Objectifs
L’objectif de l’étude a été d’évaluer l’impact de l’utilisation d’un dispositif de massage cardiaque externe (MCE) automatique (ceinture thoracique CT Autopulse ™) sur la pression ...artérielle (PA) au cours de la réanimation spécialisée de l’arrêt cardiaque (AC) extrahospitalier, en particulier sur la PA diastolique, déterminant de la perfusion coronaire.
Matériel et méthodes
Il s’agit d’une étude prospective bicentrique ayant inclus des patients adultes présentant un AC prolongé. Les PA systoliques, diastoliques et moyennes (PAS, PAD, PAM) procurées par un MCE manuel ont été comparées à celles obtenues par MCE automatique (mesures de PA invasives). Le critère principal a été la différence entre les valeurs des PAD procurées par MCE manuel et par la CT.
Résultats
L’analyse a été effectuée pour 29 patients. La PAD médiane était augmentée après mise en route d’Autopulse™ de 17 mmHg 11–25 à 23 mmHg 18–28 (
p
< 0,001). La PAS était augmentée de 72 mmHg 55–105 à 106 mmHg 78–135 (
p
= 0,02). La PAM était augmentée de 29 mmHg 25–38 à 36 mmHg 30–15 (
p
= 0,002). En revanche, on n’observait pas d’augmentation de l’ETCO
2
sous Autopulse ™(21 mmHg 13–36 vs 22 mmHg 12–35,
p
= 0,80).
Conclusion
Ces résultats montrent que l’utilisation de la CT améliore de façon significative la PA au cours de la réanimation de l’AC, notamment la PAD, témoin de la perfusion coronaire. En attendant la démonstration d’un éventuel bénéfice en termes de survie, cet accroissement de la PAD et de la PAM est prometteur.

Toute crise d’asthme aigu grave (CAAG) impose une prise en charge médicale préhospitalière spécialisée par un Smur.

En dehors des critères cliniques, la gravité d’une CAAG repose avant tout sur ...l’existence d’un débit expiratoire de pointe (DEP, ou peak-flow) inférieur à 30 % de la valeur prédite par abaque ou de la valeur de référence du patient.

Le traitement initial associe des nébulisations continues de bêta2-mimétiques et de bromure d’ipratropium, sous oxygène, et une injection de corticoïdes.

Devant une crise réfractaire, sont recommandés en deuxième intention le sulfate de magnésium, les bêta2-mimétiques en perfusion au pousse-seringue électrique, ou de préférence l’adrénaline en cas de choc associé.

En cas de recours à la ventilation mécanique, il faut utiliser de petits volumes courants, une fréquence basse et prolonger le temps expiratoire.

Nous rapportons un cas original d’érythème pigmenté fixe (EPF) au fulvestrant, confirmé par patch-tests. Le fulvestrant est un agoniste des récepteurs aux œstrogènes, indiqué chez les patients ...atteints de cancer du sein métastatique hormonodépendant. Aucun cas d’EPF au fulvestrant n’a été rapporté dans la littérature.
Une femme de 69 ans était traitée depuis septembre 2016 par injections sous-cutanées de fulvestrant et dénosumab (anticorps monoclonal diminuant la résorption osseuse) tous les 15jours en alternance dans le cadre d’un cancer du sein métastatique. En janvier 2017, elle a présenté des lésions érythémateuses, prurigineuses, et bien délimitées du tronc, avec pigmentation séquellaire, évocatrice d’EPF à l’examen clinique. Les lésions récidivaient régulièrement, évoquant l’imputabilité du fulvestrant ou du dénosumab. Il n’y avait pas d’autre médicament récemment introduit. La rémission des lésions était observée après arrêt des deux médicaments. Des explorations allergologiques étaient effectuées en juin 2017. Les patch-tests au fulvestrant à la concentration de 250mg/mL étaient positifs sur peau saine (dos) et sur peau lésée (région lombaire) à la lecture à 48h et 96h. Les patch-tests au dénosumab à la concentration de 70mg/mL étaient négatifs sur peau saine et peau lésée. L’examen histologique des tests positifs au fulvestrant mettait en évidence un infiltrat lichénoïde avec nécroses kératinocytaires. Le diagnostic d’EPF au fulvestrant était retenu. Une déclaration en pharmacovigilance était effectuée. Aucune récidive des lésions n’était constatée après reprise du dénosumab et remplacement du fulvestrant par l’exemestane (inhibiteur de la synthèse des œstrogènes).
L’EPF est une forme peu fréquente de toxidermie. Il s’agit du premier cas décrit d’EPF au fulvestrant, confirmé par explorations allergologiques et analyse histologique.
Estimer l’adéquation entre le désir de réanimation exprimé par des sujets âgés pris en charge en préhospitalier en situation d’urgence vitale et la prise en charge proposée par l’équipe médicale.
...Étude prospective observationnelle.
Tous les patients de plus de 80ans pris en charge en préhospitalier pour une détresse vitale ont été inclus, sauf en cas de barrière de langue ou d’état clinique incompatible avec la réponse et d’absence de famille sur place. Les résultats portant sur l’évaluation de la qualité de vie et sur les préférences des patients ont été comparés aux réponses fournies en aveugle par l’équipe médicale sur place.
Cinquante-cinq patients ont été inclus. La qualité de vie (échelle de 0 à 10) estimée par les patients était de 7 (5–10) et celle estimée par le médecin de 7 (6–8) (p=0,69). Trente-six patients (65 %) étaient favorables à une prise en charge en réanimation, tandis que l’équipe proposait une telle prise en charge chez 44 patients (80 %) (p=0,01). Parmi les 14 patients non favorables à la réanimation, 11 auraient été proposés en réanimation par l’équipe. En analyse multivariée, l’âge (OR : 1,55 1,04–2,32, p=0,03) et les antécédents neurologiques (OR : 11,91 5,68–>100, p=0,04) étaient associés à l’existence de discordances entre patients et médecins.
Il existe une fréquente inadéquation entre les préférences des patients et les soins proposés par l’équipe. Ces résultats soulignent la nécessité d’anticiper et de recueillir les préférences des patients dès la phase préhospitalière.
To estimate the adequacy between elderly patients’ preference for ICU care when treated for a life-threatening pathology, and the strategy proposed by the medical team on scene.
Prospective, observational study.
All patients older than 80 treated out-of-hospital for a life threatening pathology were included, except in case of language barrier, or when patients were unable to answer and absence of next-of-kin. The results of the questionnaire on quality of life and patients’ preference concerning ICU care were compared to the responses provided blindly by the medical team.
Fifty-five patients were included. Quality of life as expressed by the patients was 7 (5–10) and by the physician 7 (6–8) (P=0.69). Thirty-six patients (65%) expressed the wish to be resuscitated, while ICU admission would have been proposed for 44 patients (80%) by the doctors (P=0.01). Among the 14 patients reluctant to ICU admission, 11 would have been proposed for ICU admission. In multivariate analysis, age (OR: 1.55 1.04–2.32, P=0.03) and history of neurological pathology (OR: 11,91 5.68–>100, P=0.04) were associated with such an inadequacy.
The inadequacy between elderly patients’ preferences and doctors’ opinion concerning ICU cares is frequent. The present results support a more systematic collection of patients’ preferences when treated on scene for a life-threatening pathology.
Nonlinear quasi-static field problems are generally solved by means of the finite-element method coupled with some classical time-stepping technique, such as the Crank-Nicolson iterative scheme, for ...solution in the time domain. At each time step, the nonlinearity must be treated iteratively, and a new linear system must be solved every iteration, which is costly in terms of computation time. Here, a novel algorithm based on transmission-line modeling is proposed to deal both with the nonlinearity and solution in the time domain. The underlying idea is the analogy that exists between the finite-element matrix and the node-admittance matrix of an equivalent network where resistors and capacitors hold the material properties. The nonlinear material is replaced by a fictitious linear and homogeneous one, which permits a global stiffness matrix to remain unchanged throughout the iterative process. The emerging time-stepping scheme is precisely the same as the Crank-Nicolson scheme, and so is the accuracy. Two numerical examples demonstrate the efficiency of the method.
This paper expands on a hybrid numerical method coupling the finite element method (FEM) and boundary integral equation method (BIEM), developed to calculate magnetic fields in a switched reluctance ...motor (SRM). This hybrid approach is a solution to the issues stemming from the small size and odd shape of the SRM airgap, where meshing an FEM grid is difficult. In the past, the stator and rotor FEM domains kept their actual shapes, which resulted in problems with the definition of the unit outward normal vector n/spl I.oarr/ at the corner points of the SRM poles. Therefore, the normal derivative of the magnetic vector potential, i.e., the tangential component of the flux density, was undefined at these points. In this paper, a new approach with new definitions of the stator and rotor FEM domains is presented. The new concept defines the SRM airgap as a ring in which circular boundaries permit the correct definition of the normal derivative quantities and exploit the natural Neumann condition. The electromagnetic parameters of a 7.5 kW SRM prototype machine were calculated using this new approach. They compare favorably with test data. Phase magnetization characteristics also have good precision and smoothness. These electromagnetic parameters were then used to assess and compare two different SRM models for the calculation of the torque on the motor shaft, the linear trapezoidal model and the nonlinear model. The comparison between the measured torque and the torque as calculated with the two different models shows that the more simple trapezoidal model should be satisfactory in many cases and could, in particular, be used in automated SRM designs.
Nonlinear magnetostatic analysis generally involves an iterative approach such as direct iteration, the Newton-Raphson method, or a combination of both. However, those techniques always require the ...solution of a new linear system at each iteration that is CPU time consuming. Some acceleration can be carried out in order to partially overcome this drawback, but this may lead to a loss of convergence. In this paper, it is shown that the 2-D nonlinear magnetostatic problem may be solved by using the transmission-line modeling (TLM) method that is usually exploited for the solution of electrical networks. The fundamental mechanism of this method is the modeling of the nonlinearity as fictitious sources so that the stiffness matrix of the problem remains unchanged at all iterations. Substantial reduction in execution time is obtained with excellent convergence properties. Numerical results are presented, and a comparison to the classical Newton-Raphson algorithm points out the efficiency of our method.
