Previous meta-analyses reported no significant or weak association between hyperuricemia (HU) and coronary heart disease (CHD). We updated the literature search, systematically reviewing retrieved ...papers. The peer-reviewed literature published from 1965 to December 2014 was searched using Medline and Embase. We included prospective cohort studies involving adults (sample size ≥100) with no cardiovascular disease (CVD) and a follow-up of at least 1 year. Studies were excluded if they considered as outcome the CVD incidence/mortality without separately reporting data on CHD, did not adjusted for major confounders and if the 95% confidence interval (CI) for risk ratio (RR) was not available. Relative risk or hazard ratio estimates, with the corresponding CIs, were obtained. For CHD incidence 12 populations were included (457,915 subjects 53.7% males). For CHD mortality seven populations were included (237,433 subjects 66.3% males). The overall combined RR were 1.206 (CI 1.066–1.364, p=0.003) for CHD incidence and 1.209 (CI 1.003–1.457, p=0.047) for CHD mortality, respectively. Subgroup analysis showed a marginal (incidence) and not significant (mortality) association between HU and CHD in men, but an increased risk for CHD incidence and mortality in hyperuricemic women (RR 1.446, CI 1.323–1.581, p<0.0001, and RR 1.830, CI 1.066–3.139, p=0.028, respectively). The risk markedly increases for urate concentrations >7.0 mg/dL. HU appears to increase the risk of CHD events in the general population, mainly in adult women. This finding requires, however, further investigation.
Human epididymis protein 4 (HE4) measurements in serum have been proposed for improving the specificity of laboratory identification of ovarian cancer (OC).
To critically revise the available ...literature on the comparison between the diagnostic accuracy of HE4 and carbohydrate antigen 125 (CA-125) to confirm the additional clinical value of HE4.
A literature search was undertaken on electronic databases and references from retrieved articles; articles were analysed according to predefined criteria. Meta-analyses for HE4 and CA-125 biomarkers with OR, diagnostic sensitivity, specificity, positive (LR+) and negative (LR-) likelihood ratios as effect sizes were performed.
16 articles were originally included in meta-analyses, but two for HE4 and one for CA-125 were eliminated as outliers. Furthermore, for HE4 a publication bias was detected. ORs for both HE4 (37.2, 95% CI 19.0 to 72.7, adjusted for publication bias) and CA-125 (15.4, 95% CI 10.4 to 22.8) were significant, although in a heterogeneous set of studies (p<0.0001). By combining sensitivity and specificity, the overall LR+ and LR- were 13.0 (95% CI 8.2 to 20.7) and 0.23 (95% CI 0.19 to 0.28) for HE4 and 4.2 (95% CI 3.1 to 5.6) and 0.27 (95% CI 0.23 to 0.31) for CA-125, respectively.
HE4 measurement seems to be superior to CA-125 in terms of diagnostic performance for identification of OC in women with suspected gynaecological disease. Due to the high prevalence of OC in post-menopausal women and the need for data focused on early tumour stages, more studies tailored on these specific subsets are needed.
Determination of serum soluble transferrin receptor (sTfR) is proposed to distinguish between iron-deficiency anemia and anemia of chronic disease. Here we conducted a meta-analysis of the literature ...to evaluate the diagnostic efficacy of sTfR and sTfR/log ferritin index. The meta-analysis included 18 sTfR and 10 sTfR index studies. Three sTfR index studies were, however, eliminated as outliers. The odds ratio was significant for both sTfR (22.9, 95% confidence interval CI, 9.6-55.0) and sTfR index (9.5, 95% CI, 5.0-18.1) in a heterogeneous set of studies. Meta-analysis for sensitivity, specificity, and likelihood ratios (LRs) was performed only in a subset of 10 sTfR studies. The overall sensitivity, specificity, and positive and negative LRs were 86%, 75%, 3.85, and 0.19, respectively, with an area under summary receiver operating characteristic curve of 0.912 (standard error, 0.039). Additional studies are needed to define the overall diagnostic accuracy of sTfR.
Background Highly sensitive cardiac troponin assays (hs-cTn) are not available as point-of-care (POC) measurements. As rapid testing cannot be achieved at the expense of clinical performance, there ...is an urgent need to develop and rigorously validate POC hs-cTn. Konica Minolta (KM) has recently developed a surface plasmon-field enhanced fluorescence spectroscopy-based POC hs-cTn I system. Methods We validated the analytical characteristics of the KM POC system according to the international guidelines. Results Limit of blank (LoB) and limit of detection (LoD) were 0.35 and 0.62 ng/L, respectively, hs-cTn I concentrations corresponding to a total CV of 20%, 10% and 5% were 1.5, 3.9 and 11.0 ng/L, respectively. Method comparison studies showed that KM calibration was successfully traced to higher-order references. Limit of quantitation (LoQ), i.e. the hs-cTn I concentration having a total error of measurement of ≤34%, was 10.0 ng/L. The upper reference limit (URL) for 600 healthy blood donors was calculated at 12.2 ng/L (90% confidence interval CI: 9.2-39.2), while sex-partitioned URLs were 20.6 (males) and 10.7 ng/L (females), respectively (p < 0.0001). KM assay measured hs-cTn I concentrations >LoD in 65.7% of all reference individuals, in 76.7% of males and in 54.7% of females, respectively. Conclusions The KM system joins the characteristics of POC systems to the analytical performance of hs-cTn.
In this study we evaluate the capabilities of three satellite sensors for assessing water composition and bottom depth in Lake Garda, Italy. A consistent physics-based processing chain was applied to ...Moderate Resolution Imaging Spectroradiometer (MODIS), Landsat-8 Operational Land Imager (OLI) and RapidEye. Images gathered on 10 June 2014 were corrected for the atmospheric effects with the 6SV code. The computed remote sensing reflectance (Rrs) from MODIS and OLI were converted into water quality parameters by adopting a spectral inversion procedure based on a bio-optical model calibrated with optical properties of the lake. The same spectral inversion procedure was applied to RapidEye and to OLI data to map bottom depth. In situ measurements of Rrs and of concentrations of water quality parameters collected in five locations were used to evaluate the models. The bottom depth maps from OLI and RapidEye showed similar gradients up to 7 m (r = 0.72). The results indicate that: (1) the spatial and radiometric resolutions of OLI enabled mapping water constituents and bottom properties; (2) MODIS was appropriate for assessing water quality in the pelagic areas at a coarser spatial resolution; and (3) RapidEye had the capability to retrieve bottom depth at high spatial resolution. Future work should evaluate the performance of the three sensors in different bio-optical conditions.
This study presents a first assessment of the Top-Of-Atmosphere (TOA) radiances measured in the visible and near-infrared (VNIR) wavelengths from PRISMA (PRecursore IperSpettrale della Missione ...Applicativa), the new hyperspectral satellite sensor of the Italian Space Agency in orbit since March 2019. In particular, the radiometrically calibrated PRISMA Level 1 TOA radiances were compared to the TOA radiances simulated with a radiative transfer code, starting from in situ measurements of water reflectance. In situ data were obtained from a set of fixed position autonomous radiometers covering a wide range of water types, encompassing coastal and inland waters. A total of nine match-ups between PRISMA and in situ measurements distributed from July 2019 to June 2020 were analysed. Recognising the role of Sentinel-2 for inland and coastal waters applications, the TOA radiances measured from concurrent Sentinel-2 observations were added to the comparison. The results overall demonstrated that PRISMA VNIR sensor is providing TOA radiances with the same magnitude and shape of those in situ simulated (spectral angle difference, SA, between 0.80 and 3.39; root mean square difference, RMSD, between 0.98 and 4.76 mW m
sr
nm
), with slightly larger differences at shorter wavelengths. The PRISMA TOA radiances were also found very similar to Sentinel-2 data (RMSD < 3.78 mW m
sr
nm
), and encourage a synergic use of both sensors for aquatic applications. Further analyses with a higher number of match-ups between PRISMA, in situ and Sentinel-2 data are however recommended to fully characterize the on-orbit calibration of PRISMA for its exploitation in aquatic ecosystem mapping.
An observation system integrating satellite images, in situ water parameters and hydrodynamic measurements was implemented in a tidal inlet of the Venice Lagoon (Northern Adriatic Sea, Italy). The ...experimental infrastructure was developed to autonomously investigate suspended sediment dynamics in the two channels of the Lido inlet in relation to the longshore currents in the littoral zone and the tidal circulation along the lagoon channel network. It provided time series of turbidity at the surface, water flow and acoustic backscatter, which was converted into turbidity along the vertical column during different tidal phases and meteo-marine conditions. Accurate turbidity maps were derived from Sentinel-2 (Copernicus) and Landsat 8 (NASA) satellites. Long-term in situ data from field surveys enabled the calibration and intercalibration of the instrumental setup and validation of satellite-derived products. Time series from the instrumental network were analyzed in order to evaluate the temporal variability of suspended sediment in relation to tidal phases and the different meteo-marine conditions. The integration of available datasets with satellite images also permitted the testing of the methodology for a 3-D reconstruction of the suspended sediment pattern in calm sea conditions, under the effect of the sole hydrodynamical forcing. Remotely sensed data provide a synoptic distribution of turbidity in the inlet area allowing the analysis of the surficial patterns of suspended sediment and the inferring of information on the transport processes at different spatial scales. In calm sea conditions, the results show that the transport is driven by tidal currents with a net seaward transport related to a larger export of materials from the northern basin of the Lagoon of Venice. During typical northeasterly storms, materials mobilized on the beaches and in the shoreface are transported into the inlet and distributed into the lagoon channel network, following the flood tidal currents and determining net import of materials. The multitude of information provided by this system can support research on aquatic science (i.e., numerical simulations) and address end-user community practices. The ecosystem management will also benefit operational purposes, such as the monitoring of morphological transformations, erosion processes and planning of coastal defense in the future scenarios of sea level rise. The developed approach will also help to understand how the regulation of the inlet flow introduced by the operation of the flood barriers will affect the fluxes of particles and, in the long term, the lagoon morphodynamics.
The definition and enforcement of reference measurement systems, based on the implementation of metrological traceability of patient results to higher-order (reference) methods and/or materials, ...together with a clinically acceptable level of measurement uncertainty (MU), are fundamental requirements to produce accurate and equivalent laboratory results. The MU associated with each step of the traceability chain should be governed to obtain a final combined MU on clinical samples fulfilling the requested performance specifications. MU is useful for a number of reasons: (a) for giving objective information about the quality of individual laboratory performance; (b) for serving as a management tool for the medical laboratory and
diagnostics (IVD) manufacturers, forcing them to investigate and eventually fix the identified problems; (c) for helping those manufacturers that produce superior products and measuring systems to demonstrate the superiority of those products; (d) for identifying analytes that need analytical improvement for their clinical use and ask IVD manufacturers to work for improving the quality of assay performance and (e) for abandoning assays with demonstrated insufficient quality. Accordingly, the MU should not be considered a parameter to be calculated by medical laboratories just to fulfill accreditation standards, but it must become a key quality indicator to describe both the performance of an IVD measuring system and the laboratory itself.
Background Growth differentiation factor 15 (GDF-15) is an emerging cardiovascular biomarker, and a fully automated immunoassay has recently become available. The objectives of the study were to ...identify biological and lifestyle factors affecting serum GDF-15 concentrations and derive robust reference intervals, and to estimate GDF-15 within-subject biological variation and derived indices. Methods A presumably healthy population of 533 questionnaire-screened adults was used to identify the biological and lifestyle determinants of serum GDF-15. Following stringent exclusion criteria, a final group of 173 individuals was selected to establish GDF-15 reference interval. Twenty-six healthy volunteers were enrolled in the biological variation substudy. Results Using a multiple regression model, age, B-type natriuretic peptide and C-reactive protein as well as smoking status were significantly related to serum GDF-15 concentrations. The upper reference limit (URL) for serum GDF-15 concentrations (90% confidence interval CI) was 866 ng/L (733-999 ng/L), with no sex-related difference. Although GDF-15 tended to increase with age, the weak dependence of marker from age does not justify age-related URL. The within-subject CV was 6.3% (95% CI, 4.5%-8.5%), with no sex difference in intraindividual variances. The reference change value (RCV) for GDF-15 was 23%, and two are the specimens required to ensure that the mean GDF-15 result is within ±10% of the individual's homeostatic set point. Conclusions By identifying the main factors influencing serum GDF-15 concentrations, we robustly established the URL to be applied in adult population. As intraindividual variation of GDF-15 is relatively low, monitoring longitudinal changes in its concentrations over time using RCV can be a good alternative for interpreting GDF-15 in clinical setting.
PRISMA is the Italian Space Agency’s first proof-of-concept hyperspectral mission launched in March 2019. The present work aims to evaluate the accuracy of PRISMA’s standard Level 2d (L2d) products ...in visible and near-infrared (NIR) spectral regions over water bodies. For this assessment, an analytical comparison was performed with in situ water reflectance available through the ocean color component of the Aerosol Robotic Network (AERONET-OC). In total, 109 cloud-free images over 20 inland and coastal water sites worldwide were available for the match-up analysis, covering a period of three years. The quality of L2d products was further evaluated as a function of ancillary parameters, such as the trophic state of the water, aerosol optical depth (AOD), observation and illumination geometry, and the distance from the coastline (DC). The results showed significant levels of uncertainty in the L2d reflectance products, with median symmetric accuracies (MdSA) varying from 33% in the green to more than 100% in the blue and NIR bands, with higher median uncertainties in oligotrophic waters (MdSA of 85% for the entire spectral range) than in meso-eutrophic (MdSA of 46%) where spectral shapes were retained adequately. Slight variations in the statistical agreement were then noted depending on AOD values, observation and illumination geometry, and DC. Overall, the results indicate that water-specific atmospheric correction algorithms should be developed and tested to fully exploit PRISMA data as a precursor for future operational hyperspectral missions as the standard L2d products are mostly intended for terrestrial applications.
