Le syndrome de l’ovaire rémanent, encore appelé « syndrome des restes ovariens », est défini comme la persistance de fragments de cortex ovarien laissés en place de manière non-intentionnelle par le ...chirurgien censé avoir accompli une ovariectomie bilatérale. La symptomatologie associe classiquement des algies pelviennes chroniques à divers troubles compressifs intestinaux et/ou urinaires liés à la formation de kystes sous-péritonéaux.
Nous rapportons un cas de dégénérescence adénocarcinomateuse papillaire sur tissu ovarien rémanent révélé par une masse pelvienne hémorragique, 5 ans après hystérectomie totale avec annexectomie bilatérale pour fibrome utérin chez une femme de 57 ans.
Il s’agissait d’un stade IIIc FIGO en raison d’un envahissement ganglionnaire iliaque primitif tandis qu’il n’y avait pas d’ascite, pas de nodule de carcinose intra-abdominale ni d’implant omental mais une urétérohydronéphrose consécutive au mode d’extension néoplasique sous-péritonéal inhabituel.
L’exérèse tumorale complète a été effectuée au prix d’une cystectomie partielle avec résection urétérale basse associée à la colpectomie, à l’omentectomie et au curage ilio-lombo-aortique. Neuf cures de chimiothérapie adjuvante ont été administrées, protocole Taxol-Carboplatine.
Ovarian remnant syndrome is defined as residual ovarian tissue non intentionally left in place by the surgeon during a bilateral salpingo-oophorectomy. Patients present various symptoms usually including chronic pelvic pain, pelvic mass, bowel obstruction, hydronephrosis due to ureteral compression.
We report a case of adenocarcinoma arising in such an ovarian remnant revealed by vaginal bleeding 5 years after total abdominal hysterectomy and bilateral oophorectomy for uterine fibroids.
It was regarded as stage IIIc according to the FIGO classification because of common iliac lymph node involvement while there was no ascitis, no peritoneal nor omental implant but a unilateral hydronephrosis induced by extrinsec ureteral obstruction.
Complete cytoreductive surgery was achieved including partial bladder and lower ureteral resection with colpectomy, omentectomy, pelvic and para-aortic lymphadenectomy. Paclitaxel-Platinum combination chemotherapy was given for nine cycles.

Mecinarone (18.25 mg/kg twice a day orally for 6 months) induces an increase in melanin content of pigmented skin and mucosae in the Beagle dog. This effect is reversible. This effect is not due to a ...central or an endocrine mechanism because plasma levels of M.S.H., A.C.T.H. and cortisol remain normal. High levels of Mecinarone are found in melanized areas (skin, hair and non tapetal area of the eye). This effect is reversible. In contrast wih the Beagle, studies in pigmented guinea-pig and rabbit have been negative.
