The aim of this pilot study was to demonstrate the potential of simultaneously acquired 68-Gallium-DOTA-D-Phe1-Tyr3-octreotide (68Ga-DOTATOC) positron emission tomography/magnetic resonance imaging ...(PET/MRI) in comparison with 68Ga-DOTATOC PET/computed tomography (PET/CT) in patients with known gastroenteropancreatic neuroendocrine tumors (NETs).
Eight patients (4 women and 4 men; mean SD age, 54 17 years; median, 55 years; range 25-74 years) with histopathologically confirmed NET and scheduled 68Ga-DOTATOC PET/CT were prospectively enrolled for an additional integrated PET/MRI scan. Positron emission tomography/computed tomography was performed using a triple-phase contrast-enhanced full-dose protocol. Positron emission tomography/magnetic resonance imaging encompassed a diagnostic, contrast-enhanced whole-body MRI protocol. Two readers separately analyzed the PET/CT and PET/MRI data sets including their subscans in random order regarding lesion localization, count, and characterization on a 4-point ordinal scale (0, not visible; 1, benign; 2, indeterminate; and 3, malignant). In addition, each lesion was rated in consensus on a binary scale (allowing for benign/malignant only). Clinical imaging, existing prior examinations, and histopathology (if available) served as the standard of reference. In PET-positive lesions, the standardized uptake value (SUV max) was measured in consensus. A descriptive, case-oriented data analysis was performed, including determination of frequencies and percentages in detection of malignant, benign, and indeterminate lesions in connection to their localization. In addition, percentages in detection by a singular modality (such as PET, CT, or MRI) were calculated. Interobserver variability was calculated (Cohen's κ). The SUVs in the lesions in PET/CT and PET/MRI were measured, and the correlation coefficient (Pearson, 2-tailed) was calculated.
According to the reference standard, 5 of the 8 patients had malignant NET lesions at the time of the examination. A total of 4 patients were correctly identified by PET/CT, with the PET and CT component correctly identifying 3 patients each. All 5 patients positive for NET disease were correctly identified by PET/MRI, with the MRI subscan identifying all 5 patients and the PET subscan identifying 3 patients. All lesions considered as malignant in PET/CT were equally depicted in and considered using PET/MRI. One liver lesion rated as "indetermined" in PET/CT was identified as metastasis in PET/MRI because of a diffusion restriction in diffusion-weighted imaging. Of the 4 lung lesions characterized in PET/CT, only 1 was depicted in PET/MRI. Of the 3 lymph nodes depicted in PET/CT, only 1 was characterized in PET/MRI. Interobserver reliability was equally very good in PET/CT (κ = 0.916) and PET/MRI (κ = 1.0). The SUV max measured in PET/CT and in PET/MRI showed a strong correlation (Pearson correlation coefficient, 0.996).
This pilot study demonstrates the potential of 68Ga-DOTATOC PET/MRI in patients with gastroenteropancreatic NET, with special advantages in the characterization of abdominal lesions yet certain weaknesses inherent to MRI, such as lung metastases and hypersclerotic bone lesions.
Selective Internal Radiation Therapy (SIRT) with 90yttrium (Y-90) is an increasingly used therapeutic option for unresectable liver malignancies. Nontarget embolization of extrahepatic tissue ...secondary to vascular shunting can lead to SIRT associated complications. Our aim was to assess whether extrahepatic shunts can reliably be diagnosed based on hepatic digital subtraction angiography (DSA) or whether subsequent SPECT/CT data can provide additional information.
825 patients with hepatocellular carcinoma (n = 636), hepatic metastases (n = 158) or cholangiocellular carcinoma (n = 31) were retrospectively analyzed. During hepatic DSA 128 arteries causing shunt flow to gastrointestinal tissue were coilembolized (right gastric artery n = 63, gastroduodenal artery n = 29; branches to duodenum / pancreas n = 36). Technectium-99m-labeled human serum albumin (HSA) was injected in all 825 patients. SPECT/CT data was used to identify additional or remaining shunts to extrahepatic tissue.
An unexpected uptake of HSA in extrahepatic tissue was found by SPECT/CT in 54/825 (6.5%) patients (located in stomach n = 13, duodenum n = 26, distal bowel segments n = 12, kidney n = 1, diaphragm n = 2). These patients underwent repeated DSA and newly identified shunt vessels were coilembolized in 22/54 patients, while in 12/54 patients a more distal catheter position for repeat injection of HSA was chosen. In 20/54 patients the repeated SPECT/CT data still revealed an extrahepatic HSA uptake. These patients did not receive SIRT.
Most extrahepatic shunts can be identified on DSA prior to Y-90 therapy. However, SPECT-CT data helps to identify additional shunts that were initially not seen on DSA.
To evaluate the added value of the application of the liver-specific contrast phase of Gadobenate dimeglumine (Gd-BOPTA) for detection and characterization of liver lesions in 18F-FDG PET/MRI.
41 ...patients with histologically confirmed solid tumors and known / suspected liver metastases or not classifiable lesions in 18F-FDG PET/CT were included in this study. All patients underwent a subsequent Gd-BOPTA enhanced 18F-FDG PET/MRI examination. MRI without liver-specific contrast phase (MRI1), MRI with liver-specific contrast phase (MRI2), 18F-FDG PET/MRI without liver-specific contrast phase (PET/MRI1) and with liver-specific contrast phase (PET/MRI2) were separately evaluated for suspect lesions regarding lesion dignity, characterization, conspicuity and confidence.
PET/MRI datasets enabled correct identification of 18/18 patients with malignant lesions; MRI datasets correctly identified 17/18 patients. On a lesion-based analysis PET/MRI2 provided highest accuracy for differentiation of lesions into malignant and benign lesions of 98% and 100%. Respective values were 95% and 100% for PET/MRI1, 93% and 96% for MRI2 and 91% and 93% for MRI1. Statistically significant higher diagnostic confidence was found for PET/MRI2 and MRI2 datasets compared to PET/MRI1 and MRI1, respectively (p < 0.001).
The application of the liver-specific contrast phase in 18F-FDG PET/MRI further increases the diagnostic accuracy and diagnostic confidence for correct assessment of benign and malignant liver lesions.
The purposes of this study were to evaluate the feasibility of simultaneous 18F choline positron emission tomography (PET) and magnetic resonance imaging (MRI) of the prostate and to present the ...first clinical results of the method.
From March 2012 to October 2012, a total of 15 consecutive patients were examined with simultaneous 18F choline PET/MRI. At the time of the examination, 8 patients had histologically proven prostate cancer, 2 patients had repeated prostate biopsies with negative results, and 5 patients had suspected prostate cancer with an elevated or rising prostate specific antigene level but did not have a prostate biopsy. Sequence protocol comprised T2-weighted high-resolution images and diffusion-weighted images of the prostate in addition to PET imaging. Image quality was assessed by 2 radiologists, and the PET images were evaluated qualitatively and quantitatively.
Simultaneous PET/MRI of the prostate was accomplished successfully in all patients. The method proved to be robust without technical failure, and the image quality was rated to be diagnostic in all examinations except in 1 diffusion-weighted imaging (DWI) data set that was judged to be nondiagnostic because of susceptibility artifacts. High-resolution T2-weighted images allowed exact correlation of elevated focal or diffuse choline uptake to suspicious T2-weighted lesions of the prostate. A high accordance was found between PET and DWI. However, PET-positive lesions were found in 3 patients wherein DWI did not indicate tumor in suspicious T2-weighted lesions.
Simultaneous positron emission tomography/magnetic resonance imaging of the prostate has the advantage of combining high-resolution prostate images, functional studies, and metabolic/molecular imaging. The PET component adds diagnostic confidence to the MRI-based parameters in identifying and localizing tumor in the prostate.
Aim of the study was to assess the feasibility and to compare three non-enhanced T1-weighted (w) sequences for liver vessel imaging at 7 Tesla (T).
12 healthy volunteers were examined on a 7 T ...whole-body MR-system. The following non-enhanced sequences were acquired: T1w 2D FLASH, T1w 3D FLASH and Time of flight (TOF)-MRA. Qualitative image analysis was performed by two radiologists including over all image quality as well as vessel delineation of the liver arteries, liver veins and portal vein and the presence of artifacts using a five-point scale (5 = excellent vessel delineation to 1 = non-diagnostic). Contrast ratios (CR), SNR und CNR of the above named vessels in correlation to adjacent liver tissue were calculated for quantitative assessment. For statistical analysis, a Wilcoxon Rank Test was applied.
All three sequences provided a homogenous hyperintense delineation of the assessed liver vessels. Qualitative image analysis demonstrated the superiority of TOF-MRA, providing best overall image quality (TOF 4.17, 2D FLASH 3.42, 3D FLASH 3.46; p<0.01) as well as highest image quality values for all analyzed liver vessel segments. TOF-MRA was least impaired by B1 inhomogeneity (4.13) and susceptibility artifacts (4.63) out of all three sequences (p<0.01). Quantitative image analysis confirmed the superiority of TOF MRA showing significant higher CR values for all liver vessels (e.g. right hepatic artery TOF 0.47, 2D FLASH 0.09, 3D FLASH 0.11 with p = 0.02 and 0.01, respectively). Providing the lowest standard deviation in noise, TOF showed highest values for SNR and CNR.
Non-enhanced T1w imaging in general and TOF MRA in particular, appear to be promising techniques for high quality non-enhanced liver vessel assessment at 7 T.
The objective of this study was to evaluate whether the use of automatically generated maps of bowel motility (motility mapping) in magnetic resonance imaging (MRI) leads to an increased detection ...rate of inflammatory bowel disease.
Fifty consecutive patients with suspected or known inflammatory bowel disease who underwent bowel MRI using a 1.5-T scanner were analyzed retrospectively. In addition to standard small bowel magnetic resonance protocol, a dynamic coronal T2-weighted sequence (dynamic MRI) was acquired. Dynamic sequences were used to automatically generate a parametric map depicting bowel motility. Two readings of the MRI were performed: first, evaluation of static MRI alone and second, evaluation of static MRI combined with dynamic MRI (motility mapping). Static MRI was analyzed on parameters defining inflammation (morphology, enhancement). Dynamic MRI (motility mapping) was evaluated on the basis of a color-coded scheme displaying hypomotility and hypermotility.
Using motility maps, additional inflammatory lesions were found in 13 (26%) of the 50 patients, resulting in a significantly higher detection rate using static MRI together with motility mapping compared with static MRI alone (P = 0.0002). Overall, 66 inflammatory lesions of the bowel were detected in a total of 38 patients (static MRI + motility mapping) versus 51 lesions in 34 patients (sole evaluation of static MRI).
Motility assessment of the bowel provides additional information and improves the detection of inflammatory lesions in MRI.
Diffusion-weighted imaging (DWI) is routinely used in magnetic resonance imaging (MRI) of prostate cancer. However, the routine use of b values higher than 1000 s/mm(2) is not clear up to present. ...Moreover, the complex diffusion behavior of malignant and benign prostate tissues hampers precise predictions of contrast in DWI images and apparent diffusion coefficient (ADC) maps.
To quantitatively analyze DWI with different b values in prostate cancer and to identify b values best suitable for cancer detection.
Forty-one patients with histologically proven prostate cancer were examined with high resolution T2-weighted imaging and DWI at 3 Tesla. Five different b values (0, 800, 1000, 1500, 2000 s/mm(2)) were applied. ADC values of tumors and reference areas were measured on ADC maps derived from different pairs of b values. Furthermore, signal intensities of DW images of tumors and reference areas were measured. For analysis, contrast ratios of ADC values and signal intensities of DW images were calculated and compared.
No significant differences were found between contrast ratios measured on ADC maps of all analyzed b value pairs (P = 0.43). Contrast ratios calculated from signal intensities of DW images were highest at b values of 1500 and 2000 s/mm(2) and differed significantly from contrast ratios at b values of 800 and 1000 s/mm(2) (P < 0.01).
Whereas contrast in ADC maps does not significantly change with different b values, contrast ratios of DW images are significantly higher at b-values of 1500 and 2000 s/mm(2) in comparison to b values of 800 and 1000 s/mm(2). Therefore, diagnostic performance of DWI in prostate cancer might be increased by application of b values higher than 1000 s/mm(2).
The purpose of this study was to assess oral contrast agents, volumes of the agents, and time points of data acquisition in regard to small-bowel distention and patient acceptance.
Six healthy ...volunteers underwent imaging on 16 different days. Four volumes (450, 900, 1,350, and 1,800 mL) of each of the four contrast compounds (0.2% locust bean gum plus 2.5% mannitol, VoLumen containing 2.0% sorbitol, VoLumen containing 1.4% sorbitol, and tap water) were used. Two-dimensional true fast imaging with steady-state free precession data sets were acquired at 5-minute intervals after contrast ingestion. Distention values for small-bowel segments (duodenum, proximal and distal jejunum, ileum) and occurrence of side effects were documented.
Analysis of bowel distention revealed significantly greater distention for all carbohydrate sugar alcohol-containing solutions compared with water but no significant difference among the three contrast agents. Sufficient duodenal distention was achieved with 900 mL of any of the contrast agents, but imaging had to be performed soon after ingestion. For MRI of the distal jejunum and ileum, a volume of 1,350 mL is preferable, and the time point of data acquisition plays a minor role. Ingestion of 1,800 mL of the carbohydrate sugar alcohol solutions led to a significantly higher rate of side effects such as abdominal cramps than did ingestion of smaller volumes.
The data indicate that sufficient contrast consumption and optimal timing of data acquisition are essential to distention of the small bowel. Oral contrast agent protocols should be adapted to the bowel region in question.
To evaluate a potential correlation of the maximum standard uptake value (SUVmax) and the minimum apparent diffusion coefficient (ADCmin) in primary and recurrent cervical cancer based on integrated ...PET/MRI examinations.
19 consecutive patients (mean age 51.6 years; range 30-72 years) with histopathologically confirmed primary cervical cancer (n = 9) or suspected tumor recurrence (n = 10) were prospectively enrolled for an integrated PET/MRI examination. Two radiologists performed a consensus reading in random order, using a dedicated post-processing software. Polygonal regions of interest (ROI) covering the entire tumor lesions were drawn into PET/MR images to assess SUVmax and into ADC parameter maps to determine ADCmin values. Pearson's correlation coefficients were calculated to assess a potential correlation between the mean values of ADCmin and SUVmax.
In 15 out of 19 patients cervical cancer lesions (n = 12) or lymph node metastases (n = 42) were detected. Mean SUVmax (12.5 ± 6.5) and ADCmin (644.5 ± 179.7 × 10(-5) mm2/s) values for all assessed tumor lesions showed a significant but weak inverse correlation (R = -0.342, p < 0.05). When subdivided in primary and recurrent tumors, primary tumors and associated primary lymph node metastases revealed a significant and strong inverse correlation between SUVmax and ADCmin (R = -0.692, p < 0.001), whereas recurrent cancer lesions did not show a significant correlation.
These initial results of this emerging hybrid imaging technique demonstrate the high diagnostic potential of simultaneous PET/MR imaging for the assessment of functional biomarkers, revealing a significant and strong correlation of tumor metabolism and higher cellularity in cervical cancer lesions.
