Purpose: Retinitis pigmentosa is the most common inherited retinal dystrophy. The factors associated with visual acuity in patients with other retinal diseases are well known, but are poorly ...understood in patients with retinitis pigmentosa. This knowledge is useful for prognosis and to support secondary endpoints in clinical trials.
Methods: We conducted a cross-sectional study of consecutive patients recruited from the inherited retinal disease service from January 2012 to December 2012. Central macular thickness (CMT) was measured using spectral domain optical coherence tomography.
Results: Data were available for 81 patients and 162 eyes. After multivariable analyses, older age, earlier age of onset of symptoms, and thicker CMT were associated with lower visual acuity. Gender and inheritance pattern were not associated with visual acuity. Each decade older age, younger age of onset, and thicker CMT was associated with 0.12, 0.10, and 0.11 worse logarithm of the minimal angle of resolution units of visual acuity, respectively (p < 0.05 for all).
Conclusions: Age, age of onset, and CMT are associated with visual acuity and important factors to measure in studies of retinitis pigmentosa.
As global climate changes, there is increasing need to understand how changes in the frequencies of environmental variability affect populations. Age-structured populations have recently been shown ...to filter specific frequencies of environmental variability, favoring generational frequencies, and very low frequencies, a phenomenon known as
cohort resonance
. However, there has been little exploration of how changes in the spectra of environmental signals will affect the stability and persistence of age-structured populations. To examine this issue, we analyzed a likely example to show how changes in the frequency of an influential climate phenomenon, the El Niño-Southern Oscillation (ENSO), could affect a marine bird population. We used a density-dependent, age-structured population model to calculate the transfer function (i.e., the frequency-dependent sensitivity) of Brandt’s cormorant (
Phalacrocorax penicillatus
), a representative marine bird species known to be influenced by ENSO. We then assessed how the population would be affected by ENSO forcing that was doubled and halved in frequency. The transfer function indicated this population is most sensitive to variance at low frequencies, but does not exhibit the sensitivity to generational frequencies (cohort resonance) observed in shorter-lived species. Doubling the frequency of ENSO unexpectedly resulted in higher mean adult population abundance, lower variance, and lower probability of extinction, compared to forcing with the historical or reduced ENSO frequency. Our results illustrate how long-lived species with environmentally driven variability in recruitment, including many species of marine birds and fish, may respond in counterintuitive ways to anticipated changes in environmental variability.
Purpose We retrospectively evaluated the accuracy of gadolinium enhanced magnetic resonance urography to detect upper urinary tract tumors. Materials and Methods A total of 91 magnetic resonance ...urography studies for suspected upper tract malignancy were done in 70 males and 18 females with a mean age of 71.7 years. Breath hold coronal T2-weighted single shot fast spin-echo and breath-hold coronal 3-dimensional T1-weighted spoiled gradient-recalled echo images with fat suppression were obtained during the nephrographic and excretory phases after intravenous injection of gadolinium based contrast material. Two radiologists independently reviewed magnetic resonance images for a tumor by 4 regions (right/left and renal collecting system/ureter). Sensitivity, specificity and accuracy were calculated. Results A total of 35 urinary tract regions in 18 males and 7 females with a mean age of 70.4 years were confirmed to have an upper tract malignant tumor and 219 urinary tract regions were confirmed to be tumor-free. Sensitivity, specificity and accuracy to detect upper urinary tract malignancy were 74.3%, 96.8% and 93.7% for reviewer 1, and 62.9%, 96.3% and 91.7% for reviewer 2, respectively. When patients with a ureteral stent or nephrostomy tube were excluded from analysis, sensitivity, specificity and accuracy were 86.2%, 99.5% and 97.7% for reviewer 1, and 72.4%, 97.9% and 94.6% for reviewer 2, respectively. Conclusions Gadolinium enhanced magnetic resonance urography is accurate to detect upper urinary tract malignant tumors.
Objective We sought to define the prevalence of definitive end-states and their determinants in children with double-outlet right ventricle. Methods We performed a clinical record review of 393 ...children with double-outlet right ventricle presenting to our institution from 1980 to 2000. Results Double-outlet right ventricle classification was as follows: subaortic ventricular septal defect with or without pulmonary stenosis in 47%, subpulmonic ventricular septal defect in 23%, noncommitted ventricular septal defect in 26%, and doubly committed ventricular septal defect in 4%. Hypoplastic ventricles were present in 39%, pulmonary stenosis was present in 65%, and aortic arch obstruction was present in 24%. Biventricular repair was performed in 194 patients (55%) at a median age of 10 months (range: birth to 14.0 years), and the Fontan operation (n = 182; 23%) was performed at a median age of 3.7 years (range: 6 months to 14.9 years). Results improved over time ( P < .001). Factors discriminating among end-states included younger patient age at presentation ( P < .001), lower weight ( P < .001), and adequacy of left-sided heart structures, especially the size of the left ventricle ( P < .001), aortic arch ( P < .001), and mitral valve ( P = .004). For complex double-outlet right ventricle, Rastelli-type repair increased early reintervention risk ( P = .04) and late post-repair mortality ( P = .02), whereas the arterial switch operation increased early post-repair mortality ( P = .02) with a benefit of improved late post-repair survival. Conclusions Biventricular repair, especially Rastelli-type reconstruction, is associated with higher late mortality and reintervention than is Fontan repair. The wisdom of extending biventricular repair to borderline anatomic candidates with hypoplastic left-sided structures or a nonsubaortic ventricular septal defect is questionable.

The rise of extremist parties and candidates in Europe, the ...United States, and around the globe has led even mainstream political commentators to begin using the term "fascism" to describe dangerous movements that have revived and repackaged many of the strategies long thought to have been relegated to the margins of political rhetoric. No longer just confined to the state regimes of the past, fascism thrives today as a globally self-augmenting, self-propagating rhetorical phenomenon with a variety of faces and expressions.The Rhetoric of Fascism defines and interprets the common persuasive devices that characterize fascist discourse to understand the nature of its enduring appeal. By approaching fascism from a rhetorical perspective, this volume complements established political and sociological understandings of fascism as a movement or regime. A rhetorical approach studies fascism less as a party one joins than as a set of persuasive strategies one adopts. Fascism spreads precisely because it is not a coherent entity. Instead, it exists as a loosely bound and often contradictory collection of persuasive trajectories that have attained enough coherence to mobilize and channel the passions of a self-constituted mass of individuals.Introductory chapters focus on general theories of fascism drawn from twentieth-century history and theory. Habitat loss and fragmentation can negatively impact native wildlife and facilitate establishment of introduced species. On islands, introduced species are a primary cause of extinction and can alter ...community membership through predation or competition for resources. Consequently, elucidating the distribution of introduced and native species can improve understanding of the potential synergistic effects of land use and introduced species on native island species. The island of Saipan in the Commonwealth of the Northern Mariana Islands has a long history of herpetofaunal introductions. However, the distribution of native and introduced herpetofauna on Saipan is poorly understood, and the effects of land use and introduced species on the native herpetofauna are largely unknown. In 2018, we randomly selected sites on Saipan from established island-wide transects and used multiple detection methods, repeated surveys, and single-season occupancy models to account for imperfect detection and construct species distribution models. We investigated the role of biotic and abiotic factors in explaining occupancy and detection of Saipan’s native and introduced herpetofauna. We recorded 2568 observations representing 1 amphibian and 11 reptile species. Introduced species were encountered 3.8 times more frequently and were 33% more broadly distributed than native species. Occupancy for the native forest dwelling species Emoia caeruleocauda and Perochirus ateles was positively associated with elevation and enhanced vegetation index, suggesting that human habitat modification at lower elevations may be reducing their suitable habitat. We found evidence of a range contraction for the imperiled P. ateles and suggest potential avenues for implementing targeted conservation of native herpetofauna.
We sought to evaluate the safety and feasibility of mild therapeutic hypothermia using an endovascular temperature management system in awake acute ischemic stroke patients. The Intravascular Cooling ...in the Treatment of Stroke (ICTuS) study was an uncontrolled, multicenter development and feasibility study of conscious patients (n = 18) presenting within 12 hours of onset of an acute ischemic stroke at 5 clinical sites in the United States. Enrolled patients were to undergo core temperature management using an endovascular cooling system to induce and maintain mild, therapeutic hypothermia (target temperature of 33.0°C) for a period of either 12 or 24 hours, followed by controlled rewarming to 36.5°C over the subsequent 12-hour period. Nine patients underwent 12 hours of cooling followed by 12 hours of controlled rewarming, and 6 patients underwent 24 hours of cooling followed by 12 hours of controlled rewarming. Three patients underwent <1.5 hours of hypothermia due to clinical or technical issues. We also developed an antishivering regimen using buspirone and meperidine administered prophylactically to suppress shivering. The endovascular cooling catheter was well tolerated, with acceptable adverse event rates. Increasing the duration of hypothermia administration from 12 hours to 24 hours did not appear to increase the incidence or severity of adverse effects. Endovascular cooling with a proactive antishivering regimen can be accomplished in awake stroke patients. Further studies are needed to establish the safety and efficacy of this approach.
Appropriate use and interpretation of serological tests for assessments of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) exposure, infection and potential immunity require accurate ...data on assay performance. We conducted a head-to-head evaluation of ten point-of-care-style lateral flow assays (LFAs) and two laboratory-based enzyme-linked immunosorbent assays to detect anti-SARS-CoV-2 IgM and IgG antibodies in 5-d time intervals from symptom onset and studied the specificity of each assay in pre-coronavirus disease 2019 specimens. The percent of seropositive individuals increased with time, peaking in the latest time interval tested (>20 d after symptom onset). Test specificity ranged from 84.3% to 100.0% and was predominantly affected by variability in IgM results. LFA specificity could be increased by considering weak bands as negative, but this decreased detection of antibodies (sensitivity) in a subset of SARS-CoV-2 real-time PCR-positive cases. Our results underline the importance of seropositivity threshold determination and reader training for reliable LFA deployment. Although there was no standout serological assay, four tests achieved more than 80% positivity at later time points tested and more than 95% specificity.
Cilia are present on cells of many eukaryotic organisms and recent data in the mouse suggest that ciliary defects can cause severe developmental abnormalities and disease. Studies across eukaryotic ...systems indicate that cilia are constructed and maintained through a highly conserved process termed intraflagellar transport (IFT), for which many of the proteins involved have yet to be identified. IFT describes the movement of large protein particles consisting of an A and a B complex along the cilia axoneme in anterograde and retrograde directions. Herein we describe a novel
C. elegans gene,
F59C6.7/9, that is required for cilia assembly and whose function is disrupted in
che-13 ciliogenic mutants. As previously shown for all IFT complex B genes identified to date, expression of
che-13 (F59C6.7/9) is regulated by the RFX-type transcription factor DAF-19, suggesting a conserved transcriptional pathway in ciliogenesis. Fluorescent-tagged CHE-13 protein concentrates at the base of cilia and moves along the axoneme as expected for an IFT protein. Furthermore, loss of
che-13 differentially affects the localization of two known IFT complex B proteins, OSM-5 and OSM-6, implying that CHE-13 functions as part of this complex. Overall, our data confirm that CHE-13 is an IFT protein and further that the IFT particle assembles in an ordered process through specific protein–protein interactions.
