Kreiranje stoma je relativno čest problem u hitnoj i elektivnoj abdominalnoj kirurgiji no često zanemaren i prepušten mlađim ili manje iskusnim članovima operativnog tima. U Hrvatskoj postoji oko ...5000 bolesnika koji žive sa ileostomom ili kolostomom. Bolesnici se preoperativno i postoperativno savjetuju kako živjeti sa stomom. Većini bolesnika se može osigurati potrebna skrb odnosno olakšati svakodnevno funkcioniranje sa stomom i komplikacijama koje se mogu pojaviti vezano uz stomu. Važno je naglasiti da je stoma ponekad neophodna: kao jedina opcija nakon resekcije crijeva ili u stanjima hitnosti, kao zaštita zbog teških abdominalnih infekcija ili poboljšanje kvalitete života u nekim stanjima (crijevne upalne bolesti, inkontinencija). Postoje radne grupe bolesnika koji žive sa stomom. Međutim, koncept života sa stomom i stigma koju ona nosi bolesniku mora se olakšati bolesniku i putem multidisciplinarnog medicinskog tima uključujući i liječnike obiteljske medicine. Na koncu ključno je razvijati koncept enterostomalnog terapeuta-specijalista koji će pomoći bolesniku po završetku kirurškog liječenja i otpuštanja iz bolnice. U ovome članku nastojali smo objasniti kirurške principe formiranja stome, moguće komplikacije i tim koji bi trebao biti na raspolaganju bolesniku kojem je kreirana stoma.
Le retour à une certaine autonomie est souvent source d’angoisses importantes pour le patient stomisé. Il se pose de nombreuses questions sur la réalisation des soins, les troubles du transit, la ...prise de médicaments et le maintien des activités quotidiennes. L’équipe officinale doit mettre à sa disposition son expertise pour le rassurer et l’accompagner au mieux, tout au long de sa réadaptation à la vie de tous les jours.
The return to a certain degree of autonomy is often a source of major anxiety for the ostomate patient. They ask themselves many questions about how to carry out care, transit problems, taking medication and maintaining daily activities. The pharmacy team must put its expertise at his disposal to reassure him and accompany him as best as possible, throughout his readaptation to everyday life.
PREMESSA: Il 3%-82% dei pazienti stomizzati va incontro a complicanze stomali, che determinano un aggravio assistenziale con allungamento della degenza e dei tempi di recupero dell’autonomia ...gestionale dello stoma. In letteratura emerge come la mancanza del disegno stomale preoperatorio risulti uno dei fattori che aumenta l’incidenza di complicanze stomali, insieme al carattere d’urgenza dell’intervento, all’altezza, al tipo di stoma e all’indice di massa corporea.
SCOPO: Valutare il numero e le caratteristiche delle complicanze stomali, nel periodo precedente all’introduzione della procedura del disegno stomale preoperatorio eseguito dall’infermiere e confrontare tali numeri con quelli riguardanti le complicanze rilevate nel periodo successivo all’introduzione della procedura di stoma siting.
METODI: Studio osservazionale retrospettivo monocentrico presso le U.U.O.O. di Chirurgia generale e Urologia nell’Ospedale San Paolo dell’ASST Santi Paolo e Carlo di Milano. Campione di convenienza composto da pazienti sottoposti a confezionamento di una stomia enterale e urinaria.
RISULTATI: Sono stati reclutati 264 pazienti. Il 51,1% ha manifestato complicanze stomali; il 42,9% aveva effettuato lo stoma siting il 61,5% no. Le complicanze più frequenti sono state le alterazioni cutanee peristomali (29,9%); la differenza di numero tra i singoli tipi di complicanze nei due gruppi è clinicamente rilevante.
CONCLUSIONI: Lo stoma siting è una procedura di competenza infermieristica che, se ben eseguita, porta alla riduzione delle complicanze stomali. È responsabilità dell’infermiere garantire a ciascun paziente sottoposto a confezionamento di stomia la corretta esecuzione del disegno stomale, ogni volta che le condizioni del quadro clinico lo consentono.
Introductionles stomies digestives constituent l´aboutissement d´un certain nombre d´interventions chirurgicales. Elles peuvent être temporaires ou définitives. Le patient porteur de stomie se trouve ...confronté à des aléas d´ordre physique et psychologique. Il nécessite donc une adaptation aux changements et une acceptation de la situation qu´elle soit temporaire ou définitive. Le but de ce travail est d´évaluer l´impact des stomies digestives sur la qualité de vie des patients et leur retentissement professionnel.Méthodesil s´agit d´une étude rétrospective, menée de janvier 2010 à décembre 2014. Au cours de cette période 115 patients avaient eu des stomies digestives. Parmi ces patients, soixante étaient en âge d´activité professionnelle, avaient un emploi fixe et avaient repris leurs travail; ils constituent l´effectif de notre étude. Le recueil des données était réalisé par un questionnaire spécifique: le questionnaire généraliste SF-36 réalisé en post opératoire.Résultatsle score moyen pour la qualité de vie globale pour les patients stomisés était 41. Quarante patients avaient une mauvaise qualité de vie avec un score SF-36 qui était inférieur à 50. Notre étude a démontré que les patients stomisés ont éprouvé des difficultés de fonctionnement dans des situations de travail. Quarante-huit patients décrivaient une gêne directement liée à la stomie lors de l´exercice de leurs activités professionnelles. Les causes évoquées étaient d´ordre physique dans 6 cas et d´ordre psychologique dans 3 cas. Six patients étaient mutés vers des postes plus adaptés à leur situation.Conclusionl´objectif de la prise en charge des patients porteurs de stomie digestive doit être la réintégration sociale et professionnelle adéquate. Ceci ne peut se concevoir qu´en impliquant le médecin de travail, le psychologue, le stomathérapeute et les associations dans cette prise en charge.
La stomie constitue un véritable handicap dans nos régions. Nous reportons une série de patients bénéficiant d'un rétablissement précoce. Il s'agit d'une étude rétrospective descriptive sur une ...période de six ans. Elle concernait tous les patients qui avaient bénéficié dans le service de chirurgie générale du CHU Aristide Le Dantec de Dakar d'une fermeture de stomie dans un délai maximum de 30 jours. Il s'agissait de 66 patients: 43 colostomies et 23 ileostomies. Le délai moyen de rétablissement de la continuité digestive était de 21 jours. La morbidité était de 9% et la mortalité de 1,5%. Le rétablissement précoce de la stomie digestive est réalisable avec des résultats comparables au rétablissement conventionnel.
Cette recherche a pour objectif d’explorer l’évolution de l’image du corps, de l’anxiété et de la dépression au cours du traitement chirurgical chez des patients atteints d’un cancer du rectum et ...porteurs d’une stomie temporaire.
La veille de l’opération (T0), un mois après l’opération (T1) et deux mois après le retrait de la stomie temporaire (T2), la Hospital and Anxiety Depression Scale et le Questionnaire d’Image du Corps ont été complétés par 31 participants (13 femmes et 18 hommes, Mâge=62,48) atteints d’un cancer du rectum et traités par exérèse rectale avec stomie temporaire.
La création d’une stomie temporaire entraîne une altération significative de la satisfaction corporelle (F (2,58)=6,402, p<0,01), altération qui ne persiste pas à T2 et une augmentation des scores de dépression (F (2,60)=6,935, p<0,01) qui persiste à T2. À T0 et T2, la satisfaction corporelle est corrélée négativement avec l’anxiété et la dépression (p<0,05) mais pas à T1.
L’exérèse rectale avec stomie temporaire pour cancer du rectum a des répercussions psychologiques majeures. Il conviendrait de mettre en place un accompagnement psychologique et d’en évaluer l’efficacité.
In 2012, rectal cancer represented 30 % of colorectal cancers. In the case of cancer, three treatments are available, alone or associated: radiotherapy, chemotherapy and surgery. The main treatment for rectal cancer is surgery and more particularly rectal resection. The most important complication of this surgery is fistula. To reduce the risk, surgeons put a temporary stoma who can have repercussions like psychological difficulties, alteration of body image for example. The aims of this research were to explore the evolution of body satisfaction, anxiety and depression in the surgical treatment and study the associations between these variables.
Participants are recruited in a visceral surgery department of CHU of Bordeaux. On the eve of the operation (T0), one month after the operation (T1) and two months after reversal stoma (T2), the Hospital Anxiety and Depression Scale and the Questionnaire d’Image du Corps were completed by 31 participants (13 women et 18 men, Mage=62.48 years) diagnosed with rectal cancer treated with rectal excision and temporary stoma.
Temporary stoma had an impact on body satisfaction (F (2.58)=6.402, P<0.01) but the alteration of body satisfaction does not persist after reversal stoma. The mean score of body satisfaction at T0 (M=64.16, SD=10.92) was significantly (t (30)=3.075, P<0.01) higher than mean score of body satisfaction at T1 (M=55.97, SD=12.48). The mean score of body satisfaction at T1 was significantly lower (t (30)=2.881, P<0.01) than mean score of body satisfaction at T2 (M=61.51, SD=12.47). There was no significant difference between the average scores of body satisfaction at T0 and T2 (t (30)=1.065, P>0.05). In total, 19.3 % of participants have severe anxiety and 4.3 % severe depression. The mean scores of anxiety at T0, T1 and T2 were not significantly different (F (2,58)=1.163, P>0.05). Temporary stoma had an impact also depression (F (2.60)=6.935, P<0.01) who persist after reversal stoma. The mean score of depression at T0 (M=2.94, SD=2.5) was significantly lower than the mean score of depression at T1 (M=5.14, SD=3.15) and at T2 (M=4.14, SD=3.07). Finally, body satisfaction was negatively correlated with anxiety and depression at T0 and T2 (P<0.05).
Cancer remains the second leading cause of death in our advanced societies. In France and oncology, patients's psychological distress would be undervalued and insufficiently supported. That is why we focused on this variable and also on body image from a target population, yet little studied: patients with rectal cancer and temporary ostomy. With advances in recent years in the treatment of rectal cancer, sphincter preservation becomes more systematic, to the detriment of the final stoma, but it involves the implementation of a temporary ostomy. As we have seen, surgical treatment with temporary stoma for rectal cancer appears to have psychological repercussions. In a society where the fashion and advertising standards valuing a beautiful body hurt the ostomate person where their suffering is often ignored and little discussed, it seems essential and necessary to set up psychological counseling and measure its effectiveness.
Las estenosis benignas son complicaciones frecuentes posterior a la cirugía colorrectal, con una incidencia de hasta el 20%. El tratamiento endoscópico es seguro y efectivo, pero no hay evidencia ...suficiente para establecer el manejo de la estenosis a ese nivel anatómico.
Determinar los factores de riesgo asociados a el desarrollo de estenosis en pacientes con cáncer colorrectal y describir el tratamiento endoscópico en estos pacientes.
Se realizó un estudio retrospectivo en pacientes con cáncer colorrectal sometidos a cirugía y anastomosis, evaluados mediante colonoscopia entre 2014 y 2019.
De los 213 pacientes incluidos en el estudio, el 18.3% presentó una estenosis que fue asociada con el tipo de cirugía. La resección interesfintérica fue un factor de riesgo (OR = 18.81 IC 95%: 3.31-189.40, p < 0.001). El 69.2% de los pacientes con estenosis tenían un estoma, que fue identificado como factor de riesgo para estenosis (OR = 7.07, IC 95%: 3.10-16.57, p < 0.001). Se realizó anastomosis mecánica con engrapadora en el 87,4% de los pacientes que no presentaron estenosis, identificandose como factor de protección (OR = 0.41, IC 95%: 0.16-1.1, p = 0.04).
De los pacientes con estenosis el 69.2% de los pacientes requirieron tratamiento endoscópico con resultados favorables en el 83.3%. Solo el 2.6% de los pacientes tuvieron recurrencia y no se identificaron complicaciones asociadas al procedimiento.
La resección interesfintérica y la presencia de un estoma fueron factores de riesgo independientes para el desarrollo de estenosis; la anastomosis mecánica fue un factor protector contra el desarrollo de estenosis. El tratamiento endoscópico fue seguro y efectivo.
Benign strictures are frequent complications following colorectal surgery, with an incidence of up to 20%. Endoscopic treatment is safe and effective but there is not enough evidence for establishing stricture management at that anatomic level.
To determine the risk factors associated with the development of stricture in patients with colorectal cancer and describe endoscopic treatment in those patients.
A retrospective study was conducted on patients with colorectal cancer that underwent surgery and anastomosis, evaluated through colonoscopy, within the time frame of 2014 to 2019.
Of the 213 patients included in the study, 18.3% presented with stricture that was associated with the type of surgery. Intersphincteric resection was a risk factor (OR = 18.81, 95% CI: 3.31-189.40, p < .001). A total of 69.2% patients with stricture had a stoma, identifying it as a risk factor for stricture (OR = 7.07, 95% CI: 3.10-16.57, p < .001). Mechanical anastomotic stapling was performed in 87.4% of the patients that did not present with stricture, identifying it as a protective factor (OR = 0.41, 95% CI: 0.16-1.1, p = .04). Endoscopic treatment was required in 69.2% of the patients and provided favorable results in 83.3%. Only 2.6% of the patients had recurrence. No complications were reported.
Intersphincteric resection and the presence of a stoma were independent risk factors for stricture, and mechanical anastomosis was a protective factor against stricture development. Endoscopic treatment was safe and effective.
