    Chronic venous insufficiency (CVI) is a very common disorder, but still neglected because of low mortality and morbidity. New diagnostic devices and new classifications provide the basis for a better ... understanding and more objective comparison between different modalities of treatment, including surgery. Today, surgery for symptomatic varicose veins should be minimally invasive, on the outpatient basis and should yield good functional and cosmetic results, sparing the saphenous vein for possible bypass surgery in the future. Deep venous valve reconstruction in the surgical management of a patient with CVI is indicated for the most severe forms of CVI with marked reflux along all deep veins in the entire leg and after all non-surgical (lessinvasive and risky) management methods have failed.
