Cryo-ablation is a common procedure used in hospitals to eliminate certain arrhythmia, such as Atrial Fibrillation. This procedure, sometimes, involves treatments in areas close to the phrenic nerve ...with the subsequent risk of later damage to the aforementioned nerve. To avoid this, clinical practice incorporates manual safety protocols during ablation. We propose the development of an automated classifier that facilitates the clinical evaluation of possible conduction disorders produced in the phrenic nerve. To achieve this goal, polygraph signals extracted during the ablation process of ten patients were used. To unmask the residue of cellular muscle potential during the phrenic nerve stimulation process we compare utilizing signal processing the results when the sensor was placed on the phrenic nerve (activation capture) and when the sensor was displaced from the phrenic nerve (no capture). A linear classifier was applied to both situations to characterize muscle activity resulting from nerve activation. The results confirmed that it is possible to automatically classify the level of muscle activity from the phrenic nerve with 100% accuracy in this data set. The method proposed in this work constitutes an automated protocol to evaluate the eventual deterioration of the phrenic nerve conduction due to ablation in the vicinity, improving the existing protocol for clinical convenience.
To study the prevalence of Delayed HIV Diagnosis (DHD) and its associated risk factors, to evaluate the effect of DHD on virological and immunological responses to HAART and to estimate the impact of ...DHD on all-causes mortality. Prospective cohort of 2, 564 HIV-positive HAART-naïve subjects attending 19 hospitals in Spain, 2004-2006. Estimations were made by logistic regression and survival analyses by Cox regression models. Prevalence of DHD was 37.3% (35.0-39.6). DHD was related to low educational level (OR:1.31, 95% CI:1.0-1.7). Compared to men who have sex with men (MSM), DHD was more frequent in heterosexuals (OR:1.9 95% CI:1.5-2.5) and injection drug users (IDUs) (OR:2.0 95% CI:1.5-2.8). An interaction between age and sex was found. Although risk of having DHD did not increase after age 30 in women, it increased linearly with age in men. No differences in virological (OR 1.2 95% CI: 0.8-1.8) and CD4 T cell (OR 1.1 95% CI: 0.7-1.8) responses to HAART were seen. The adjusted hazard ratio for death in patients with DHD was 5.2, (95% CI: 1.9-14.5). DHD is very common, especially in older men, heterosexuals and IDUs. Although we did not find differences in virological and immunological responses to HAART, we did observe higher mortality in people with DHD. Increased efforts to early diagnose HIV infection are urgently needed.

With the advent of highly active antiretroviral therapy in 1996, patients infected with HIV are now living longer and are dying from illnesses other than acquired immunodeficiency syndrome (AIDS). ...Liver disease due to chronic hepatitis C is now a leading cause of mortality among HIV-infected patients in the developed world. The prevalence of end-stage kidney or heart disease is also increasing among HIV-infected patients. For these patients, solid organ transplantation (SOT) is the only therapeutic option and HIV infection alone is not a contraindication. Accumulated experience in North America and Europe in the last few years indicates that 3- to 5-year survival in liver recipients coinfected with HIV and HCV is lower than that of HCV-monoinfected recipients. Conversely, 3- to 5-year survival of non-HCV-coinfected liver recipients and kidney recipients was similar to that of HIV-negative patients. Infections in the post-transplant period in HIV-infected recipients are similar to those seen in HIV-negative patients, although the incidence of some of them (e.g. tuberculosis and fungal infections) is higher. In the USA and Europe the number of immigrants from areas with endemic geographically-restricted infections has increased significantly in recent years. These changes in the population profile have led to an increase in the percentage of foreign-born transplant candidates and donors. Organ transplant recipients may develop endemic diseases in four ways: Transmission through the graft; de novo infection; reactivation of dormant infection; and reinfection/reactivation in a healthy graft. In foreign-born recipients, there is the possibility of endemic infections manifesting in the post-transplant period as a consequence of immunosuppression. These issues are modifying the criteria for donor selection and have also expanded pre-transplant screening for infectious diseases in both donors and transplant recipients. Some infectious diseases such as Chagas disease, endemic fungal infections, tuberculosis (which could be multidrug- or extensively drug-resistant according the origin of the recipient), leishmaniasis and other viral and parasitic diseases should always be considered in the differential diagnosis of post-transplant infections in foreign-born recipients.

. We performed a retrospective study in 411 patients followed for 24 months with a DNA sample available for genotyping. The change in CD4
T-cells count during the follow-up was considered as the primary outcome. The rs6897932 polymorphism had a minimum allele frequency (MAF) >20% and was in Hardy-Weinberg equilibrium (
= 0.550). Of 411 patients, 256 carried the CC genotype, while 155 had the CT/TT genotype. The CT/TT genotype was associated with a higher slope of CD4
T-cells recovery (arithmetic mean ratio; AMR = 1.16;
= 0.016), higher CD4
T-cells increase (AMR = 1.19;
= 0.004), and higher CD4
T-cells count at the end of follow-up (AMR = 1.13;
= 0.006). Besides, rs6897932 CT/TT was related to a higher odds of having a value of CD4
T-cells at the end of follow-up ≥500 CD4
