Scintillating synthesis: 18F‐Containing radiopharmaceuticals can be prepared by using 18Ffluoride ions to displace a sulfonate linker and release a radiotracer from a solid support (see scheme; ...EOM=ethoxymethyl). The method has allowed the synthesis of the widely used imaging agent 18Ffluoro‐2‐deoxy‐D‐glucose in good radiochemical yield and with high chemical purity.
In 1997, a community-wide outbreak of Salmonella enterica serotype Typhimurium
(S. typhimurium) infection occurred in France. The investigation included case searching and a
case-control study. A ...case was defined as a resident of the Jura district with fever or diarrhoea
between 12 May and 8 July 1997, from whom S. typhimurium was isolated in stool or blood.
One hundred and thirteen cases were identified. Thirty-three (83%) of 40 cases but only 23
(55%) of 42 community controls, matched for age and area of residence, reported eating
Morbier cheese (Odds ratio: 6·5; 95% Confidence Interval: 1·4–28·8). Morbier cheese samples
taken from the refrigerators of two case-patients and one symptom-free neighbour cultured
positive for S. typhimurium of the same phage type as the human isolates. The analysis of
distribution channels incriminated one batch from a single processing plant. These findings
show that an unpasteurized soft cheese is an effective vehicle of S. typhimurium transmission.

La douleur aiguë est le premier motif de consultation en urgence. Les étiologies fréquentes et graves sont bien connues et leur connaissance permet d’orienter rapidement la prise en charge. Il existe ...des causes plus rares et moins communes qu’il faut savoir évoquer afin d’orienter rapidement le diagnostic, de limiter les examens complémentaires et de proposer un traitement adapté.
Nous rapportons le cas d’un homme de 24 ans consultant aux urgences pour des douleurs abdominales aiguës (DAA) résistant à une automédication par paracétamol, codéine, nefopam, tramadol, phloroglucinol, esomeprazole dont les doses ne pourront pas être précisées. Les douleurs étaient diffuses, brutales, très intenses, irradiant dans les jambes et le dos. S’y associaient des vomissements suivis de deux épisodes d’hématémèse de très faible abondance. Ses antécédents étaient : colique néphrétique, ulcère gastroduodénal perforé et 2 mois auparavant un accident de la voie publique en voiture, compliqué d’hématome péri-hépatique, d’une éventration et d’un abcès digestif pour lesquels il avait été réalisé une laparotomie. Il existait également une intoxication chronique au tabac et au cannabis. À l’examen physique, le patient était apyrétique, tachycarde et normotendu. La douleur était décrite comme insupportable. Il existait une cicatrice abdominale paramédiane non suspecte à l’inspection. Le patient ne tolérait pas la position allongée avec une agitation marquée et l’abdomen n’était pas palpable de fait. Le toucher rectal était refusé par le patient. Les analyses sanguines montraient une glycémie normale, hémoglobine à 9,6g/dL, CRP à 57mg/L ; bilan hépatique, lipasémie, ionogramme et hémostase normaux. La bandelette urinaire n’avait pas pu être réalisée. Malgré l’administration de paracétamol, nefopam, morphine IV, kétamine, protoxyde d’azote, métoclopramide et esomeprazole la douleur restait très intense. Une échographie abdominale en position assise a été difficilement réalisée, ne retrouvant pas d’anomalies urologique, biliaire, hépatique et splénique. L’exploration le long de la cicatrice de laparotomie n’était pas possible. Un scanner abdominal injecté sous sédation par propofol et oxygénothérapie aux lunettes a été réalisé qui ne montrait pas d’anomalies pouvant expliquer la symptomatologie et quelques opacités pulmonaires basales droites. Finalement devant la consommation avouée ancienne et importante de cannabis il sera évoqué un syndrome d’hyperémèse cannabinoïde (SHC). Le patient a bénéficié de « bain de serviettes chaudes », permettant la résolution rapide des douleurs. La surveillance à 12h montrait une baisse spontanée de la CRP et une hémoglobinémie stable. Le patient a quitté les urgences contre avis médical ne permettant pas d’explorer les autres anomalies constatées.
Le SHC décrit depuis 2004 entraîne une errance diagnostic importante du fait de sa méconnaissance : en moyenne 7 consultations durant 9 années sont nécessaires pour que le diagnostic soit évoqué. Il fait partie des douleurs abdominales de diagnostic difficile et peut être confondu avec un syndrome des vomissements cycliques 1. Il évolue en 3 phases : une phase prodromique, une phase hyperémique et une phase de récupération avec de nombreuses atypies. Il répond à des critères diagnostic précis décrit par Simonetto et al. 2. Les symptômes les plus fréquents sont : nausées, vomissements, douleur abdominale 3. Un élément important de la phase hyperémique est la réponse au traitement par application d’eau chaude sur la peau (douche ou bain) ou de capsaïcine topique J.R Richard, 2017. Au long cours seul le sevrage en cannabis permet d’éviter les récidives et quand cela s’avère impossible un traitement de fond, comme l’haloperidol, peut être proposé. Plusieurs mécanismes physiopathologiques sont intriqués et restent encore à préciser. L’effet thérapeutique de l’eau chaude relèverait de multiples mécanismes : thermorégulation via le système hypothalamo-hypophysaire, vasomotricité ; la capsaïcine aurait également un effet vasomoteur, antiémétique et antaglique. Devant la hausse de la consommation de cannabis en France il est à craindre une augmentation de l’incidence de ce syndrome comme motif de consultation.
Les consultations pour douleurs, notamment abdominales, font partie du quotidien des médecins. À côtés des étiologies fréquentes et graves à évoquer en priorité, il faut savoir reconnaitre des maladies plus rares. Le SHC illustre bien cette difficulté et nécessite d’être connu afin d’éviter les surenchères diagnostique et thérapeutique.
Nucleolin is an abundant 70 kDa nucleolar protein involved in many aspects of ribosomal RNA biogenesis. The central region of nucleolin contains four tandem consensus RNA-binding domains (RBD). The ...two most N-terminal domains (RBD12) bind with nanomolar affinity to an RNA stem-loop containing the consensus sequence UCCCGA in the loop. We have determined the solution structure of nucleolin RBD12 in its free form and have studied its interaction with a 22 nt RNA stem-loop using multidimensional NMR spectroscopy. The two RBDs adopt the expected βαββαβ fold, but the position of the β2 strand in both domains differs from what was predicted from sequence alignments. RBD1 and RBD2 are significantly different from each others and this is likely important in their sequence specific recognition of the RNA. RBD1 has a longer α-helix 1 and a shorter β2-β3 loop than RBD2, and differs from most other RBDs in these respects. The two RBDs are separated by a 12 amino acid flexible linker and do not interact with one another in the free protein. This linker becomes ordered when RBD12 binds to the RNA. Analysis of the observed NOEs between the protein and the RNA indicates that both RBDs interact with the RNA loop
via their β-sheet. Each domain binds residues on one side of the loop; specifically, RBD2 contacts the 5′ side and RBD1 contacts the 3′.
Anti-CA125 antibodies have been used in immunoassays to quantify levels of shed antigen in the serum of patients who are under surveillance for epithelial ovarian cancer (EOC). However, there is ...currently no molecular imaging probe in the clinic for the assessment of CA125 expression in vivo. The present study describes the development of an (18)F-labeled single-chain variable fragment (scFv) for PET imaging of CA125 in preclinical EOC models. Anti-CA125 scFv was derived from MAb-B43.13 by recombinant expression of the fragment in E.coli. Fragment scFv-B43.13 was purified via immobilized metal affinity chromatography and characterized for antigen binding via immuno-staining and flow cytometry. Prosthetic group N-succinimidyl 4-(18)Ffluorobenzoate ((18)FSFB) was used for radiolabeling of scFv-B43.13. Preclinical ovarian cancer models were developed based on ovarian cancer cell lines OVCAR3 (CA125-positive) and SKOV3 (CA125-negative) in NIH-III mice. The radiopharmacological profile of (18)F-labeled scFv-B43.13 ((18)FFBz-scFv-B43.13) was studied with PET. (18)FFBz-scFv-B43.13 was prepared in radiochemical yields of 3.7 ± 1.8% (n = 5) at an effective specific activity of 3.88 ± 0.76 GBq/µmol (n = 5). The radiotracer demonstrated selective uptake in CA125-positive OVCAR3 cells and virtually no uptake in CA125-negative SKOV3 cells. Standardized uptake values (SUV) of radioactivity uptake in OVCAR3 tumors was 0.5 (n = 3) and 0.3 (n = 2) in SKOV3 tumors after 60 min post injection (p.i.).
Septic arthritis due to Mycobacterium kansasii is rare; only 40 cases have been published. A French national inquiry revealed the occurrence of 10 new cases between 1992 and 1997 (8 men and 2 women: ...mean age, 37 years; range, 25–54 years). Seven had an underlying condition: AIDS (n =4), chronic skin psoriasis and AIDS (n = 2), or a renal transplant (n = 1). Trauma to the joint, use of intra-articular corticosteroid(s) 1 month to 2 years after the event, and chronic skin psoriasis were risk factors. The mean interval between appearance of the first symptoms of arthritis and the diagnosis was 5 months. Monarthritis was localized to the knee (n = 4), wrist (n = 3), finger (n = 1), elbow (n = 1), or ankle (n = 1). The main diagnostic procedure was culture of a synovial biopsy specimen. In all cases, debridement was associated with antimycobacterial treatment. Three patients died of AIDS during treatment, and another is still undergoing treatment; the other 6 patients were cured. M. kansasii infection should be considered in all cases of indolent arthritis with any of the following risk factors: local trauma, local or systemic corticosteroid therapy, chronic skin psoriasis, and immunodepression, especially that due to human immunodeficiency virus infection.
