Se evaluaron 43 pacientes adultos con leucemia mieloide crónica, Philadelphia positivo, que recibieron tratamiento con mesilato de imatinib como droga de segunda línea por resistencia o intolerancia ...al interferón alfa recombinante. La manifestación más frecuente al inicio de la enfermedad fue la esplenomegalia. El tratamiento con mesilato de imatinib se inició por resistencia (33; 76,7 %) o intolerancia grado 3 o 4 (10; 23,3 %). El mayor porcentaje de respuesta citogenética mayor (22; 91,7 %) y completa (11; 61,1 %) se alcanzó a los 18 y 24 meses de evolución. El 74,3 % no mostró respuesta molecular y el 5,1 % ya presentaba respuesta molecular antes del tratamiento; 9 (26,5 %) mostraron pérdida de la remisión hematológica completa, de ellos, 7 fallecieron por progresión de la enfermedad. La sobrevida global fue de 90,7 %, 83,3 %, 82,6 % y 78,9 % a los 5, 6, 7 y 8 años de evolución, respectivamente. La sobrevida global y libre de eventos a los 3 años de iniciado el mesilato de imatinib fue de 92,3 % y 81,8 %, respectivamente. Se encontró diferencia significativa entre la sobrevida libre de eventos y el índice pronóstico de Sokal. Las reacciones clínicas secundarias más frecuentes fueron dolores óseos, musculares o ambos; y las hematológicas: anemia hemolítica autoinmune y trombocitopenia.Forty three patients presenting with chronic positive-Philadelphia myeloid leukemia were assessed treated with Imatinib Mesilate as a second line drug by resistance or intolerance to recombinant alpha Interferon. At onset, the more frequent manifestation of this condition was the splenomegalia. Imatinib Mesilate treatment was started by resistance (33; 7.6 %) or 3 or 4 degree intolerance (10; 23.3 %). The greater percentage of cytogenetic response (22; 91.7 %) and complete (11; 61.1 %) was achieved at 18 and 24 course months. The 74.3 % hadn't "molecular response and the 5.1 % yet had it before treatment; 9 (26.5 %) showed a loss of complete hematologic remission, from them, 7 deceased from disease progression. Global survival was of 90.7 %, 83.3 %, 82.6 % and 78.9 % at 5, 6, 7 and 8 course years, respectively. Global survival and free of events at three years of Imatinib Mesilate appearance was of 92.3 % and 81.8 %, respectively. There was a significant difference between survival free of events and the Sokal prognostic rate. The more frequent secondary clinical reactions were the bone and/or muscular pain and the hematologic ones were the autoimmune hemolytic anemia and the thrombocytopenia.

En 18 pacientes el donante fue un hermano HLA idéntico y en 1 caso un hermano ...haploidéntico. La edad media fue de 28,3 años. Dieciséis pacientes fueron del sexo masculino y 3 del femenino. La media del número de células de médula ósea infundidas fue de 3,05 x 10(8) /kg de peso y del número de unidades formadoras de colonias gránulomonocíticas fue de 12,79 x 10(4)/kg de peso. La media de recuperación de neutrófilos fue de 18,3 días y de las plaquetas 20,8 días. No se encontró correlación significativa entre el número de células de médula ósea infundidas y el día de la recuperación hematopoyética, lo que tampoco se correlacionó con el número de unidades formadoras de colonias gránulomonocíticas, ni con la existencia de enfermedad injerto contra huésped19 patients with hematologic diseases that underwent allogeneic bone marrow transplantation were evaluated. The average recovery of neutrophils was 18.3 days and that of platelets 20.8 days. No significant correlation was found between the number of bone marrow cells infused and the day of the hematopoietic recovery. This was not correlated either with the number of granulocyte-monocyte colony forming units or with the existance of graft vs. host disease

We reviewed the role of OS and its downstream signaling in aging eyes.
A search of the literature and current knowledge on the physiological and pathological mechanisms of OS were revisited in relation to the eyes and the aging process. Most prevalent ocular diseases have been analyzed herein in relation to OS and nutraceutic supplements, such as dry-eye disorders, glaucoma, age-related macular degeneration, and diabetic retinopathy.
Clinical, biochemical, and molecular data from anterior and posterior eye segment diseases point to OS as the common pathogenic mechanism in the majority of these ocular disorders, many of which are pathologies causing visual impairment, blindness, and subsequent loss of life quality. Studies with nutraceutic supplements in aging eye-related pathologies have also been reviewed.
OS, nutritional status, and nutraceutic supplements have to be considered within the standards of care of older ophthalmologic patients. OS biomarkers and surrogate end points may help in managing the aging population with ocular diseases.
Bacterial infections may complicate the course of COVID-19 patients. The rate and predictors of bacterial infections were examined in patients consecutively admitted with COVID-19 at one tertiary ...hospital in Madrid between March 1st and April 30th, 2020. Among 1594 hospitalized patients with COVID-19, 135 (8.5%) experienced bacterial infectious events, distributed as follows: urinary tract infections (32.6%), bacteremia (31.9%), pneumonia (31.8%), intra-abdominal infections (6.7%) and skin and soft tissue infections (6.7%). Independent predictors of bacterial infections were older age, neurological disease, prior immunosuppression and ICU admission (
p
< 0.05). Patients with bacterial infections who more frequently received steroids and tocilizumab, progressed to lower Sap02/FiO2 ratios, and experienced more severe ARDS (
p
< 0.001). The mortality rate was significantly higher in patients with bacterial infections as compared to the rest (25% vs 6.7%, respectively;
p
< 0.001). In multivariate analyses, older age, prior neurological or kidney disease, immunosuppression and ARDS severity were associated with an increased mortality (
p
< 0.05) while bacterial infections were not. Conversely, the use of steroids or steroids plus tocilizumab did not confer a higher risk of bacterial infections and improved survival rates. Bacterial infections occurred in 8.5% of patients hospitalized with COVID-19 during the first wave of the pandemic. They were not independently associated with increased mortality rates. Baseline COVID-19 severity rather than the incidence of bacterial infections seems to contribute to mortality. When indicated, the use of steroids or steroids plus tocilizumab might improve survival in this population.
The Andalusian Registry of Pregnancies in patients with multiple sclerosis is the largest Spanish registry on multiple sclerosis (MS) and family planning. For the first time, it includes information ...on the fertility of men with MS. The influence of the use of a disease-modifying treatment (DMT) on the health of the foetus/newborn and the impact of breastfeeding on MS are also analysed.
This is a multicentre, prospective and observational study. Recruitment of patients took place between December 2018 and December 2020. Women were followed up for one year after delivery. Altogether 100 women and 16 men were included, with a total of 103 newborn infants.
The annualised relapse rate of the women with MS decreased significantly during pregnancy (from 0.23 to 0.065). A total of 11.2% of patients resorted to assisted reproductive techniques in order to conceive a child. No association was found between the use of a DMT at conception and/or pregnancy and the risk of miscarriage, prematurity or low birth weight. Over half the women with MS (54.2%) chose to breastfeed (26.7% of them while on a DMT).
MS does not affect the fertility of men. Neither does the use of a DMT at the time of conception affect their fertility or their children's health. Assisted reproductive techniques did not have a negative impact on the course of MS. Breastfeeding is a common practice among women with MS and there is no evidence of positive or negative effects on disease progression.
Zika virus disease is a challenge for public health due to its rapid spread and potential foetal complications. Although it is imported in Spain, there is a risk of autochthonous transmission due to ...Aedes albopictus presence. Zika disease and congenital cases have been under surveillance since 2016. The objective of this study is to explore the epidemiology of disease and pregnancies result.
A descriptive study was carried out into cases reported to the National Surveillance Network (RENAVE) during the 30/11/2015 to 31/12/2017 period. The case definition and the survey are included in the RENAVE protocol. The variables were: date; notifying region (Autonomous Community (AC)); pregnancy and its evolution; case classification; mode of transmission; country or region of infection; socio-demographical, clinical and microbiological data. A descriptive analysis of the cases and their distribution according to the other variables was carried out.
A total of 512 cases were reported by 17 ACs. 507 were non-congenital, of which 327 (64.5%) were women (52.5% of childbearing age). 403 cases (79.5%) corresponded to 2016 and 193 (38.1%) resided in regions with A. albopictus presence between May and October. 96.1% of imported cases were infected in America (51.7% while visiting relatives). Three cases (3.9%) of congenital Zika virus infection were detected among 77 pregnant women.
The evolution of reported cases was in accordance with that of the epidemic in America. The largest group of travellers was young women who travelled to Latin America on family visits. Pregnancy monitoring resulted in the identification of Zika related foetal complications.
La enfermedad por virus Zika constituye un reto para la salud pública por su rápida expansión y sus potenciales complicaciones fetales. En España la enfermedad por virus Zika es importada, aunque la presencia de Aedes albopictus no descarta su transmisión autóctona. La enfermedad por virus Zika y los casos congénitos se vigilan desde 2016. El objetivo es conocer su epidemiología y el resultado de la gestación en las embarazadas.
Estudio descriptivo de casos notificados a la Red Nacional de Vigilancia Epidemiológica (RENAVE) entre el 30/11/2015 y el 31/12/2017. La definición de caso y la encuesta se recogen en los protocolos de la RENAVE. Las variables fueron: fecha, comunidad autónoma (CC.AA.), clasificación, modo y lugar de infección, variables sociodemográficas, clínicas, microbiológicas, embarazo y su evolución. Se realizó un análisis descriptivo de los casos y su distribución según el resto de variables.
Diecisiete CC.AA. notificaron 512 casos, de los cuales 507 fueron no congénitos: 327 (64,5%) eran mujeres (52,5% en edad fértil); 403 casos (79,5%) correspondieron a 2016 y 193 (38,1%) residían en CC.AA. colonizadas por A. albopictus entre mayo y octubre. El 96,1% de casos importados se infectaron en América y el 51,7% en visitas familiares. Se detectaron 3 casos de infección congénita en 77 embarazadas.
La evolución de notificaciones mostró paralelismo con la de la epidemia en América. El grupo más numeroso de viajeros fue el de mujeres jóvenes que fueron a Latinoamérica en visitas familiares. El seguimiento de las embarazadas permitió identificar complicaciones fetales.
