  • Označevanje endoskopsko ugotovljenih sprememb pred kirurškim posegom (kovinske sponke Olympus HX-5/6) = Preoperative marking of lesions identified by endoscopy - Olympus HX-5/6 metal clips
    Stefanovič, Milan ...
Background. The introduction of endoscopy in the gastrointestinal diagnosis and therapy made it possible to identify and remove most polyps (adenomas) of the large bowel, and thereby prevent the ... development of malignant lesions. Surgery is occasionally required despite previous endoscopic extraction of polyps. It is much easier for the operating surgeon to determine the site and extent of segmental resection if the original polyp location has been previously marked by the endoscopist. Method. Tatooing with stains and markingwith metal clips are the two widely used techniques of endoscopic marking. Olympus HX-5/6 metal clips were invariably used in our series. Results. Prior to the operation, the site of the endoscopically resected malignant polyp was markerd with metalic clips in 5 patients: in one (20%) at endoscopy, and in 4 (80%) after histological confirmation of inadequate polypremoval by endoscopic electrocautery. In all cases (100%), the clips wereleft in place until surgery. In one case (20%), the operating surgeon missed the clip and the marked location for segmental resection. Conclusions. Our method of preoperative marking with metal clips has proved very successfuland was associated with no complications. Since identifying the marked site by palpation is not easy, radiologic or diaphanoscopic visualization can prove of great help to the operating surgeon. For a more accurate evaluation of the results and complications this marking technique will have to be used in a larger series of patients.
