  • Management of the myelodysplastic syndromes in the year 2000 = Zdravljenje mielodisplastičnih sindromov v letu 2000

    Author presents overwiew of treatment modalities for myelodysplastic syndromes(MDS). Among non-intensive approaches, treatment with growth factors and cytokines, differentiating agents, antiapoptotic ... agents and antilymphocyteglobulin are described. Semi-intensive approach is due to predominantly cytotoxic effect of low dose cytosine-arabinoside, 5-azacytidine, hydroxiurea and novel topisomerase-I inhibitor - topotecan. Themost toxic and intensive approach is combination of several cytotoxic drugs, usually fludarabine, ida- rubicine and cytosine-arabinoside. Such treatment can be augmented by autologous stem cell transplant. But currently the only potentially curative modalitiy is allogeneic stem cell transplant. the later is associated with high treatment related mortality rate and requires suitable donor. Maybe non-myeloablative stem cell transplants with related and unrelated donors present future for treatment of MDS.
