Limited information is available regarding the factors related to short-term hospital admission following an exacerbation of chronic obstructive pulmonary disease (eCOPD).
The aim of this study was ...to identify variables related to short-term admission in patients with an eCOPD.
This was a prospective cohort study of patients with an eCOPD who attended an emergency department (ED) at 1 of 16 hospitals. Information on possible predictor variables was recorded during the ED stay, 24 h after admission to the hospital or after ED discharge home, and at hospital discharge or 1 week later if discharged home from the ED. An admission after an eCOPD within 2 months was the outcome of interest. Multivariate models were employed for patients admitted to the hospital or discharged home from the ED.
For patients discharged home from the ED, eCOPD-related hospital admissions in the previous year odds ratio (OR) 1.98 and 2.33, pCO2 at ED admission (ORs 2.02 and 2.90), the number of ED visits within 1 week of the index ED visit (OR 5.14) and dyspnea level 1 week after the index ED visit (ORs 2.66 and 1.40) were predictors of short-term admission area under the curve (AUC) 0.82. For patients admitted to the hospital during the index ED visit, baseline FEV1% (ORs 1.32 and 1.88), eCOPD-related hospital admissions in the previous year (ORs 1.28 and 2.51), severe baseline dyspnea (OR 2.57) and dyspnea level 1 week after the index ED visit (ORs 2.15 and 1.74) were predictors of short-term readmission (AUC 0.73).
Just a few easily recorded parameters may allow clinicians to identify patients at a higher risk of short-term readmission and establish preventive strategies.
Una alta capacidad para representar el análisis de la sostenibilidad es la exigencia central de cualquier metodología que pretenda encontrar en dicho análisis una herramienta para el desarrollo ...social y un diagnóstico de relación de las sociedades humanas con el entorno. Los requerimientos de dicha capacidad representadora se cifran en un lenguaje lo suficientemente recursivo y sintético en sus rasgos y principios como para ser flexible y en ello ofrecer pertinencia y operacionalidad para diversidad de escenarios y situaciones que, en todo caso, se caracterizan por la complejidad del tejido entre sus múltiples niveles y dimensiones. A través de una gramática el MUSIASEM ha cifrado en ello su potencial analítico y predictivo. El presente artículo argumenta las ventajas y alcances de las gramáticas como alternativa a los modelos en la representación del análisis de la sostenibilidad, explica cómo está fundamentado y construido el caso de la gramática del MUSIASEM y expone aplicaciones ya elaboradas a los temas de alimentos, suelos, energía, construcción y energía.

SARS-CoV-2 main protease is a common target for ...inhibition assays due to its high conservation among coronaviruses. Since flavonoids show antiviral activity, several in silico works have proposed them as potential SARS-CoV-2 main protease inhibitors. Nonetheless, there is reason to doubt certain results given the lack of consideration for flavonoid promiscuity or main protease plasticity, usage of short library sizes, absence of control molecules and/or the limitation of the methodology to a single target site. Here, we report a virtual screening study where dorsilurin E, euchrenone a11, sanggenol O and CHEMBL2171598 are proposed to inhibit main protease through different pathways. Remarkably, novel structural mechanisms were observed after sanggenol O and CHEMBL2171598 bound to experimentally proven allosteric sites. The former drastically affected the active site, while the latter triggered a hinge movement which has been previously reported for an inactive SARS-CoV main protease mutant. The use of a curated database of 4.8 k flavonoids, combining two well-known docking software (AutoDock Vina and AutoDock4.2), molecular dynamics and MMPBSA, guaranteed an adequate analysis and robust interpretation. These criteria can be considered for future screening campaigns against SARS-CoV-2 main protease.
