Se estudiaron 120 pacientes adultos con anemia drepanocítica que por diferentes causas ingresaron en el Instituto de Hematología e Inmunología durante los años 1996-1997. En 16 (13,3 %) de estos ...enfermos se encontraron los criterios diagnósticos de la crisis de secuestro hepático. Hubo disminución significativa de la cifra de hemoglobina (p<0,01) y aumento significativo de la hepatomegalia (p<0,01), el incremento de la reticulocitosis no fue estadísticamente significativo. Los síntomas clínicos más frecuentes fueron la fiebre (62,5 %) y la crisis vasooclusiva (50 %). Las variables que miden la función hepática no se modificaron significativamente durante la crisis. Los hallazgos histológicos más frecuentes fueron la dilatación de los sinusoides hepáticos con hematíes falciformados y la hiperplasia de las células de Kupffer con eritrofagocitosis. Estos resultados indican que la crisis de secuestro hepático es una complicación relativamente frecuente y que está relacionada con atrapamiento agudo de hematíes falciformados en los sinusoides hepáticos

La proteinosis alveolar pulmonar (PAP) es un raro trastorno caracterizado por la acumulación de material proteináceo en los alvéolos pulmonares de los individuos afectados. En pacientes con ...enfermedades hematológicas se ha reportado una incidencia de PAP de 5,3 %, de los cuales el 10 % corresponde a trastornos en las líneas granulopoyéticas. Esta entidad se ha comunicado también en el curso de las leucemias mieloides crónicas (LMC). En este trabajo comunicamos 2 casos de pacientes hematológicos, el primero de una paciente femenina de 54 años con una LMC fallecida por un cuadro de insuficiencia respiratoria; el segundo una paciente de 49 años portadora de una aplasia medular posterior a tratamiento con cloranfenicol, que presentó síntomas de infecciones respiratorias a repetición hasta su ingreso final y fallecimiento. Ambas pacientes se diagnosticaron en el estudio necrópsico. Se realizó revisión de la literatura

The diagnostic criteria of the hepatic sequestration crisis were found in 16 (13,3 %) of these individuals. A significant decrease of the figure of hemoglobin (p<0,01) and a marked increase of hepatomegaly (p < 0,01) were observed. The rise of reticulocytosis was not statistically important. The most frequent clinical symptoms were fever (62,5 %) and vasoocclusive crisis. The variables that measure the hepatic function were not significantly modified, during the crisis. The commonest histological findings were the dilatation of the hepatic sinusoids with falciform erythrocytes and hyperplasia of the kupffer cells with eritrophagocytosis. These results show that the hepatic sequestration crisis is a relatively frequent complication and it is related to acute catching of falciform erythrocytes in the hepatic sinusoids

Controlling the global COVID-19 pandemic depends, among other measures, on developing preventive vaccines at an unprecedented pace. Vaccines approved for use and those in development intend to elicit ...neutralizing antibodies to block viral sites binding to the host’s cellular receptors. Virus infection is mediated by the spike glycoprotein trimer on the virion surface via its receptor binding domain (RBD). Antibody response to this domain is an important outcome of immunization and correlates well with viral neutralization. Here, we show that macromolecular constructs with recombinant RBD conjugated to tetanus toxoid (TT) induce a potent immune response in laboratory animals. Some advantages of immunization with RBD-TT conjugates include a predominant IgG immune response due to affinity maturation and long-term specific B-memory cells. These result demonstrate the potential of the conjugate COVID-19 vaccine candidates and enable their advance to clinical evaluation under the name SOBERANA02, paving the way for other antiviral conjugate vaccines.

We present photometric light curves of the stellar occultation event of the star UCAC4 341-187633 on 2018 August 15 by the minor planet (134340) Pluto. Photometric observations were carried ...out using the 2.1-m telescope at the San Pedro Mártir Observatory and the 1.3-m telescopes at Sites 2 and 3 of the Trans-Neptunian Automated Occultation Survey (TAOS II) project, and using a portable 0.4-m telescope from Bahía Asunción, Baja California Sur, Mexico. Different filters were used with the 2.1-m telescope and the TAOS II telescopes, whilst observations with the portable system were performed with no filter. The resulting light curves from the San Pedro Mártir Observatory show clear structures, with at least two bright spikes observed on ingress and one more observed on egress as the star traverses the atmosphere of the dwarf planet. The light curve from the portable telescope (440 km away) measured a longer duration for the occultation event, because the shadow of Pluto was observed at a lower latitude. Normalized light curves were created for the 2.1-m telescope, the Site 3 telescope of the TAOS II and the portable telescope. These normalized light curves show a difference in amplitude.

To develop updated guidelines for the pharmacological management of rheumatoid arthritis (RA).
A group of experts representative of different geographical regions and various medical services ...catering to the Mexican population with RA was formed. Questions based on Population, Intervention, Comparison, and Outcome (PICO) were developed, deemed clinically relevant. These questions were answered based on the results of a recent systematic literature review (SLR), and the evidence's validity was assessed using the GRADE system, considered a standard for these purposes. Subsequently, the expert group reached consensus on the direction and strength of recommendations through a multi-stage voting process.
The updated guidelines for RA treatment stratify various therapeutic options, including different classes of DMARDs (conventional, biologicals, and JAK inhibitors), as well as NSAIDs, glucocorticoids, and analgesics. By consensus, it establishes the use of these in different subpopulations of interest among RA patients and addresses aspects related to vaccination, COVID-19, surgery, pregnancy and lactation, and others.
This update of the Mexican guidelines for the pharmacological treatment of RA provides reference points for evidence-based decision-making, recommending patient participation in joint decision-making to achieve the greatest benefit for our patients. It also establishes recommendations for managing a variety of relevant conditions affecting our patients
Desarrollar guías actualizadas para el manejo farmacológico de la artritis reumatoide (AR).
Se conformó un grupo de expertos que fueran representativos de las distintas regiones geográficas y los diferentes servicios médicos que atienden a la población mexicana con A. Se desarrollaron preguntas basadas en Población, Intervención, Comparación y Desenlace Outcome (PICO) que fueron consideradas relevantes desde el punto de vista clínico; las preguntas encontraron su respuesta en los resultados de una revisión sistemática de la literatura (RSL) reciente y la validez de la evidencia fue evaluada mediante el sistema GRADE, considerado un estándar para estos fines. Posteriormente el grupo de expertos desarrollaró acuerdo en la dirección y fuerza de las recomendaciones mediante un proceso de votación en distintas etapas.
Las guías actualizadas para el tratamiento de AR y categorizan en forma estratificada a las distintas opciones terapéuticas incluyendo las distintas familias de FARME (convencionales, biológicos e inhibidores de JAK), además de AINE, glucocorticoides y analgésicos. Establece por consenso el uso de todos ellos en distintas subpoblaciones de interés de pacientes con AR y aborda además aspectos relacionados con vacunación, COVID-19, cirugía, embarazo y lactancia y otros.
La presente actualización de las guías mexicanas para el tratamiento farmacológico de la AR brinda elementos de referencia en la toma de decisiones basados en la evidencia científica más reciente y recomienda la participación del paciente para la toma de decisiones conjuntas en la búsqueda del mayor beneficio de nuestros pacientes; establece además recomendaciones para el manejo de una diversidad de condiciones relevantes que afectan a nuestros pacientes.
