Al igual que en otras dictaduras del Cono Sur, durante la última dictadura militar argentina (1976-1983), las mujeres detenidas fueron víctimas de distintas formas de violencia sexual. Recién en los ...últimos años, luego de la reapertura de los juicios por delitos de lesa humanidad y con una cantidad considerable de resistencias, se empezó a considerar la posibilidad de juzgar la violencia sexual como un delito diferenciado de los tormentos. En el presente trabajo nos proponemos dar cuenta de los principales debates jurídicos en torno a la posibilidad de juzgar estos delitos y argumentaremos en favor de la posibilidad de dicho juzgamiento.
Most pancreatic cancers and extrahepatic cholangiocarcinomas are unresectable at the time of diagnosis, and even in case of a resectable cancer, for elderly or patients with coexistent comorbidities, ...surgery is not an option. Current treatment alternatives in these scenarios are very limited. Biliary stenting with selfexpanding metal stents(SEMS) is the mainstay palliative treatment of biliary obstruction due to unresectable pancreatic cancer or cholangiocarcinoma. Nevertheless, more than 50% of SEMS become occluded after 6 mo due to tumour over- and ingrowth, leading to hospital readmissions and reinterventions that significantly impair quality of life. Regimes of chemotherapy or chemoradiotherapy also provide minimal survival benefits. Therefore, novel therapies are eagerly awaited. Radiofrequency(RF) energy causes coagulative necrosis leading to local destruction of the accessed malignant tissue and has an established role in the treatment of malignancies in several solid organs, especially liver cancers. However, pancreatic and extrahepatic biliary cancers are not easily accessed by a percutaneous route, making the procedure dangerous. Over the past five years, the development of dedicated devices compatible with endoscopic instruments has offered a minimally invasive option for RF energy delivery in biliopancreatic cancers. Emerging experience with endoscopic RF ablation(RFA) in this setting has been reported in the literature, but little is known about its feasibility, efficacy and safety. A literature review makes it clear that RFA in biliopancreatic tumours is feasible with high rates of technical success and acceptable safety profile. Although available data suggest a benefit of survival with RFA, there is not enough evidence to draw a firm conclusion about its efficacy. For this reason, prospective randomized trials comparing RFA with standard palliative treatments with quality-of-life and survival endpoints are required. Anecdotal reportshave also highlighted a potential curative role of RFA in small pancreatic tumours and benign conditions, such as ductal extension of ampullomas, intrahepatic adenomas or non-tumoural biliary strictures. These newest indications also deserve further examination in larger series of studies.

Lutein, a xanthophyll, is associated to decreased risk of age-related macular degeneration, atherosclerosis and other diseases (Bovier et al., 2013; El-raey, Ibrahim, & Eldahshan, 2013). When lutein is extracted, it becomes highly unstable, reducing its functionality as an antioxidant. The aim of this research was to improve the stability of lutein using maltodextrin, arabic gum and a modified starch, to obtain micro-particles using spray-drying. Each of the formulations was characterized in terms of yield, encapsulation efficiency, particle size distribution, water activity and moisture content.
The formulations with arabic gum (100%) and arabic gum:maltodextrin:modified starch (33.3:33.3:33.3%), with encapsulation efficiencies of 91.94 ± 6.88 and 65.72 ± 0.93%, respectively, were selected to study stability at 45 °C and 75% RH (relative humidity). Based on our results, encapsulation could be considered as an alternative for the generation of high value-added functional ingredients that can be used in different industries.
How does the use of parent–daycare mobile communication applications reconfigure the emplacement and timing of parenting? In what ways, if any, do parents’ practices challenge the distinction between ...warm care and cold technology? Based on 35 interviews with 18 parents, we identified 4 ways in which Aula, Denmark's parent–daycare app, reconfigures the time-space dimensions of parenting. While parents and children are apart, mobile communication via Aula allows parents to “plan ahead,” and “synchronize” expectations and schedules with those of their children and the daycare center and “look back” on their children's time away. In addition, communication via Aula enhances co-presence. Overall, this paper's findings offer an alternative to views that oppose technology and care, showing how mobile communication may reshape responsibility for childcare in the welfare state.
The COVID-19 pandemic is probably the greatest health catastrophe of the modern era. Spain's health care system has been exposed to uncontrollable numbers of patients over a short period, causing the ...system to collapse. Given that diagnosis is not immediate, and there is no effective treatment for COVID-19, other tools have had to be developed to identify patients at the risk of severe disease complications and thus optimize material and human resources in health care. There are no tools to identify patients who have a worse prognosis than others.
This study aimed to process a sample of electronic health records of patients with COVID-19 in order to develop a machine learning model to predict the severity of infection and mortality from among clinical laboratory parameters. Early patient classification can help optimize material and human resources, and analysis of the most important features of the model could provide more detailed insights into the disease.
After an initial performance evaluation based on a comparison with several other well-known methods, the extreme gradient boosting algorithm was selected as the predictive method for this study. In addition, Shapley Additive Explanations was used to analyze the importance of the features of the resulting model.
After data preprocessing, 1823 confirmed patients with COVID-19 and 32 predictor features were selected. On bootstrap validation, the extreme gradient boosting classifier yielded a value of 0.97 (95% CI 0.96-0.98) for the area under the receiver operator characteristic curve, 0.86 (95% CI 0.80-0.91) for the area under the precision-recall curve, 0.94 (95% CI 0.92-0.95) for accuracy, 0.77 (95% CI 0.72-0.83) for the F-score, 0.93 (95% CI 0.89-0.98) for sensitivity, and 0.91 (95% CI 0.86-0.96) for specificity. The 4 most relevant features for model prediction were lactate dehydrogenase activity, C-reactive protein levels, neutrophil counts, and urea levels.
Our predictive model yielded excellent results in the differentiating among patients who died of COVID-19, primarily from among laboratory parameter values. Analysis of the resulting model identified a set of features with the most significant impact on the prediction, thus relating them to a higher risk of mortality.
Endoscopic ultrasonography (EUS) has gained wide acceptance as an important, minimally invasive diagnostic tool in gastroenterology, pulmonology, visceral surgery and oncology. This review focuses on ...data regarding risks and complications of non-interventional diagnostic EUS and EUS-guided fine-needle biopsy (EUS-FNB). Measures to improve the safety of EUS und EUS-FNB will be discussed. Due to the specific mechanical properties of echoendoscopes in EUS, there is a low but noteworthy risk of perforation. To minimize this risk, endoscopists should be familiar with the specific features of their equipment and their patients’ specific anatomical situations (e.g., tumor stenosis, diverticula). Most diagnostic EUS complications occur during EUS-FNB. Pain, acute pancreatitis, infection and bleeding are the primary adverse effects, occurring in 1% to 2% ofpatients. Only a few cases of needle tract seeding and peritoneal dissemination have been reported. The mortality associated with EUS and EUS-FNB is 0.02%. The risks associated with EUS-FNB are affected by endoscopist experience and target lesion. EUS-FNB of cystic lesions is associated with an increased risk of infection and hemorrhage. Peri-interventional antibiotics are recommended to prevent cyst infection. Adequate education and training, as well consideration of contraindications, are essential to minimize the risks of EUS and EUS-FNB. Restricting EUS-FNB only to patients in whom the cytopathological results may be expected to change the course of management is the best way of reducing the number of complications.
Over the last decade, the development of stabilised microbubble contrast agents and improvements in available ultrasonic equipment, such as harmonic imaging, have enabled us to display microbubble ...enhancements on a greyscale with optimal contrast and spatial resolution. Recent technological advances made contrast harmonic technology available for endoscopic ultrasound(EUS) for the first time in 2008. Thus, the evaluation of microcirculation is now feasible with EUS, prompting the evolution of contrast-enhanced EUS from vascular imaging to images of the perfused tissue. Although the relevant experience is still preliminary, several reports have highlighted contrast-enhanced harmonic EUS(CHEUS) as a promising noninvasive method to visualise and characterise lesions and to differentiate benign from malignant focal lesions. Even if histology remains the gold standard, the combination of CH-EUS and EUS fine needle aspiration(EUS-FNA) can not only render EUS more accurate but may also assist physicians inmaking decisions when EUS-FNA is inconclusive,increasing the yield of EUS-FNA by guiding the puncture with simultaneous imaging of the vascularity.The development of CH-EUS has also opened up exciting possibilities in other research areas,including monitoring responses to anticancer chemotherapy or to ethanolinduced pancreatic tissue ablation,anticancer therapies based on ultrasound-triggered drug and gene delivery,and therapeutic adjuvants by contrast ultrasound-induced apoptosis.Contrast harmonic imaging is gaining popularity because of its efficacy,simplicity and noninvasive nature,and many expectations are currently resting on this technique.If its potential is confirmed in the near future,contrast harmonic imaging will become a standard practice in EUS.
Minimally processed fruits are an alternative to dairy products to deliver probiotics. Bio-protection against several factors that affect their viability has been proposed in the food industry. In ...this study, probiotic Lactobacillus rhamnosus CECT 8361 was added to alginate-based coatings enriched with inulin and oligofructose and applied on fresh-blueberries. Probiotic viability, microbiological, physicochemical and sensory quality parameters of blueberries were monitored during 21 days of refrigerated storage. Also, L. rhamnosus CECT 8361 was tested for its antagonistic effect against inoculated Listeria innocua and E. coli O157:H7.
Advantageously, prebiotic compounds allowed improving probiotic viability with counts above 6.2 log CFU/g for the entire period. Native microbiota counts remained under safe levels. Overall visual quality, odor and flavor were acceptable up to day 14 of storage. Regarding antimicrobial activity, L. rhamnosus CECT 8361 was able to reduce L. innocua counts by 1.7 log in inoculated blueberries. These results encourage further implementation of new fruit-based foods with multifunctional properties.

The increased popularity of vegetarianism, lactose intolerance, and the high cholesterol content of dairy products are all factors that have recently increased the demand for non-dairy probiotic ...foods. This study aimed to evaluate the effect of probiotic supplementation of fresh-cut apple coated with alginate-prebiotic on its nutritional, safety, and sensory characteristics. Also, probiotic viability during refrigerated storage and after simulated gastrointestinal digestion (GID) was evaluated. A new non-dairy probiotic food was developed by adding prebiotics (oligofructose and inulin) and probiotic cultures (Lactobacillus rhamnosus and Bifidobacterium animalis subsp. lactis) to fresh-cut apple. Both probiotics successfully remained above 9.0 log CFU/g in the product after 8 days of storage. Also, high viability retention was achieved after GID with 7.8 and 8.0 log for L. rhamnosus and B. lactis, respectively, without significant effect of prebiotic addition. Microbiological and nutritional quality were maintained, however only those samples with prebiotics and B. lactis remained sensorially acceptable after 8 d. Furthermore, both probiotics exerted bactericidal effect against L. innocua inoculated on apple cubes with reductions higher than 1.8 log meanwhile, the effect on E. coli O157:H7 was less pronounced. Thereby, apple pieces coated with prebiotic-alginate solution constitute a promising alternative to design new carriers for probiotic bacteria.
