual therapies to different control interventions.
Out of 348 nonduplicate records, 11 articles reporting on 10 studies on a total of 1198 pregnant women were included in this meta-analysis. The therapeutic interventions predominantly involved massage and osteopathic manipulative therapy. Meta-analyses found positive effects for manual therapy on pain intensity when compared to usual care and relaxation but not when compared to sham interventions. Acceptability did not differ between manual therapy and usual care or sham interventions.
There is currently limited evidence to support the use of complementary manual therapies as an option for managing low back and pelvic pain during pregnancy. Considering the lack of effect compared to sham interventions, further high-quality research is needed to determine causal effects, the influence of the therapist on the perceived effectiveness of treatments, and adequate dose-response of complementary manual therapies on low back and pelvic pain outcomes during pregnancy.
It is critical to develop methods to quantify the early pharmacodynamic effects of targeted therapeutics in vivo to make drug development more efficient and ensure biologically relevant dosing. ...Furthermore, an ability to identify patients likely to respond to targeted therapeutics would decrease the size, duration, and cost of clinical trials, resulting in more efficient translation to improved patient outcomes. Recent studies suggest that perifosine inhibits the phosphatidylinositol-3'-kinase (PI3K) pathway by preventing cell membrane recruitment of the AKT pleckstrin homology domain.
A novel functional proteomics technology, reverse phase protein array, was used to establish and quantify pharmacodynamic markers of perifosine efficacy.
Perifosine selectively prevents AKT recruitment to the membrane and blocks activation of downstream effectors. Perifosine inhibited breast, ovarian, and prostate cancer models. Growth inhibition was associated with apoptosis. Activation of AKT as a consequence of genomic aberrations predicted perifosine efficacy. In cell lines and xenografts, there was a highly statistically significant correlation between the degree of antitumor efficacy of different perifosine doses and quantified down-regulation of phosphorylation of AKT and of its downstream targets, particularly S6.
Because of a strong correlation between proportional modulation of PI3K pathway biomarkers and quantified perifosine efficacy, it is likely that early measurement of such pharmacodynamic biomarkers with reverse phase protein array will optimize selection of responding patients and guide perifosine dosing. Furthermore, PI3K pathway activation status may allow baseline selection of patients most likely to respond to perifosine alone or in combination with other therapies.
We present the results of searches for gravitational waves from a large selection of pulsars using data from the most recent science runs (S6, VSR2 and VSR4) of the initial generation of ...interferometric gravitational wave detectors LIGO (Laser Interferometric Gravitational-wave Observatory) and Virgo. We do not see evidence for gravitational wave emission from any of the targeted sources but produce upper limits on the emission amplitude. We highlight the results from seven young pulsars with large spin-down luminosities. We reach within a factor of five of the canonical spin-down limit for all seven of these, whilst for the Crab and Vela pulsars we further surpass their spin-down limits. We present new or updated limits for 172 other pulsars (including both young and millisecond pulsars). Now that the detectors are undergoing major upgrades, and, for completeness, we bring together all of the most up-to-date results from all pulsars searched for during the operations of the first-generation LIGO, Virgo and GEO600 detectors. This gives a total of 195 pulsars including the most recent results described in this paper.
Characterization of a population of green turtles inhabiting the northeastern Gulf of Mexico was made possible by the mortality of a subset of >4500 sea turtles that stranded during a mass cold ...stunning event in Florida, USA, during January 2010. In total, 434 dead, stranded green turtlesChelonia mydaswere evaluated through necropsy and skeletochronological analysis to characterize morphology, sex, body condition, disease status, age structure, and growth patterns. Standard straightline carapace lengths ranged from 18.1 to 78.5 cm (mean ± SD = 36.3 ± 10.4 cm) and did not significantly differ from those of stranded green turtles that survived this event. Prevalence of fibropapilloma (FP) was low, at 6%, and sex ratio was significantly biased toward females (2.45F:1M). Age estimates ranged from 2 to 22 yr (mean ± SD = 9 ± 4 yr) and female age distribution was significantly greater than that of males. Mean stage durations, as calculated through summation of size class-specific growth rates and fitting smoothing spline models to length-at-age data, were similar and ranged from 17 to 20 yr. Generalized additive models and generalized additive mixed models were used to assess the potential influence of discrete and continuous covariates on growth rates. Somatic growth was significantly influenced by size, age, and calendar year; however, no effect of sex, FP status, or body condition was found. Increased understanding of population parameters will improve population models for the species and can also serve as a reference for assessing potential effects of the 2010 Deepwater Horizon oil spill in the Gulf of Mexico.
Patient-specific computational fluid dynamics (CFD) models permit analysis of complex intra-aortic hemodynamics in patients with aortic dissection (AD), where vessel morphology and disease severity ...are highly individualized. The simulated blood flow regime within these models is sensitive to the prescribed boundary conditions (BCs), so accurate BC selection is fundamental to achieve clinically relevant results.
This study presents a novel reduced-order computational framework for the iterative flow-based calibration of 3-Element Windkessel Model (3EWM) parameters to generate patient-specific BCs. These parameters were calibrated using time-resolved flow information derived from retrospective four-dimensional flow magnetic resonance imaging (4D Flow-MRI). For a healthy and dissected case, blood flow was then investigated numerically in a fully coupled zero dimensional-three dimensional (0D-3D) numerical framework, where the vessel geometries were reconstructed from medical images. Calibration of the 3EWM parameters was automated and required ~3.5 min per branch.
With prescription of the calibrated BCs, the computed near-wall hemodynamics (time-averaged wall shear stress, oscillatory shear index) and perfusion distribution were consistent with clinical measurements and previous literature, yielding physiologically relevant results. BC calibration was particularly important in the AD case, where the complex flow regime was captured only after BC calibration.
This calibration methodology can therefore be applied in clinical cases where branch flow rates are known, for example, via 4D Flow-MRI or ultrasound, to generate patient-specific BCs for CFD models. It is then possible to elucidate, on a case-by-case basis, the highly individualized hemodynamics which occur due to geometric variations in aortic pathology high spatiotemporal resolution through CFD.
Campus shutdowns during the SARS-CoV-2 pandemic posed unique challenges to faculty and students engaged in laboratory courses. Formerly hands-on experiments had to be quickly pivoted to emergency ...remote learning. While some resources existed prior to this period, many currently available online modules and/or simulations focus on a single technique. The Biochemistry Authentic Scientific Inquiry Lab (BASIL) curriculum has, for several years, provided a robust, linked, holistic inquiry experience that allows students to make connections between multiple techniques, both computational in nature as well as wet-lab-based. As a course-based undergraduate research experience (CURE), this flexible, module-based curriculum allows students to generate original hypotheses based on analysis of proteins of unknown function. We have taught this curriculum as the upper-level laboratory course on our campuses and were obliged to transition to remote instruction at various points in the course sequence. We report on the experiences of faculty and students over the transition period in this course. Additionally, we report as a case study results of one of our campus’ ongoing discipline-based education research (DBER) on the BASIL curriculum prior to and during remote delivery.
This study examined the risk of adverse outcomes during non-fatal encounters with subjects exhibiting features of Excited Delirium Syndrome (ExDS). Data for the study was collected over a five-year ...period through standardized reporting in a large Canadian law enforcement agency. Consistent with previous research, the presence of six or more of the ten features of ExDS was used to identify a probable case. Force was applied on 10,718 subjects, 197 (1.8%) of which were probable ExDS. Logistic regression were used to model the odds that use of force (UoF) interventions used on subjects in a state of probable ExDS resulted in adverse outcomes. Probable ExDS was one of the most important predictors of adverse outcomes in UoF encounters, even after controlling for associated risk factors. There were significantly higher odds that UoF was ineffective on subjects exhibiting more features of ExDS, resulting in an increased amount of force applied. In contrast, there were significantly lower odds of injury from UoF for individuals exhibiting probable ExDS. Officers, however, were at a higher risk of injury when dealing with those displaying a greater number of features. These results underscore the risks inherent to incidents involving cases of probable ExDS. A greater understanding of such risks may improve response strategies and promote public and police safety.
Colonic mucosa-associated Escherichia coli are increased in Crohn's disease (CD) and colorectal cancer (CRC). They variously haemagglutinate, invade epithelial cell lines, replicate within ...macrophages, translocate across M (microfold) cells and damage DNA. We investigated genes responsible for these effects and their co-association in colonic mucosal isolates.
A fosmid library yielding 968 clones was prepared in E coli EPI300-T1 using DNA from a haemagglutinating CRC isolate, and resulting haemagglutinating clones were 454-pyrosequenced. PCR screening was performed on 281 colonic E coli isolates from inflammatory bowel disease (IBD) (35 patients), CRC (21) and controls (24; sporadic polyps or irritable bowel syndrome).
454-Pyrosequencing of fosmids from the haemagglutinating clones (n=8) identified the afimbrial adhesin afa-1 operon. Transfection of afa-1 into E coli K-12 predictably conferred diffuse adherence plus invasion of HEp-2 and I-407 epithelial cells, and upregulation of vascular endothelial growth factor. E coli expressing afaC were common in CRC (14/21, p=0.0009) and CD (9/14, p=0.005) but not ulcerative colitis (UC; 8/21) compared with controls (4/24). E coli expressing both afaC and lpfA (relevant to M-cell translocation) were common in CD (8/14, p=0.0019) and CRC (14/21, p=0.0001), but not UC (6/21) compared with controls (2/24). E coli expressing both afaC and pks (genotoxic) were common in CRC (11/21, p=0.0015) and UC (8/21, p=0.022), but not CD (4/14) compared with controls (2/24). All isolates expressed dsbA and htrA relevant to intra-macrophage replication, and 242/281 expressed fimH encoding type-1 fimbrial adhesin.
IBD and CRC commonly have colonic mucosal E coli that express genes that confer properties relevant to pathogenesis including M-cell translocation, angiogenesis and genotoxicity.

This article examines comparative homicide rates in the United States and Western Europe in an era of increasingly globalized neo-liberal economics. The main finding of this preliminary analysis is ...that historical and spatial correlations between distinct forms of political economy and homicide rates are consistent enough to suggest that social democratic regimes are more successful at fostering the socio-cultural conditions necessary for reduced homicide rates. Thus Western Europe and all continents and nations should approach the importation of American neo-liberal economic policies with extreme caution. The article concludes by suggesting that the indirect but crucial causal connection between political economy and homicide rates, prematurely pushed into the background of criminological thought during the ‘cultural turn’, should be returned to the foreground.
