    In the article's introduction the author writes about the aetiolology and localisation of arterial atherosclerotic changes in arteries of lower limbs indiabetic patients, that are the most common ... cause for chronic critical ischemia and limb loss. In continuation, the author discusses the morphologic conditions for endovascular revascularisation treatment and operative treatment of lower limb ischemia with in-situ venous by-pass graft. The results of endovascular treatment are, as in non-diabetic patients, most successful in the aortoiliac segment. However, isolated lesions of the iliac arteries are uncommon in diabetics, and accompanying stenoses or occlusions ofdistal arteries are frequently found. For better results of treatment of proximal lesions, we perform endovascular treatment on distal segments to establish better out-flow. The atherosclerotic disease most often affects arteries of the calf, frequently together with stenoses or occlusions of the superficial femoral artery, popliteal artery, deep femoral or internal iliac artery. The percentage of 5-year patency of recanalized calf arteries is not as high as it is in higher segments, but still plays an important role in limbsalvage, lowering the level of amputation and prolonging life expectancy. Endovascular procedures can be repeated and they do not prevent the operative revascularisation.
