Palmoplantar pustulosis (PPP) sometimes presents with nail lesions, which affect the patients’ quality of life (QOL). However, little is known about nail lesions in PPP, and there is currently no ...established method for assessing them.
This study aimed to analyze the impact of PPP-related nail lesions with the patients’ QOL. In addition, we considered whether they might constitute a risk factor of pustulotic arthro-osteitis (PAO).’
A total of 178 patients with PPP were enrolled. Among the 178 patients, 66 patients participated in the following quality of life questionnaires; GHQ28, DLQI, and Skindex-16. The severity of the nail lesions was classified according to the Nail Psoriasis Severity Index (NAPSI), and the types of nail lesion were investigated.
The DLQI, Skindex-16 and PPPASI scores were significantly higher in patients with nail lesions than in those without them. Indentions, transverse ridging, and nail thickening were relatively common in PPP. Nail lesions were unrelated to the presence of PAO complications, but leukonychia and discoloration were likely to be related to PAO lesion site.
The study demonstrated that the presence of nail lesions is associated with a decreased QOL regardless of the severity of the skin lesions. The nail lesions were not a risk factor of PAO, but a predictor of skin lesion severity and PAO lesion site. Given this association, indention, transverse ridging, and thickening of the nail, currently not included in the NAPSI, should be added as an assessment item in the evaluation of PPP nail lesions.

Nasip izrađen od zrna dobivenih drobljenjem meke stijene može biti podložan dodanom slijeganju uzrokovanim raspucavanjem zrna unutar strukture nasipa. Ovo dodatno slijeganje nije uzrokovano promjenom ...stanja naprezanja unutar nasipa i ne može biti procijenjeno standardnim metodama proračuna na osnovi modula stišljivosti izmjerenog nakon zbijanja nasipa. Raspucavanje zrna je uzrokovano procesom rastrošbe, koji je u mekim stijenama kao što je lapor, uglavnom uzrokovan procesom sušenja i vlaženja. Ako su zrna u strukturi nasipa podvrgnuta sušenju i vlaženju, rezultat je usitnjavanje zrna do razine raspada u materijal koji se može opisati kao tlo. Usitnjeni materijal odvaja se od zrna i popunjava makro pore u strukturi nasipa što za posljedicu ima dodatno slijeganje nasipa. Uzorci za ispitivanje su izrađeni od drobljenog lapora u laboratorijskim uvjetima. Ispitivanje je provedeno sa modificiranim edometarskim uređajem. Izmjerene veličine se mogu iskoristiti u svrhu procjene dodatnog slijeganja nasipa uzrokovanog rastrošbom.
Nasipi za obranu od poplava su geotehničke građevine. One su bitne građevine za zaštitu ljudskih života i materijalnih dobara. Eurocode 7: Geotehničko projektiranje-dio 1.: Opća pravila, o njima ne ...vodi dovoljno računa. Projektanti su dužni pridržavati se naputaka iz ovog propisa, a oni se nalaze u nekoliko poglavlja EC7/1, što za projektante može djelovati zbunjujuće.
Ovim radom nastojalo se pomoći projektantima pri projektiranju i dimenzioniranu nasipa za obranu od poplava prema Eurokodu 7. Manjkavost EC7 u odnosu na nasipe za obranu od poplava uočila je i grupa za praćenje projekta pod nazivom „International Levee Handbook (ILH)“ (CIRIA C731, (2013.), te je za potrebe Ujedinjenog Kraljevstva i Irske izradila 2014. godine nacionalne upute pod nazivom “Primjena Eurokoda 7 na projektiranje nasipa za obranu od poplava” (dostupne na internetu).
