Resumen Introducción y objetivo. La demanda masculina de tratamiento estético es cada vez más relevante. Sin embargo, todavía son pocas las evidencias sobre Medicina Estética en el hombre y aunque ...algunos estudios incluyen recomendaciones sobre las pautas de tratamiento con toxina y con rellenos para el varón, no todas son válidas en nuestro entorno mediterráneo. Por ello se ha elaborado el consenso MAPA (Male Aesthetic Patient Approach) a modo de guía de orientación práctica para ayudar al médico estético que se inicia en el tratamiento del paciente masculino. Material y Método. Un panel de 9 expertos en Medicina Estética de diferentes zonas de España con amplia experiencia en el uso de la toxina Abobotulinum y rellenos de ácido hialurónico se reunieron para poner en común sus conocimientos y prácticas clínicas habituales y desarrollar recomendaciones fundamentales en el campo de la Medicina Estética masculina. Resultados. Los temas discutidos incluyeron cuestiones relativas a la práctica clínica, perfil de pacientes tratados, planificación del tratamiento en función del objetivo deseado y principales técnicas de Medicina Estética recomendadas para los hombres. Finalmente, también se debatieron recomendaciones sobre los tratamientos a aplicar en cada área de tratamiento facial. Conclusiones. Este consenso recoge las opiniones, consejos y experiencia de médicos estéticos españoles con amplia experiencia en el uso de la toxina Abobotulinum y rellenos de ácido hialurónico para ayudar a los profesionales a conseguir los mejores resultados posibles en términos de eficacia y satisfacción del paciente masculino.
The description of atomic structure by Niels Bohr set the basis for the emergence of quantum physics. Based on these fundamentals, Einstein published in 1917 a paper on the amplification of energy by ...Stimulated Emission of Radiation as part of his quantum theories. In 1955, Townes and Gordon turned Einstein's theories into practice, creating a coherent and amplified microwave device using ammonia gas in an optical medium. But it was at the beginning of the 1980s, that Anderson and Parrish published an article about the selective photothermolysis model which revolutionized clinical practice. The use of laser in photoaging began with CO2 (10,600 nm). In 1989, it was first used for resurfacing of a face with prominent photoaging. Ablative lasers have therefore had great popularity in the 1980s and 1990s, but prolonged postoperative time and significant risk of side effects have lowered the acceptance by patients. In 2004, the description of the fractionated radiation for the treatment of photoaging, by Mainstein, represented a great event. The stimulation of collagen occurred through fractional laser beams, which would reach the selected area while saving islands of sound skin. These islands accelerated the process of cicatrization of the treated tissue and shortened the postprocedure time. Furthermore, the fractionated radiation presented a smaller range of side effects, increasing the safety of the procedure. As mentioned earlier, as fractional lasers incise on the skin, they leave islands of healthy skin that accelerate recovery, while generating necrosis columns. Such necrosis columns remove damaged extracellular matrix material, allowing resettlement of fibroblasts. Such resettled fibroblasts, under the influence of a new tensile strength, restart to produce structures for extracellular matrix, such as collagen, elastin, and proteoglycans, in a more physiological way. Fractional lasers are considered by many dermatologists as the best choice in laser therapy for the treatment of photoaging.
Acute rhinosinusitis (ARS) is defined as an inflammation of the mucosa of the nose and paranasal sinuses and affects 1–5 % of general population in Europe. Sinonasal diseases represent the main cause ...of smell alterations in adult patients and lead to mucosal congestion, increased quantity and density of secretions and altered mucociliary transport. For this reason the odorous molecules contained in the inspired air, cannot interact with the olfactory epithelium. Medical therapy of ARS has to reduce the severity and duration of symptoms and prevent complications. Recent studies have shown that Sodium hyaluronate modulate inflammation and has a reparative effect on the nasal mucosa. 48 patients affected by acute rhinosinusitis proven by CT scan, were enrolled. They were submitted to nasal endoscopy, olfactometric and mucociliary transport evaluation (MCTt), Visual Analogue Scale Questionnaire (VAS) at T0, after 14–18 days (T1) and after 30–35 days (T2). The patients were randomized into two treatment groups, A and B, and were treated for 30 days; each group was composed of 24 subjects. All patients received Levofloxacin (500 mg for 10 days) and Prednisone (50 mg for 8 days, 25 mg for 4 days and 12, 5 mg for 4 days). Moreover, Group A received twice a day for 30 days high molecular weight Sodium Hyaluronate (3 %) plus saline solution (3 mL sodium chloride-NaCl—0.9 %) using a nebulizer ampoule for nasal douche. Group B received twice a day for 30 days saline solution (6 mL sodium chloride-NaCl—0.9 %) using a nebulizer ampoule for nasal douche. At T1 Group A shown lower values in MCTt and threshold score was significantly higher than in Group B. VAS showed statistically significant differences between the two groups, in particular for smell, nasal obstruction and for nasal discharge. At T2 Group A MCTt was significantly lower than in Group B; odour threshold improved in both groups but in Group A was still significantly higher than in Group B. No statistical differences between two groups regarding odour discrimination and odour identification were confirmed at T1 and T2. VASy score showed statistically significant differences between the two groups only for nasal discharge.
JUSTIFICATIVA E OBJETIVOS: As formas de tratamento consideradas não invasivas para as alterações internas das articulações temporomandibulares (ATM) descritas na literatura são muitas, incluindo ...aconselhamento, farmacoterapia, fisioterapia e dispositivos interoclusais. No entanto, alguns pacientes tornam-se refratários aos tratamentos conservadores, sendo indicados procedimentos como artrocentese, artroscopia e cirurgias das ATM. A viscossuplementação é uma abordagem pouco invasiva, de baixo custo e com bons resultados em curto e médio prazo. O objetivo deste estudo foi discutir a técnica de viscossuplementação como tratamento das alterações internas da ATM, com relato de 2 casos clínicos com acompanhamento de 12 meses. RELATOS DOS CASOS: Duas pacientes com artralgia das ATM e refratárias à tratamento conservador foram submetidas a uma infiltração semanal de hialuronato de sódio por três semanas. Observou-se melhora da dor e da amplitude de abertura mandibular nas duas primeiras semanas, resultado que se manteve constante ao longo de um ano de acompanhamento. CONCLUSÃO: A viscossuplementação das ATM mostrou ser eficiente no controle da dor articular, melhorando também a função mandibular nos casos clínicos apresentados. Ensaios clínicos controlados com amostras significativas devem ser realizados para compreensão de sua real eficácia no tratamento das DTM.
