La acidosis tubular renal distal (ATRD) es una enfermedad rara que se debe al fallo del proceso normal de acidificación de la orina a nivel tubular distal y colector. Se caracteriza por una acidosis ...metabólica hiperclorémica persistente, con anión gap normal en plasma, en presencia de un pH urinario elevado y baja excreción urinaria de amonio.
Se han descrito hasta el momento 5 genes cuyas mutaciones dan lugar a ATRD primaria. Las alteraciones de los genes ATP6V1B1 y ATP6V0A4 se heredan de forma recesiva y están asociadas a formas de inicio más precoces y con sordera neurosensorial en muchos casos. Las variantes patogénicas en el gen SLC4A1 se heredan habitualmente de forma dominante y dan lugar a cuadros más leves, con un diagnóstico más tardío y alteraciones electrolíticas menores. Sin embargo, la evolución a nefrocalcinosis y litiasis, y el desarrollo de enfermedad renal crónica a medio-largo plazo se ha descrito de forma similar en estos 3grupos. Por último, se han descrito también formas recesivas de ATRD asociadas a mutaciones en los genes FOXI1 y WDR72.
El manejo clínico de la ATRD se basa en sales de bicarbonato o citrato, que no logran corregir en todos los casos las alteraciones metabólicas descritas y, por lo tanto, las consecuencias asociadas a ellas. Recientemente, un nuevo tratamiento basado en sales de bicarbonato y citrato de liberación prolongada ha recibido la denominación de medicamento huérfano en Europa para el tratamiento de la ATRD.
Distal renal tubular acidosis (DRTA) is a rare disease resulting from a failure in the normal urine acidification process at the distal tubule and collecting duct level. It is characterised by persistent hyperchloremic metabolic acidosis, with a normal anion gap in plasma, in the presence of high urinary pH and low urinary excretion of ammonium.
To date, 5 genes whose mutations give rise to primary DRTA have been described. Alterations in the ATP6V1B1 and ATP6V0A4 genes are inherited recessively and are associated with forms of early onset and, in many cases, with neurosensorial deafness. Pathogenic variants in the SLC4A1 gene are habitually inherited dominantly and give rise to milder symptoms, with a later diagnosis and milder electrolytic alterations. Nonetheless, evolution to nephrocalcinosis and lithiasis, and the development of chronic kidney disease in the medium to long term has been described in a similar manner in all 3groups. Lastly, recessive forms of DTRA associated to mutations in the FOXI1 and WDR72 genes have also been described.
The clinical management of DTRA is based on bicarbonate or citrate salts, which do not succeed in correcting all cases of the metabolic alterations described and, thus, the consequences associated with them. Recently, a new treatment based on slow-release bicarbonate and citrate salts has received the designation of orphan drug in Europe for the treatment of DTRA.
El complejo de esclerosis tuberosa es un desorden neurocutáneo autosómico dominante causado por mutaciones en los genes TSC1 o TSC2. El diagnóstico se basa en criterios clínicos o el criterio ...genético. La presentación clínica es altamente variable y las manifestaciones de la enfermedad pueden desarrollarse durante toda la vida. Se reporta el caso de un niño que cumple criterios clínicos para el diagnóstico de esclerosis tuberosa y cuyo estudio molecular identificó una variante nueva del gen TSC2. Se trata de una mutación sin sentido, esporádica, no reportada previamente (c.583_586dupATCG) localizada en el exón 6, que provoca un codón de parada temprano y altera la estructura de la proteína. Puede considerarse una variante patogénica por el tipo de mutación y permite ampliar el espectro de variantes del gen TSC2 como causa del complejo de esclerosis tuberosa.
La polykystose rénale récessive (PKR) est due à des mutations du gène PKHD1. L’objectif de l’étude est de caractériser la variabilité phénotypique en fonction des différentes mutations de PKHD1.
...L’étude a été réalisée chez 308 patients atteints de PKR, diagnostiqués dans notre laboratoire de génétique. Les médecins prescripteurs des analyses génétiques avaient fourni des données cliniques et biologiques minimales.
Les patients ont été divisés en trois groupes génotypiques : le premier groupe (G1 ; n=65) concernait les patients ayant deux mutations tronquantes, le second groupe (G2 ; n=117) des patients avec une mutation tronquante et une mutation non tronquante, et le troisième groupe (G3 ; n=126) était constitué des patients ayant deux mutations non tronquantes. Au total, 31 % des grossesses ont fait l’objet d’une interruption médicale de grossesse, 18 % des enfants sont décédés en période néonatale, et 51 % des patients ont survécu après la période néonatale ; la proportion des patients ayant un phénotype sévère (défini par une interruption médicale de grossesse ou un décès néonatal) était significativement plus élevée dans le groupe G1 : 94 %, par rapport aux groupes G2 (47 %) et G3 (27 % ; p<0,001).
La présence de deux mutations tronquantes du gène PKHD1 est associée à un phénotype périnatal et post-natal sévère. Toutefois, la variabilité phénotypique des autres groupes génotypiques nécessite une discussion au cas par cas lors du diagnostic anténatal.
ARPKD is a recessive rare disease due to PKHD1 mutation. The main objective of the study was to characterize the phenotypic variability according to the different types of PKHD1 mutations.
This study was performed in 308 ARPKD patients with a genetic diagnosis from our genetic center. Related physicians provided minimal clinical and biological data.
Patients were divided into three genotypic groups: the first group (G1; n=65) consisted of patients with two truncating mutations, the second group (G2; n=117) of patients with one truncating and one non-truncating mutation, and the third group (G3; n=126) of patients with two non-truncating mutations. In the entire cohort, the outcomes consisted of 31% of pregnancy termination, 18% of neonatal deaths and 51% of patient survival after the neonatal period. The proportion of severe ARPKD (pregnancy termination or neonatal death) was significantly greater in G1: 94% versus 47% in G2 and 27% in G3 (P<0.001).
The presence of two truncating mutations in PKHD1 is associated with the most severe perinatal phenotype. However, the phenotypic variability observed in the other genotypic groups requires caution for prenatal counseling.
Lesch-Nyhan disease and its attenuated variants are caused by mutations in the HPRT1 gene, which encodes the purine recycling enzyme hypoxanthine-guanine phosphoribosyltransferase. The mutations are ...heterogeneous, with more than 400 different mutations already documented. Prior efforts to correlate variations in the clinical phenotype with different mutations have suggested that milder phenotypes typically are associated with mutants that permit some residual enzyme function, whereas the most severe phenotype is associated with null mutants. However, multiple exceptions to this concept have been reported. In the current studies 44 HPRT1 mutations associated with a wide spectrum of clinical phenotypes were reconstructed by site-directed mutagenesis, the mutant enzymes were expressed in vitro and purified, and their kinetic properties were examined toward their substrates hypoxanthine, guanine, and phosphoribosylpyrophosphate. The results provide strong evidence for a correlation between disease severity and residual catalytic activity of the enzyme (kcat) toward each of its substrates as well as several mechanisms that result in exceptions to this correlation. There was no correlation between disease severity and the affinity of the enzyme for its substrates (Km). These studies provide a valuable model for understanding general principles of genotype-phenotype correlations in human disease, as the mechanisms involved are applicable to many other disorders.

Thema des Beitrags ist eine aktuelle Übersicht über die Aufgaben des Pathologen beim Magenkarzinom insbesondere im Hinblick auf therapeutische Entscheidungen.
Methode
Der Artikel basiert auf selektiven Literaturrecherchen v. a. zu klinisch-therapeutisch relevanten Fragestellungen, mit denen der Pathologe aktuell beim Magenkarzinom konfrontiert ist. Verwendet wurden verschiedene Datenbanken (PubMed, WHO).
Schlussfolgerungen
Moderne Sequenziermethoden erlaubten in jüngster Vergangenheit eine molekularbasierte Einteilung der Magenkarzinome. Diese Ergebnisse verdeutlichen, dass sich klassische morphologisch-definierte Entitäten molekular-definierten Gruppen zuordnen lassen und dass diese Subtypisierung eine Leitstruktur bei der Auswahl zu bestimmender prädiktiver Biomarker darstellt.
Le diagnostic de myopathie facioscapulo-humérale de type 1 (FSHD1) repose sur un examen clinique évocateur et la présence d’une contraction pathogène des unités répétées (UR) dans la portion ...sub-télomérique du chromosome 4 (4q35).
Trois grandes formes peuvent être distinguées : la forme classique associée à des délétions de quatre à sept UR de sévérité variable, la forme sévère infantile débutant avant dix ans associée à des délétions d’une à trois UR et la forme dite bénigne, associée à des délétions de huit à dix UR. La présence d’une atteinte sélective asymétrique des muscles faciaux et scapulaires fait fortement évoquer le diagnostic de FSHD1. Lorsque ce diagnostic est suspecté, la biologie moléculaire doit être réalisée en première intention. En l’absence de confirmation génétique, des examens complémentaires tels que l’ENMG et la biopsie musculaire doivent être réalisés à la recherche d’une autre étiologie. Si aucune autre étiologie n’est retenue, le diagnostic de FSHD2 doit être envisagé.
Le Southern Blot classiquement utilisé pour le diagnostic moléculaire peut être d’interprétation délicate. La technique de peignage moléculaire permet une visualisation directe et complète de la région D4Z4 et améliore la rentabilité du diagnostic moléculaire de la FSHD1.
Diagnosis of facioscapulohumeral dystrophy type 1 (FSHD1) is supported by a suggestive clinical presentation and associated with a heterozygous contraction of the D4Z4 repeat array on chromosome 4q35.
The FSHD1 phenotype has a widely variable course with great inter- and intrafamilial heterogeneity. Three clinical forms can be distinguished: the classical phenotype associated with four to seven repeat units (RU) and a variable course, a severe infantile form with one to three RU, and a mild phenotype associated with borderline UR (8 to 10 RU). At the molecular level, for D4Z4 contraction to be pathogenic, it needs to occur on a specific chromosomal background, namely on the 4qA allelic variant of chromosome 4. In most cases, once FSHD is clinically suspected, the diagnosis can be genetically confirmed with a DNA test using Southern Blotting and hybridization to a set of probes. However, diagnosis of FSHD1 remains challenging. Firstly, some patients may present with an atypical phenotype with highly focal or unusual symptoms. Secondly, there are potential pitfalls in the genetic diagnosis of FSHD resulting in false positive or false negative results. In the absence of genetic confirmation, other investigations, mainly EMG and muscle biopsy, are needed to rule out another diagnosis. In cases with no clear diagnosis and a permissive chromosome without contraction, FSHD2 may be suspected.
Molecular combing is a new technique which permits visualization and sizing of the D4Z4 repeat array on its genetic background on stretched single DNA fibers by fluorescence microscopy. This tool will improve genetic diagnosis in FSHD patients.
Diagnosis of FSHD1 is mainly supported by clinical features. Clinicians need to be aware of unusual presentations of this disease. The wide spectrum of intrafamilial variability and the lack of good correlation between genotype and phenotype present challenges for genetic counseling and prognostication. More studies are needed concerning penetrance and genotype–phenotype correlation.

Ichthyosen gehören zu einer klinisch und genetisch heterogenen Gruppe von Erkrankungen, die durch generalisierte Schuppung und/oder Hyperkeratosen der Haut charakterisiert sind. Seit ...der überwiegend klinisch orientierten Konsensus-Klassifikation von Sorèze werden diese Verhornungsstörungen als „Mendelian disorders of cornification“ (MEDOC) zusammengefasst und in syndromale sowie nichtsyndromale Ichthyosen unterteilt. Abgesehen von seltenen erworbenen Fällen liegen den Ichthyosen Gendefekte zugrunde, deren Genprodukte an der Differenzierung und der Barrierefunktion der Epidermis beteiligt sind. Zurzeit sind Mutationen in etwa 40 verschiedenen Genen bekannt, die zu den unterschiedlichen Formen der Ichthyosen führen können. Damit kann in etwa 80–90 % der Fälle die Diagnose molekulargenetisch gesichert werden. Die genetischen Ursachen der übrigen 10–20 % sind noch ungeklärt. Ein Großteil der bislang noch unklaren Krankheitsursachen wird sicherlich in den nächsten Jahren dank neuer Technologien wie der Exom-Sequenzierung entschlüsselt werden.

)-Gens verursacht. Die Diagnose wird mit dem In-vitro-Kontrakturtest (IVKT) gesichert. Inhalte dieser Arbeit sind die Genotyp-Phänotyp-Korrelation bei Patienten mit 2 mutierten
RYR1
-Allelen und die mutmaßliche Prävalenz der MH.
Material und Methoden
Nach Diagnosestellung einer suszeptiblen malignen Hyperthermie (MHS) durch den IVKT wurden die DNA-Proben von 44 Patienten mithilfe der Polymerase-Kettenreaktion-Amplifikation und Sequenzierung auf Mutationen des
RYR1
-Gens untersucht sowie die Daten der Mutationsanalyse mit denen des IVKT verglichen.
Ergebnisse
Identifiziert wurden 13 Patienten mit heterozygoter Mutation, ein Patient mit homozygoter Mutation (c.1840C>T) und ein Patient mit „compound“-heterozygoten Mutationen (c.1840C>T und c.6487C>T). Bei den beiden Patienten mit 2 mutierten Allelen lag eine stärkere Antwort im IVKT im Vergleich zu den Patienten mit nur einem mutierten Allel vor. Patienten mit einer

-Mutation im IVKT signifikant höhere Kontrakturen.
Schlussfolgerung
Bei beiden beschriebenen Patienten scheint das Vorliegen von 2 mutierten

-Allelen einen additiven Effekt auf die Funktionseinschränkung des RYR1-Rezeptors zu haben und sowohl zu einer stärkeren Antwort im IVKT als auch zu einer ausgeprägteren klinischen Reaktion zu führen. Patienten ohne bisher nachgewiesene

-Mutation weisen möglicherweise
RYR1
-Mutationen mit geringerer Potenz außerhalb der hier untersuchten „Hotspot“-Regionen und/oder falsch-positive IVKT-Resultate auf.
