To assess the association between quality of stimulation in the family environment and child's cognitive development considering the impact of mother's schooling on the quality of stimulation.
A ...cross-sectional study comprising 350 children aged 17-42 months was carried out in central and peripheral areas of Salvador, Northeastern Brazil, in 1999. A socio-economic questionnaire was used, along with the Home Observation for Measurement of the Environment Scale (HOME Inventory), and the Bayley Scale for Infant Development. Bivariate and multivariate analyses were carried out through linear regression at 5% level of significance.
There was a positive (beta=0.66) and statistically significant association between quality of stimulation in the family environment and child's cognitive development. Part of the effect was mediated by the mother's working circumstances and educational level. It was verified that a better quality of stimulation is provided for those who come early in the birth order in family, and live with only a few others under five years of age. This pattern of stimulation is better among children who live with their parents and whose mothers have better education, have a job and a partner involved in the family environment.
Quality of stimulation in the family environment is crucial for child's cognitive development, besides the significant role of the available resources and family dynamics. The study findings show the pertinence to cognitive development of interventions which improve the quality of the environment and the child-caregiver relationship.
A avaliação da satisfação dos usuários em serviços públicos de saúde é ainda uma prática muito recente. No Brasil, a implantação do Sistema Único de Saúde (SUS) criou circunstâncias favoráveis à ...manifestação do cidadão quanto aos serviços prestados. Em 2001, o Ministério da Saúde criou o Serviço de Atendimento ao Cidadão Usuário do Sistema Único de Saúde (SAC/ SUS), incorporando o Disque-Saúde, através do qual qualquer cidadão pode fazer reclamações, denúncias, solicitações, sugestões e elogios, bem como pedir informações. O Sistema de Atendimento de Demandas Espontâneas (SADE) é um subsistema do SAC/SUS cujo objetivo é dar andamento às solicitações dos usuários, criando um fluxo informatizado entre as três esferas de governo do SUS. O objetivo desse estudo foi analisar a resolutividade do SADE, verificando se o problema que deu origem à demanda foi resolvido, o tempo decorrido e o grau de satisfação dos usuários com o sistema. Foram identificadas as demandas concluídas no ano de 2003 (n = 123) e realizadas entrevistas telefônicas com os usuários. Dos 54 entrevistados, 48,1% consideraram que não foi resolvido o problema, 40,4% consideraram que foi resolvido, e 11,5% consideraram que foi resolvido em parte. Quanto ao tempo decorrido entre a demanda e a solução do problema, 35% dos entrevistados disseram ter levado menos de um mês, e 31% não souberam precisar. Quanto à satisfação com o sistema, 52,3% atribuíram nota entre 8,5 e 10, porém, 18,2% deram nota 0. Conclui-se que o SADE é um canal de voz do cidadão, porém, fazse necessário que os governos federal, estadual e municipal continuem aprimorando-o, especialmente na busca de solução para os problemas relatados. O objetivo desta dissertação é conceber uma vela asa rígida simétrica, de rotação livre com cauda, integrável num veleiro autónomo com um sistema de controlo baseado em Robot Operating System (ROS). ...O estudo inicial desta dissertação abrangeu: (i) veleiros autónomos com vela rígida e tradicional e as competições de veleiros autónomos existentes, (ii) os diferentes tipos de materiais e componentes eletrónicos empregues nos veleiros autónomos, (iii) seleção do perfil alar e as forças aplicadas na vela asa e (iv) o estudo das ferramentas de simulação existentes. Em seguida, desenhou-se uma vela asa com cauda (ambas com perfil alar EPPLER 479), um sensor anemómetro (que calcula o vento aparente), um sistema de controlo da cauda (que recorre ao ˆangulo de ataque atual e desejado como entradas) e um sistema de controlo de direção (que recorre `a posição atual do veleiro e à posição desejada como entradas). Posteriormente, procedeu-se à simulação da plataforma desenhada no software Autodesk Fusion 360 importada no ambiente de simulação Gazebo. Subsequentemente, foram adicionados diversos plugins ao ambiente de simulação, nomeadamente plugins reutilizados, alterados e desenvolvidos de raiz. O sistema de controlo foi desenvolvido no ambiente ROS recorrendo a um controlador Proportional - Differential (PD) para controlo da cauda e um controlador Proportional (P) para controlo da direção do veleiro. Por fim, foram realizados testes de controlo da vela asa, para validar o controlo via cauda e a propulsão do veleiro, assim como testes de controlo de direção para validar o mecanismo de direção do veleiro. Além disso, foi criado o diagrama polar para o modelo desenvolvido que descreve as características dinâmicas do veleiro.

End‐stage renal disease is a health problem that consumes public and private resources. This study aimed to identify the cost of hemodialysis (either daily or conventional hemodialysis) and ...transplantation in children and adolescents.
This was a retrospective cohort of pediatric patients with End‐stage renal disease who underwent hemodialysis followed by kidney transplant. All costs incurred in the treatment were collected and the monthly total cost was calculated per patient and for each renal therapy. Subsequently, a dynamic panel data model was estimated.
The study included 30 children who underwent hemodialysis (16 conventional/14 daily hemodialysis) followed by renal transplantation. The mean monthly outlay for hemodialysis was USD 3500 and USD 1900 for transplant. Hemodialysis costs added up to over USD 87,000 in 40 months for conventional dialysis patients and USD 131,000 in 50 months for daily dialysis patients. In turn, transplant costs in 50 months reached USD 48,000 and USD 70,000, for conventional and daily dialysis patients, respectively. For conventional dialysis patients, transplant is less costly when therapy exceeds 16 months, whereas for daily dialysis patients, the threshold is around 13 months.
Transplantation is less expensive than dialysis in children, and the estimated thresholds indicate that renal transplant should be the preferred treatment for pediatric patients.
A Doença Renal em Estágio Final é um problema de saúde que consome recursos públicos e privados. Nosso objetivo é identificar o custo da hemodiálise (hemodiálise diarias ou convencional) e transplante em crianças e adolescentes.
Uma coorte retrospectiva de pacientes pediátricos com Doença Renal em Estágio Final (DREF) submetidos à hemodiálise após transplante de rim. Todos os custos incorridos no tratamento foram cobrados e o custo total mensal foi calculado por paciente e por cada terapia renal. Então, foi estimado um modelo dinâmico com dados em painel.
Estudamos 30 crianças submetidas à hemodiálise (16 hemodiálises convencionais/14 hemodiálises diárias) após transplante renal. O gasto médio mensal para hemodiálise foi US$3,5 mil e US$1,9 mil para transplante. Os custos de hemodiálise somam mais de US$87 mil em 40 meses para pacientes submetidos a hemodiálise convencional (HC) e US$131 mil em 50 meses para pacientes submetidos a hemodiálise diária (HD). Por outro lado, os custos de transplante em 50 meses atingem US$48 e US$70 mil, para pacientes submetidos a HC e HD, respectivamente. Para pacientes submetidos à hemodiálise convencional, o transplante é menos oneroso quando a terapia ultrapassa 16 meses, ao passo que para pacientes submetidos a hemodiálise diária o limiar é cerca de 13 meses.
O transplante é menos caro que a diálise em crianças e os limiares estimados indicam que o transplante renal deve ser o tratamento preferencial para pacientes pediátricos.

The Delta:Notch signaling pathway regulates multiple aspects of hematopoietic cell differentiation and function and more recently Delta-like 4 (Dll4) has been shown also to regulate ...adult neo-vascularization (angiogenesis), namely in tumors, which led to the development of therapeutic strategies targeting Dll4 for the treatment of different cancers. Nevertheless, the current therapeutic strategies to target Dll4, which were suggested to result in non-functional vessels in the tumors, appear also to affect the vasculature of normal organs. Given the importance of the Bone marrow (BM) vascular niche in regulating hematopoiesis, we hypothesized Dll4 targeting might also affect BM function. In the present study, we describe 2 means of targeting Dll4: one using Dll4 specific neutralizing monoclonal antibodies (systemic treatment), and another using VECadCreERT2Dll4lox/lox mice, where Dll4 is specifically ablated on VE-Cadherin expressing cells. Using these 2 models, we investigated if Dll4 targeting affected the BM vascular niche and consequently affected hematopoietic recovery following irradiation and BM recovery in a transplant setting.
First, we show that the genetic ablation of Dll4 on VE-Cadherin+ vessels (VECadCreERT2Dll4lox/lox mice) and also anti-Dll4 antibody treatment of WT sub-lethally irradiated mice (300rad) does affect the BM vascular niche, resulting in increased megakaryocytes (CD41+), CD31+ and VE-Cadherin+ vessels, without significant changes on CD105+ vessels. Interestingly, the systemic anti-Dll4 treatment of non-irradiated and sub-lethally irradiated WT mice increased the BM and peripheral blood myeloid (CD11b+) content and promoted hematopoietic progenitor cell (Sca1+Flk1−) mobilization, as determined by FACS analysis. The genetic model (VECadCreERT2Dll4lox/lox mice) showed no changes in hematopoietic cells percentage or mobilization.
However, BM recovery in a transplant setting was improved in both models. Lethally irradiated WT mice transplanted with anti-Dll4 treated BM following sub-lethal irradiation or BM from VECadCreERT2Dll4lox/lox mice showed faster hematopoietic recovery than control mice transplanted with WT BM alone.
Taken together, we show that Dll4 blockade perturbs the BM vascular niche, increasing VE-Cadherin+ and CD31+ vessel number and also megakaryocyte BM content, both in the setting of Dll4 genetic ablation (using VECadCreERT2Dll4lox/lox mice) and also using a systemic approach to target Dll4. However, these vascular changes do not, at least following sub-lethal irradiation, significantly affect the BM recovery or affect the normal distribution of the hematopoietic populations. Nevertheless, the significant effects observed in a BM transplant setting (both where donor had been exposed to the systemic anti-Dll4 blockade and where Dll4 had been specifically ablated on VE-Cadherin+ vessels) suggest the existence of cell autonomous (and BM vascular niche-autonomous) effects of Dll4 targeting which deserve further detailed analysis. These data also suggest targeting Dll4-expressing vessels in the setting of BM malignancies may have therapeutic benefit, since it appears to have limited effects on normal BM/hematopoietic function. We are currently investigating the therapeutic efficacy of Dll4 targeting using the genetic model and the antibody-based approaches, in the setting of malignant myeloma and also acute leukemias.

Os indivíduos portadores de Diabetes Mellitus (DM) são no geral caracterizados como pacientes mais susceptíveis a complicações infecciosas, devido ao seu estado de imunossupressão, como processos ...bacterianos, virais e fúngicos. Dentre as fúngicas, destacam-se a Mucormicose Rinocerebral. O objetivo desse trabalho é discutir sobre a infecção por fungos Mucormicose e sua evolução em pacientes imunodeprimidos, como os diabéticos. O presente estudo foi realizado através de uma revisão de literatura, com base em artigos publicados em meio eletrônico nas bases de dados do PubMed, Medline e Scielo, utilizando os seguintes descritores: diabetes mellitus, imunossupressão e mucormicose. O paciente descompensando com cetoacidose diabética é o que frequentemente culmina no aparecimento da mucormicose. Ela pode se manifestar em diversos sítios como pulmão, trato gastrointestinal, sistema nervoso central, porém a forma mais comum de apresentação é a rinocerebral. Para iniciar a terapêutica do quadro, devemos realizar o controle da doença de base, Diabetes Mellitus, normalizando os níveis glicêmicos. É imperioso que os diabéticos tenham aderência ao tratamento correto, visto que suas complicações fomentam a suscetibilidade a diversas infeções. Reforçar a educação em saúde é um dos métodos que podem auxiliar a reduzir a alta prevalência de complicações em indivíduos diabéticos. Dessa forma, os portadores serão incentivados a cuidar de si e controlar essa condição, garantindo uma melhor qualidade de vida.
