We report a case of Listeria meningitis related to mantle cell lymphoma. A clinical pharmacist adjusted repeatedly the patient’s anti-infective therapeutic regimen by analyzing the pharmacologic and ...pharmacokinetic characteristics of antibacterial drugs (such as cefotaxime, meropenem, etc.) due to the patient’s repeated fever during hospitalization. To the best of our knowledge, this is the first case of Listeria meningitis related to mantle cell lymphoma treated successfully with meropenem reported in China. This case aims to optimize the anti-infection treatment regimen of Listeria meningitis and to provide a reference for clinicians and clinical pharmacists to use drugs rationally.
Model reference adaptive control takes the output of a reference model as the target and ensures that the actual output is consistent with the target. In this scenario, the reference model determines ...the performance of the control system. Constructing a reference model with specific performance is challenging, and most researchers construct the reference model by experience. As alternative frameworks, we propose two novel reference model construction methods, namely, zero-pole method and frequency response method. The zero-pole method establishes a nonlinear optimization model to ensure that the added zeros and poles are as far away from the imaginary axis as possible. In the case where this cannot be done, the frequency response method matches the system frequency response of the reference model and the given model to the extent possible using a nonlinear optimization model. Experimental results are presented to confirm that both methods can construct a reference model based on a given transfer function. Deviations in response between the reference models constructed using the two methods and the given model are compared, and the reasons for higher accuracy of the frequency response method are discussed.
Studies investigating the relationship between maternal passive smoking and the risk of preterm birth have reached inconsistent conclusions. A birth cohort study that included 10,095 nonsmoking women ...who delivered a singleton live birth was carried out in Lanzhou, China, between 2010 and 2012. Exposure to passive smoking during pregnancy was associated with an increased risk of very preterm birth (<32 completed weeks of gestation; odds ratio = 1.98, 95% confidence interval: 1.41, 2.76) but not moderate preterm birth (32-36 completed weeks of gestation; odds ratio = 0.98, 95% confidence interval: 0.81, 1.19). Risk of very preterm birth increased with the duration of exposure (P for trend = 0.0014). There was no variability in exposures by trimester. The associations were consistent for both medically indicated and spontaneous preterm births. Overall, our findings support a positive association between passive smoking and the risk of very preterm birth.

Background. We attempt to discuss the function of self-made prescription compound Rhodiola rosea on the kidneys of rats with type 2 diabetic nephropathy (DN), through studying the effects of ...self-made rescription compound Rhodiola rosea on the ultrastructure of podocytes in rats with DN. Methods. DN rat model was established by streptozotocin. The successfully modeled rats were divided into 4 groups, DN group, compound Rhodiola low-dose group, medium-dose group, and high-dose group. Compound Rhodiola low-dose group, medium-dose group, and high-dose group were administered for 8 weeks, and the DN group and the blank control group were administered with normal saline. The podocyte count, podocyte width, podocyte fusion rate, and average thickness of glomerular basement membrane were compared in each group, and the ultrastructural changes in podocytes were observed by transmission electron microscope. Results. Compared with the normal control group, the number of podocytes in the DN group remarkably reduced, but the width level of podocyte, the fusion rate of podocyte, and the average thickness of basilar membrane remarkably increased (P<0.05). Compared with the DN group, the number of podocytes in the high-, medium-, and low-dose groups increased remarkably, but the width level of podocyte, the fusion rate of podocyte, and the average thickness of basilar membrane decreased remarkably (P<0.05). Compared with the low-dose group, the number of podocytes in the high-dose group and the medium-dose group increased remarkably, but the width of podocyte, the fusion rate of podocyte, as well as the average thickness of basilar membrane remarkably reduced (P<0.05). Various indicators of high- and medium-dose groups had no statistical difference (P>0.05). Conclusions. Self-made prescription compound Rhodiola rosea can promote the recovery of podocyte in DN rat and protect their kidney.

A clinical data of 212 patients with malignant tumor around the knee joint underwent limb salvage surgery between January 2008 and December 2017 were retrospectively analyzed. Among them, 14 cases had infection after limb salvage surgery. Two cases of acute infection were treated with sensitive antibiotics; 12 cases of chronic infection were treated with debridement and antibiotic bone cement occupying device implantation in the first stage, and prosthesis revision (8 cases), knee joint fusion (2 cases), or amputation (2 cases) in the second stage after infection control. The aim of this study was to investigate the geographic distribution of β-globin gene mutations in different ethnic groups in Yunnan province.
From 2004 to 2014, 1,441 subjects with hemoglobin ...disorders, identified by PCR-reverse dot blot and DNA sequencing, were studied according to ethnicity and geographic origin. Haplotypes were examined among 41 unrelated thalassemia chromosomes.
Eighteen β-thalassemia mutations and seven hemoglobin variants were identified for 1,616 alleles in 22 different ethnic groups from all 16 prefecture-level divisions of Yunnan. The prevalence of β-thalassemia was heterogeneous and regionally specific. CD 41-42 (-TCTT) was the most prevalent mutation in the populations of northeastern Yunnan. CD 17 (A>T) was the most common mutation in the populations of southeastern Yunnan, especially for the Zhuang minority, whereas Hb E (CD 26, G>A) was the most prevalent mutation in populations of southwestern Yunnan, especially for the Dai minority. Among the seven types of haplotypes identified, CD 17 (A>T) was mainly linked to haplotype VII (+ - - - - - +) and IVS-II-654 (C>T) was only linked to haplotype I (+ - - - - + +).
Our data underline the heterogeneity of β-globin gene mutations in Yunnan. This distribution of β-globin mutations in the geographic regions and ethnic populations provided a detailed ethnic basis and evolutionary view of humans in southern China, which will be beneficial for genetic counseling and prevention strategies.
The aim of this study was to investigate the overall distribution of pregnancy outcomes in women with elevated second-trimester maternal serum alpha-fetoprotein (MS-AFP), and to determine the risk of ...adverse pregnancy outcomes (APOs) by MS-AFP level.
We retrospectively analyzed the clinical data of 429 women with elevated MS-AFP (≥2.5 multiple of the median (MOM)) and 1555 women with normal MS-AFP (0.5–2.49MOM) from a total of 46,741 prenatally screened singleton pregnant women. The overall distribution of APOs of the two groups, the risk of APOs by MS-AFP level, and the predictive value of elevated MS-AFP to APOs were analyzed.
The incidence rate of APOs in elevated MS-AFP group was significantly higher than that in normal MS-AFP group (42.89 vs. 8.23%). In elevated MS-AFP group, the top three APOs, in term of incidence rate, were structural fetal abnormalities (7.93%), spontaneous abortion (7.46%) and preterm birth (7.23%); regarding to the risk, the top three APOs were stillbirth, spontaneous abortion and early-onset preeclampsia (odds ratio 35.98, 20.81 and 8.58 respectively). For structural fetal abnormalities, MS-AFP had predictive values for fetal open neural tube defects (ONTDs), gastroschisis and multiple malformations.
Elevated MS-AFP is associated with increased risks of APOs. ONTDs complicate merely a small proportion of pregnancies with elevated MS-AFP, and the rest of them have high risks of obstetric complications. MS-AFP can help to identify these women at high risk of APOs in earlier second-trimester.
