The concept of nerve-magnified regeneration, part of which is end-to-side nerve transfer, provides a theoretical basis for the treatment of high ulnar nerve injuries. High injury to the ulnar nerve ...causes severe consequences, such as loss of sensation, loss of movement and “claw hand” deformity, affecting the fine motor skills of the hand. A method of nerve-magnified regeneration – distal nerve transfer – interosseous anterior (median nerve) to the deep ulnar nerve at the wrist level is presented, used in 20 patients (2 to 66 years of age) with an average follow-up period of 3 years (10 months to 5 years). This is a microsurgical method for rapid re-innervation of the most distal hand muscles in proximal neurotmesis of ulnar nerve.
Abstract-Injury to the triangular fibrocartilage complex (TFCC) can lead to instability of the distal radioulnar joint (DRUJ). In this article, we have presented a treatment method for patients with ...chronic traumatic TFCC injury. The surgical technique described here demonstrates anatomical reconstruction of the volar and dorsal distal radioulnar ligament using an autologous palmaris longus tendon graft.In the period 2014-2023, 22 patients with chronic traumatic injury to the triangular fibrocartilage complex, leading to instability of the DRUJ, were operated on in our clinic. The main parameters studied pre- and post-operatively were: degree of pain during active movements in the wrist joint and prono-supination of the forearm,range of active movements in the same joints, stability of DRUJ and grip strength relative to the contralateral limb.All 22 patients reported pain reduction with active wrist movements and forearm prono-supination. This surgical technique with anatomically based reconstruction of the distal volar and dorsal radioulnar ligament provides restoration of stability of the distal radioulnar joint and reduction of pain in patients with chronic injury to the triangular fibrocartilage complex.
Objective: To clarify the indications for the choice of operative intervention in patients with SLAC and SNAC wrist and to compare the functional results after proximal carpectomy (PRC) and partial ...intracarpal arthrodesis with scaphoid excision. Methods: A total of 20 patients underwent four corner fusion intracarpal arthrodesis with scaphoid excision, and 8 patients underwent proximal carpectomy in the period from 2016 to 2023. The mean postoperative follow-up period for patients with intracarpal arthrodesis was 3.2 years and for patients with proximal carpectomy was 3.5 years. Results: Regarding range of motion of the wrist in the flexion-extension axis, the proximal carpectomy patients in our series showed better results than the intracarpal arthrodesis patients, an average of 84% in the PRC patients and 75% in the intracarpal arthrodesis patients relative to the contralateral limb. In the patients with intracarpal arthrodesis, however, we recorded a greater preserved range of motion in the radial-ulnar deviation axis of the wrist. Grip strength was measured at 6 months postoperatively and averaged 72% for the PRC group compared with 83% for patients with intracarpal arthrodesis to the contralateral limb. Conclusion: Both proximal carpectomy and intracarpal arthrodesis are surgical interventions that achieve similar functional outcomes for the treatment of degenerative conditions of the wrist (SLAC/SNAC).
This analysis of how Bulgaria coped with socialist modernization in the field of housing takes the concept of "consumption regime" as its point of departure. Applying the lens of "regulated ...consumption" (as opposed to a laissez- faire regime) allows a differentiation between state care and state control. Bulgarian housing policy relied on the notion of the state as the main actor. The article argues that this state did not contradict traditional values in Bulgarian society but made concerted efforts to meet the essential wishes of the people vis-à-vis their own home. During the late 1940s and 1950s, this tradition was reflected in the way construction was organized as well as in the types of housing provided. By contrast, in the time of socialist large-scale slab building (from the 1960s to 1980s) this tradition was reinterpreted to cover favorable conditions of loans for housing-in the first place for building second houses (villas). The regulation of housing supply through the use of tradition explains the very high percentage of private housing in Bulgaria after the fall of socialism. Housing in socialist Bulgaria corroborates the argument that the appropriation of tradition is an important factor in the domestication of socialist modernization.

The centrality of housing is beyond any doubt. Housing is a basic component of people's quality of life and the single biggest investment that most families will make. Social housing is a term widely ...used in housing policy literature. While appropriate for a welfare country, this term was ideologically unacceptable for the communist regimes as it was associated with inequality, poverty and homelessness. For this reason all East European countries after the Second World War preferred the terms public housing or socialist housing.
