Introduction
Patients with inborn errors of immunity (IEI) have a compromised or inappropriate immune response. Although they might be considered a high-risk group for severe SARS-CoV-2 infection, ...the reported impact of COVID-19 in these patients has been reassuring, while the differential susceptibility of distinct types of IEI remains unclear.
Objective
We aimed to describe the findings and outcomes of our known patients with IEI who were diagnosed with COVID-19.
Methods
In a retrospective study from March 2020 to February 2021, four centers in Mexico collected clinical, laboratory, and genetic data from pediatric and adult patients with known diagnoses of IEI who presented with COVID-19, based on compatible symptoms and positive SARS-CoV-2 testing or known household exposure.
Results
We report 31 patients with known IEI from Mexico who presented with SARS-CoV-2 infection. Seventy-four percent were male, 52% were pediatric, and 81% survived. Their ages ranged from 5 months to 56 years, with a median of 17 years. Sixty-five percent had predominant antibody deficiencies, 48% were hospitalized, and 26% required ICU. Pediatric patients had a higher hospital admission rate than adults. Inpatient mortality was 40%, and ICU mortality rate was 63%. Forty-eight percent developed pneumonia, while 36% had evidence of hyperinflammation (4 adults and 7 children). Predominant laboratory features were lymphopenia and thrombocytopenia, seen in 70 and 44% of patients, respectively. The serum D-dimer median value was 2.6 (0.5–20.6) μg/mL, and the median highest ferritin value was 1015 (32–10,303) ng/mL. Intravenous immunoglobulin was used in 80% of patients. Other treatments included macrolides (39%) and corticosteroids (29%). Six patients died from secondary infection or uncontrolled systemic inflammation.
Discussion
Although impaired immunity due to IEI may be a predisposing factor for severe COVID-19, most of our patients with IEI who acquired the SARS-CoV-2 infection developed a well-tolerated infection and survived, as have more than 80% of worldwide reported patients to date. An impaired immune or inflammatory response may be a predisposing factor for some and a protective factor for others. A systematic review of the literature could help identify those patients at risk of severe disease and complications. Healthcare-associated infections should be aggressively prevented.
El estudio de las aves selváticas terrestres es aún incipiente en la alta Amazonía ecuatoriana. Por ello, este trabajo buscó describir la riqueza de especies y actividad diaria de Tinamiformes, ...Galliformes y Columbiformes de hábitos más terrestres, mediante el uso de cámaras trampa en el Bosque Protector de la Comunidad Pablo López de Oglán Alto, provincia de Pastaza. Se emplearon 20 cámaras instaladas desde febrero a julio del año 2020. Mediante un esfuerzo de 3317 trampas/noche se registraron 7 especies de los tres órdenes. Las especies con mayor abundancia relativa fueron: la paloma perdiz rojiza Geotrygon montana (Índice de Abundancia Relativa IAR=47,92), el pavón nocturno Nothocrax urumutum (IAR=33,96), la pava de Spix Penelope jacquacu (IAR=9,81) y el tinamú grande Tinamus major (IAR=4,53). Las especies mayormente activas en la mañana son G. montana, P. jacquacu y T. major. Se encontró un alto coeficiente de traslape entre los patrones de actividad de N. urumutum con P. jacquacu. Se confirma que el fototrampeo es una técnica útil para estudiar a especies de Galliformes, Tinamiformes y Columbiformes muy elusivas, que no se detectan con facilidad por observación directa. Con el uso de esta técnica se abre una posibilidad de incrementar mayor información útil para la conservación de estas especies de aves amazónicas amenazadas.
El virus H1N1 es mutante de la influenza humana, porcina y aviar; ha generado 4 pandemias en los últimos 100 años, la gravedad e incertidumbre para diagnosticarlos hace especial a estos pacientes, ...muchos de ellos deben manejarse en cuidados intensivos, principalmente por fallo respiratorio.
Presentamos 58 pacientes con sospecha de neumonía por H1N1, manejados en cuidados intensivos, pero solo en 11 pacientes se confirmó el diagnóstico.
Se evidenció una diferencia significativa en la mediana de la estancia en cuidados intensivos (10 días vs. 3,5 días; p=0,00), cociente de presión arterial de oxígeno/fracción inspirada de oxígeno (114,1 vs. 79; p=0,013), APACHE II (17 vs. 25; p=0,001) y tiempo de síntomas previos a la consulta (8 días vs. 5 días; p=0,007), al comparar el grupo sobrevivientes con fallecidos, respectivamente. El grupo con diagnóstico de H1N1 presentó una mediana significativamente más baja del cociente presión arterial de oxígeno/fracción inspirada de oxígeno, al ingresar a cuidados intensivos (74,3 vs. 112,1; p=0,009).
El grupo de pacientes con fallo respiratorio que mueren, presentan un mayor puntaje APACHE II; menor tiempo desde el inicio de los síntomas y la consulta, menor relación PaO2/FiO2, menor tiempo de estancia en la UCI y su procedencia es del área rural. Los pacientes admitidos en la UCI con fallo respiratorio y diagnóstico de H1N1, presentan un cociente de presión arterial de oxígeno/fracción inspirada de oxígeno al ingreso, significativamente menor que en los que no se confirma el diagnóstico.
H1N1 virus is a mutant of human, swine, and avian influenza. It has generated 4 pandemics in the last century. The seriousness and uncertainty of the diagnosis makes these patients special, many of whom have to be admitted to the ICU, mainly due to respiratory failure.
The cases are presented on 58 pneumonia patients suspected of H1N1 admitted to the ICU. H1N1 diagnosis was confirmed in only 11 of the patients.
There was a significant difference in the median stay in the ICU (10 days vs. 3.5 days, P=.00), partial pressure arterial oxygen (PaO2)/fraction inspired oxygen (FiO2) ratio (114.1 vs. 79 P=.013), APACHE II (17 vs. 25, P=.001), and time without symptoms prior to consultation (8 days vs. 5 days, P=.007) when comparing the number of deceased with the number of survivors. The groups with H1N1 diagnosis showed a significantly lower median of PaO2/FiO2 (74.3 vs. 112.1, P=.009) in the ICU.
The group of patients with respiratory failure who died showed a higher APACHE II score, less time lapse between the onset of symptoms and consultation, a lower PaO2/FiO2 ratio, a lower stay in the ICU, and they came from rural areas. The patients admitted to the ICU for respiratory failure and a confirmed H1N1 diagnosis show a much lower PaO2/FiO2 ratio than the patients without a confirmed H1N1 diagnosis.

Ensayos clínicos aleatorios que comparen el uso de ventilación mecánica no invasiva, con oxigenoterapia estándar, para disminuir la falla ventilatoria postextubación, reintubación, estancia en UCI, estancia en hospitalización, mortalidad en UCI, mortalidad en hospitalización o mortalidad a los 90 días.
Tres evaluadores independientemente extrajeron los datos y evaluaron la calidad de acuerdo a los criterios previstos; se realizó el análisis estadístico con el programa RevMan 5.3.
Se identificaron 7 ensayos clínicos de moderada a buena calidad metodológica, con 969 pacientes en total. Comparada con oxigenoterapia estándar, la ventilación mecánica no invasiva reduce la reintubación (OR: 0,43; IC 95%: 0,25-0,76; I2: 0%); disminuye la mortalidad en UCI (OR: 0,29; IC 95%: 0,13-0,67; I2: 0%); y en hospitalización (OR: 0,54; IC 95%: 0,29-1; I2: 0%), en especial en pacientes con factores de riesgo para falla ventilatoria postextubación.
El uso profiláctico de ventilación mecánica no invasiva reduce la reintubación, mortalidad en UCI y en hospitalización, en especial en pacientes con factores de riesgo identificados para falla respiratoria postextubación.
The use of prophylactic non-invasive mechanical ventilation after extubation has been increasing, but the evidence for its use is not yet clear.
To evaluate the efficacy of prophylactic non-invasive mechanical ventilation after extubation compared with standard oxygen therapy in patients with invasive mechanical ventilation and who overcome a spontaneous breathing trial.
Databases including PubMed, Embase, and LILACS (August 2014) were searched for pertinent studies without language restrictions.
Randomized trials of prophylactic non-invasive mechanical ventilation after extubation compared with standard oxygen therapy in patients with invasive mechanical were included. Effects on primary outcomes: reduce post-extubation respiratory failure, reintubation rate, ICU and/or hospital mortality, and hospital length of stay were accessed in this meta-analysis.
Three review authors independently assessed trial quality and abstracted data according to pre-specified criteria. The program RevMan 5.3 was used for analysis.
Seven trials with moderate to good quality involving 969 patients were included.
The use of NIV following extubation for patients decrease the reintubation rate (odds risk OR 0.43, 95% confidence interval CI 0.25-0.76, I2 0%); reduces ICU mortality (OR 0.29, 95% CI 0.13-0.67, I2 0%), and reduces hospital mortality (OR 0.54, 95% CI 0.29-1.0, I2 0%) compared to standard oxygen therapy, particularly in patients with risk factors associated with failed extubation.
The results of this review suggested prophylactic non-invasive mechanical ventilation after extubation seems to be effective in reducing reintubation rate, and ICU and/or hospital mortality. This is most important in patients with risk factors associated with failed extubation.

Identificamos 5 estudios aleatorizados (n = 4.360) con una adecuada calidad metodológica para combinar en el metaanálisis. La mortalidad a 90 días para tratamiento intensivo vs. conservador presentó un RR de 0,98; IC95%: 0,82-1,16; p = 0.94, I2 = 0% por el modelo de efectos fijos y aleatorio. La discapacidad para tratamiento intensivo vs. conservador a los 90 días presentó un RR de 1,0; IC95%: 0,93-1,08; p = 0,90, I2 = 0% por el modelo de efectos fijos y aleatorio. El aumento del volumen del hematoma mayor del 30% en manejo intensivo vs. conservador reveló un RR de 0,87; IC95%: 0,69-1,10; p = 0,15, I2 = 41% por el modelo de efectos aleatorios.
En los pacientes con HIC el manejo dirigido hacia el control estricto de la presión arterial no impacta en forma favorable la mortalidad, la discapacidad o el cambio en el volumen del hematoma.
The aim of this study was to compare intensive (systolic BP < 140 mmHg) versus conventional blood pressure management (systolic BP < 180 mmHg), in patients with acute intracerebral haemorrhage; our clinical outcomes were: mortality, disability, and haematoma expansion more than 30% at 24hours.
We performed a systematic review and meta-analysis. A search was made in Cochrane central, MEDLINE, and EMBASE databases up 20 July 2016. High-quality randomised controlled trials with intensive blood pressure monitoring versus conventional therapy were chosen.
Five high-quality randomised controlled trials with 4360 patients were included in the Meta-Analyses. The mortality at 90 days showed a RR: 0.98; 95% CI: 0.82-1.16; P=.94, I2= 0%. The disability showed a RR: 1.0; 95% CI: 0.93-1.08; P=.90, I2= 0%. The hematoma expansion more than 30% at 24hours showed a RR: 0.87; 95% CI0.69-1.10; P=.15, I2= 41%.
In patients with acute intracerebral haemorrhage, intensive blood pressure management or conventional therapy could be similar when the outcomes measured are: mortality at 90 days, disability (Rankin scale), and haematoma expansion.

La escasez de agua amenaza el desarrollo la calidad de vida y la salud de los habitantes del mundo. El fenómeno de cambio climático ha acelerado exponencialmente el problema de sequias prolongadas, ...periodos de lluvia más cortos o lluvias intensas. La necesidad de captar la lluvia y utilizarla para su consumo, aumenta la presión sobre los países para que implementen prácticas más sostenibles e innovadoras. El objetivo general de este trabajo consistió en estudiar la composición química de agua de lluvia colectada durante el periodo 2015-2019 en el Observatorio Atmosférico de la UNAM (Ciudad Universitaria UNAM, Ciudad de México) y en el Observatorio Atmosférico Altzomoni (Parque Nacional Izta-Popo, Estado de México), pertenecientes a la Red Universitaria de Observatorios Atmosféricos (RUOA) del Instituto de Ciencias de la Atmósfera y Cambio Climático. Se obtuvo el factor de enriquecimiento (FE) utilizando, como valor de referencia análisis químicos de relación metal/Al y ión/Ca, obtenidos a partir de la concentración promedio ponderada de los metales analizados. Los FE altos sugirieron que, en general, los metales tienen un origen antropogénico ligado al cambio de uso de suelo y a la deforestación indiscriminada de sitios boscosos adyacentes a los sitios de estudio. Las concentraciones de Al, Cd, Fe, Pb y V fueron consistentes con una contribución antropogénica importante, debido probablemente al transporte desde diferentes ciudades del país, y a la contribución intrínseca de cada sitio de muestreo. El estudio de la persistencia, la presencia y la permanencia de metales pesados en agua de lluvia es necesario para promover proyectos actuales y futuros sobre su tratamiento y uso.
According to the WHO, low birth weight (LBW) affects 15–20% of newborns worldwide. In Mexico, there are no national, state, nor municipal estimates that inform the country’s situation over time. The ...purpose of this study was to estimate the incidence of LBW at the national, state, and municipal levels from 2008 to 2017, and to estimate the LBW incidence based on maternal sociodemographic characteristics, prenatal care and marginalization indexes at the national level using open national data. We used spatial data analysis to georeferenced LBW incidence at the three levels of geographical disaggregation studied. At the national level, the incidence of LBW increased progressively from 6.2% (2008) to 7.1% (2017), and the country’s capital represented the area with the highest incidence. Southeastern and central states reported the highest LBW regional incidence. At the municipal level, the number of municipalities with an incidence of LBW ≥8% increased in both male and female newborns. The incidence of LBW was higher as the marginalization indexes increases. The results from this study may assist in the identification of vulnerable groups and the development of public health programs and policies with an intersectoral approach that improves maternal and child nutrition.
Due to cartilage’s limited capacity for regeneration, numerous studies have been conducted to find new drugs that modify osteoarthrosis’s progression. Some evidence showed the capability of chitosan ...nanoparticles with glutathione (Np-GSH) to regulate the oxide-redox status in vitro in human chondrocytes. This work aimed to evaluate the capacity of Np-GSH in vivo, using Wistar rats with induced surgical osteoarthritis. Radiographic, biochemical (GSH and TBARS quantification), histopathological, and immunohistochemical (Col-2 and MMP-13) analyses were performed to evaluate the progress of the osteoarthritic lesions after the administration of a single dose of Np-GSH. According to the results obtained, the GSH contained in the NPs could be vectored to chondrocytes and used by the cell to modulate the oxidative state reduction, decreasing the production of ROS and free radicals induced by agents oxidizing xenobiotics, increasing GSH levels, as well as the activity of GPx, and decreasing lipid peroxidation. These results are significant since the synthesis of GSH develops exclusively in the cell cytoplasm, and its quantity under an oxidation–reduction imbalance may be defective. Therefore, the results allow us to consider these nanostructures as a helpful study tool to reduce the damage associated with oxidative stress in various diseases such as osteoarthritis.
