A total of 37,860 households from the 2017 Household Income and Expenditure Survey (HIES) were included in the study. Data on dental care utilization, age, gender and education attainment of the head of household, socioeconomic status of households, health insurance coverage, living areas and provinces were obtained for the survey. The concentration curve and the normalized concentration index (C
) was used to illustrate and quantify socioeconomic inequalities in dental care utilization among Iranian households. The C
was decomposed to identify the main determinants of the observed socioeconomic inequality in dental care utilization in Iran.
The study indicated that the prevalence of dental care utilization among Iranian's households was 4.67% (95% confidence interval CI: 4.46 to 4.88%). The results suggested a higher concentration of dental care utilization among socioeconomically advantaged households (C
= 0.2522; 95% CI: 0.2258 to 0.2791) in Iran. Pro-rich inequality in dental care utilization also found in rural (C
= 0.2659; 95%CI: 0.2221 to 0.3098) and urban (C
= 0.0.2504; 95% CI: 0.0.2159 to 0.2841) areas. The results revealed socioeconomic status of households, age and education status of head of households and residing provinces as the main contributing factors to the concentration of dental care utilization among the wealthy households.
This study revealed pro-rich inequalities in dental care utilization among households in Iran and its provinces. Thus, health policymakers should focus on designing effective evidence-based interventions to improve healthcare utilization among household with the older head of households, lower education status, and living in relatively poor provinces to reduce socioeconomic inequality in dental care utilization in Iran.
Abstract Diabetes mellitus (DM) is among the most widespread non-communicable diseases and poses a substantial global health challenge. The aim of this study was to examine the incidence of DM and ...its nutritional, anthropometric, laboratory, demographic, and behavioral determinants, as well as comorbidities, within a Kurdish population residing in western Iran. This research was conducted in the Ravansar Non-Communicable Disease (RaNCD) cohort study, followed 9170 participants aged 35–65 years, for an average ± SD of 7.11 ± 1.26 years, from 2015 until 2023. A hierarchical Cox regression model was used to estimates the adjusted hazard ratios (HRs). The incidence of DM was 4.45 (95% CI 3.96, 4.99) per 1000 person-years. We found several significant predictors for DM incidence, including prediabetes, comorbidity, urban residence, total antioxidant capacity (TAC), and the interaction between gender and body mass index (BMI). Prediabetes emerged as the strongest predictor of DM incidence, with a hazard ratio of 10.13 (CI 7.84, 13.09). Additionally, having two diseases (HR = 2.18; 95% CI 1.44, 3.29) or three and more diseases (HR = 3.17; 95% CI 2.06, 4.90) increased the risk of developing DM. Also, the hazard ratios for the effects of gender on DM incidence in the normal, overweight, and obese BMI groups were 0.24, 0.81, and 1.01, respectively. The presence of prediabetes and obesity serve as the crucial indicators for the onset of DM, emphasizing the pressing need for interventions to prevent DM in these circumstances. Furthermore, there are notable disparities between urban and rural populations in this study, warranting further investigations to ascertain the underlying causes of such variations.

In this population-based cross-sectional study, we recruited 1057 Iranian adults between August 2013 and February 2015, in Tehran, a multi-ethnic city. Participants were selected using random digit dialing. Three separate logistic regression models were applied to estimate the adjusted odds ratios (95% CI).
Exposure to second-hand smoking was the most prevalent smoking type (37, 95% CI: 35-41%) followed by cigarette (23.9% (95% CI: 21-27%)) and water-pipe smoking (20.25% (95% CI: 18-23%)) in adults in Tehran. Almost 3.3 and 4.5% of adults reported three and two types of lifetime smoking behaviors, respectively. Age, sex, history of depression along with lifetime alcohol intake was the important predictors of all three types of smoking. Lifetime alcohol consumption was associated with increased prevalence of all three types of smoking (p for trend < 0.009). Lifetime drug abuse was also associated with increased prevalence of cigarette smoking (OR = 2.04, 95% CI: 1.61-2.59, p < 0.001).
Lifetime prevalence of waterpipe, cigarette and exposure to second-hand smoking is moderately high. Dual smoking behaviors are increasing in Iranian adults. An apparent age-related difference in pattern of waterpipe and cigarette smoking was observed. These findings highlight the need for further educational and preventive programs especially for dual smoking in Iranian young adults. This could provide practical information for evaluating and reforming the tobacco control programs and policies in Iran.
Polycystic ovary syndrome (PCOS) is one of the most frequent endocrine disorders among women of fertile age. Women with PCOS manifest clinical symptoms like menstrual dysfunction, hirsutism, insulin ...resistance, and hyperinsulinemia. As excessive amounts of insulin levels directly increase ovarian production of androgens, hyperinsulinemia and insulin resistance are considered as the pathogenesis factors of PCOS. The portfolio low-carbohydrate diet (PLCD) is a plant-based diet with 40% carbohydrates combined with five cholesterol-lowering foods and nutrients. On the other hand, the ketogenic diet (KD) is a nutritional protocol with 10% carbohydrates. The purpose of this study is to determine whether PLCD or KD is more effective in alleviating PCOS symptoms.
Forty-six overweight or obese women diagnosed with PCOS will be randomly stratified to receive either PLCD or KD for 8 weeks. Measures related to anthropometric and body composition, glucose, and insulin level, HOMA-IR, sex hormones, lipid profile, quality of life, dietary intake, physical activity, and Ferriman-Gallwey score of all participants will be accessed before and after the intervention.
Since the first line treatment of PCOS is lifestyle adjustment including diet control and exercise, there has not been determined the optimal diet for this population of women yet. Hence, the goal of conducting this study is to determine whether the PLCD or the KD could have more advantageous effects on attenuating PCOS manifestations. The result of this investigation will give us new insight into curing this disease and will provide evidence-based recommendations for prescribing an optimal diet for PCOS women.

Metabolic syndrome (MetS) is associated to sleep duration. It is crucial to identify factors that disrupt sleep regulation. The study aimed to assess the indirect effect of risk factors related to ...MetS severity through sleep duration by utilizing a structural equation model (SEM).
The study involving 3,935 adults from the baseline data of the Ravansar Non-Communicable Disease (RaNCD) cohort study. MetS severity scores were the outcome variables. SEM was employed to explore the relationships, utilizing IBM SPSS and AMOS version 23.
The mean MetS severity score was higher in women compared to men (0.25 vs. 0.16, P = 0.003). In men, socioeconomic status (SES) has a positive direct effect (β = 0.048) and a negative indirect effect (β=-0.006) on MetS severity. Increased physical activity is directly (β=-0.036) and indirectly (β=-0.093) associated with reducing MetS severity. Nap duration is directly linked to an increase (β = 0.072) but has an indirect effect (β=-0.008) in decreasing MetS severity. In women, SES has a direct (β=-0.020) and indirect (β=-0.001) inverse relationship with MetS severity. Increased physical activity is directly (β=-0.048) and indirectly (β=-0.036) associated with decreasing MetS severity in women. Nap duration is directly associated with an increase in MetS severity (β=-0.018) but indirectly contributes to its reduction (β=-0.002). Sleep duration not only directly affects MetS severity but is also influenced by age, SES, physical activity, obesity and nap duration.
Physical activity, SES, and nap duration directly and indirectly effect the MetS severity. Sleep duration was recognized as a mediating variable that supports the indirect effects.
Antisocial traits have severe outcomes for the perpetrator, victim, and society. Developing an assessment tool for antisocial traits that is theoretically grounded, has strong psychometric ...properties, and can be administered in the general population is very important for the identification and treatment of the problems associated with antisocial traits. The initial item pool was generated as the result of an extensive literature review based on criteria of Diagnostic and Statistical Manual of Mental Disorders (5th ed.; DSM-5). Items were examined through exploratory factor analysis in a sample of 2,051 residents of Tehran. The structural validity was tested on a separate 2,049 people. The final version explains 58.4% of the total variation of the construct and included 20 questions loaded on six factors. The resultant Antisocial Traits Scale (ASTS-20) has the potential to be a useful measure for early detection of at-risk people who should be targeted by preventive interventions aimed at reducing the likelihood of criminality or recidivism.

The overweight and obesity among military personnel, as an occupational group, beside the health issues, might affect their military performance. A systematic review and meta-analysis were conducted ...to estimate the pool prevalence of overweight and obesity among Iranian military personnel.
The national databases including Science Information Database, MagIran, and the following international databases; Web of Science, Medline via PubMed, and Scopus were searched, up to December 2017, for relevant published studies without time limitation.
Totally,1431 studies were reterived and 10 studies included in meta-analysis. The pooled prevalence of overweight and obesity were 41% (95%CI:26, 57%) and 13% (95%CI:10, 17%), respectively. In the subgroup analyses, a lower and higher prevalence of overweight was reported in the ground (12%) and Navy (69%) forces military, recpectively. For the obesity, the air forces had the lowest prevalence (11%) and the ground and Navy forces military had highest prevalence (15%). The prevalence of overweight and obesity were slightly higher in studies conducted after 2014.
Our findings suggest a high prevalence of overweight and obesity in the military personnel as a high-risk occupational group. Owing to the high observed heterogeneity among the included studies, large representative studies are needed to estimate the prevalence of overweight and obesity in the military personnel.
