Several surgical approaches to the cerebellopontine angle and internal auditory canal have been developed for the removal of acoustic neuromas. The choice of an approach may be influenced by hearing ...levels and tumor size. We reviewed the records of the primary translabyrinthine removal of 167 large (> or = 4 cm) acoustic neuromas performed between 1982 and 1990. Patients ranged in age from 15 to 83 years, with a mean of 43 years (male, 49%; female, 51%). Total removal was achieved in 95%. The facial nerve was preserved anatomically intact in 91%. At follow-up (mean, 2.1 yr), facial nerve function was acceptable (Grades I-IV) in 75% and good (Grades I-II) in 42%. Vascular complications occurred in 4.8%; however, there were no deaths. A cerebrospinal fluid leak occurred in 9.6% of cases, and meningitis occurred in 8.3%. In patients with large tumors where there is little chance to preserve preoperative hearing, we have successfully used the translabyrinthine approach for total tumor removal. The advantages and disadvantages of both the translabyrinthine and suboccipital approaches are discussed.
School psychologists have training and expertise in consultation and evidence-based interventions that position them well to support early career teachers (ECTs). The current study involved iterative ...development and pilot testing of an intervention to help ECTs become more effective in classroom management and engaging learners, as well as more connected to colleagues. The intervention included group seminars, professional learning communities, and coaching. The sample included 15 ECTs and 57 school personnel in three high-poverty, urban schools. Feasibility and initial promise of the intervention were examined using a mixed-method design, which yielded promising trends in ECTs' effectiveness and connectedness. ECTs described facilitators to effectiveness and connectedness associated with the intervention and barriers associated with the structural realities of schools and gaps in their training. ECTs described effectiveness as neither static nor global and perceived meaningful progress, leveraging individual relationships and group formats to receive instrumental and emotional support.
We propose a novel method for compression of periodic optical pulses based on all-optical repetition rate multiplication of pulses without requiring propagation in a dispersive delay line. The ...compression principle is explained using the temporal Talbot effect. The proposed method is demonstrated experimentally with the generation of ∼20 ps pulses from cw radiation of a laser diode. The repetition rate multiplication is performed with fiber Bragg gratings. The proposed method simultaneously implements two important requirements of many fields, for example, of optical communications: pulse compression and pulse repetition rate multiplication.

We report evidence for a narrow structure, X(5568), in the decay sequence X(5568)→B_{s}^{0}π^{±}, B_{s}^{0}→J/ψϕ, J/ψ→μ^{+}μ^{-}, ϕ→K^{+}K^{-}. This is evidence for the first instance of a hadronic ...state with valence quarks of four different flavors. The mass and natural width of this state are measured to be m=5567.8±2.9(stat)_{-1.9}^{+0.9}(syst) MeV/c^{2} and Γ=21.9±6.4(stat)_{-2.5}^{+5.0}(syst) MeV/c^{2}. If the decay is X(5568)→B_{s}^{*}π^{±}→B_{s}^{0}γπ^{±} with an unseen γ, m(X(5568)) will be shifted up by m(B_{s}^{*})-m(B_{s}^{0})∼49 MeV/c^{2}. This measurement is based on 10.4 fb^{-1} of ppover ¯ collision data at sqrts=1.96 TeV collected by the D0 experiment at the Fermilab Tevatron collider.
This paper demonstrates a multiple-use zoning scheme approach to the management of marine activities at a regional sea scale. The zoning scheme is based on existing legally permitted and permissible ...mechanisms within the Irish Sea and is designed to inform the development of marine spatial planning (MSP). The scheme indicates visually and descriptively the extent to which the existing planning and governance framework is able, or unable, to meet the increasing pressures of activities and developments within the marine environment. In order to test the robustness of the zoning scheme, several scenarios were developed and assessed against the de facto zonation methodology. The Irish Sea Pilot's collated data on the presence of marine landscapes was used as a case study example.
At many airports, aircraft take off from multiple departure runways. During periods of high departure demand, whether or not the departure runways are balanced directly affects the capacity and ...efficiency of the airport. The paper begins by investigating the cause of runway imbalances. Homogeneity in the direction of flight during a departure push and the procedures for runway assignments are demonstrated to be the primary source of departure runway imbalances. Second, the paper studies how well departure runways are currently balanced. A method for reconstructing the departure queues that existed at each runway is presented along with results from applying the method. Dallas/Fort Worth (DFW) airport is used as a case study throughout the paper. Finally, the paper introduces automation concepts that will reduce the occurrence and impact of imbalanced departure runways, by providing this information along with traffic management advisories.
As changes in health-care delivery impel us to refine clinical science training, the opportunity arises to reconceptualize internship training to align more closely with clinical science values and ...outcomes. We present observations on the evolution of internship training with a focus on the following issues. First, we highlight the significance of a public-health perspective for clinical science as a basis for refining training goals and practices. Second, we briefly review how internship training evolved (where it has come from) to set the context for continuing evolution (where it might go). Third, we discuss the need for an expanded definition of clinical competence for clinical science training to better align with innovations in health care and to prepare graduates for new career opportunities. Finally, we present examples of new models for internship training that might accommodate the continuing redefinition of internship training in clinical science.
