Abstract Periprosthetic joint infection is a morbid and costly complication of total knee arthroplasty. Treatment options vary depending on chronicity of the infection, causative organism, and host ...factors. Some authors advocate single-stage exchange arthroplasty to decrease patient morbidity and healthcare utilization costs. Due to its proven efficacy for infection eradication and soft tissue healing, however, two-stage exchange arthroplasty remains the gold standard for treatment of periprosthetic joint infection after total knee arthroplasty. In this review, we present the technique of two-stage exchange arthroplasty and evidence supporting its use.
Abstract The roles and responsibilities of anesthesiology core program directors have evolved, in part because the Anesthesiology Residency Review Committee of the Accreditation Council for Graduate ...Medical Education no longer requires that the department chair also serve in this role. We reviewed several core anesthesiology program director academic and demographic characteristics including age, academic rank, gender, duration of service, board certification and re-certification status, and whether the program director also serves as department chair. Anesthesiology core residency program directors range in age from 33 to 74 years, with a median of 52 years. Thirty-seven (28%) program directors are women. The majority (67%) have senior academic rank (professor or associate professor). The median appointment duration is 3.7 years. The core residency program director currently also serves as department chair in 24 of the 131 (18.3%) programs.
We show that p-mode frequency variations correlate remarkably well with the variations of six solar activity indices over a 6 yr period from 1984 October to 1990 November, including both the large ...variation from solar minimum to solar maximum and smaller variations observed over approximately 1 month intervals during solar maximum. The quality of correlation as seen visually and as measured by two statistical tests differs significantly among the six activity indices, and we briefly speculate on possible reasons for this. Observations used in this study come from the HAO/NSO Fourier tachometer (FTACH) and include the spherical harmonic degree range l greater than 20 and equal to 60 or less and frequency range between 2600 and 3200 micro-Hz. The data are divided into 18 separate epochs with time string duration ranging from a minimum of 18 days to a maximum of 45 days. We have particularly good coverage during the early part of solar maximum of cycle 22.
Abstract Posterior reversible encephalopathy syndrome refers to a neuroradiologic disorder in which seizure activity (multiple seizures are more common than single events) is commonly the initial ...presenting symptom. We describe a case of posterior reversible encephalopathy syndrome in a previously healthy parturient who presented to the labor and delivery suite with generalized tonic-clonic seizures. Prompt recognition and treatment of this potentially catastrophic disease may avert injury to the patient and neonate.
Many papers have addressed the technical aspects of free tissue transfer in head and neck cancer patients. However, there has not been a critical assessment of the impact of free tissue transfer on ...resource utilization and patient morbidity compared with pedicle flap reconstructions. Two cohorts of patients derived from 245 consecutive reconstructions were tightly matched by age, site, stage, and histology, yielding 44 patient pairs differing in method of reconstruction. Patients undergoing free flap reconstruction spent more time in the operating room than those reconstructed with pedicled flaps (993 min vs. 777 min, P < 0.0001). The group with free flap reconstruction spent fewer days in the surgical intensive care unit and hospital (2 days vs. 2.5 days; 18.5 days vs. 22.6 days). This difference is attributed to the paucity of postoperative complications in the group with free flap reconstruction (fistula formation 4.5% vs. 21%, P < 0.04). These data indicate that the continued use of sophisticated reconstructive techniques in head and neck cancer patients is economically sound as assessed by patient morbidity and resource utilization.

The Medical Outcomes Study 36-item short form (SF-36) is widely used to compare the health-related quality of life of general and specific populations, estimate the burden of disease and examine the ...benefits of treatment interventions. Studies have shown that SF-36 is reliable and able to detect differences between groups defined by age and sex. However, few reports used repeated measures of the SF-36 in populations other than patients. Hopman et al assess mean changes in SF-36 scores over a 3-year period in men and women between the ages of 40 and 59 years at baseline.
