Detailed angle and energy resolved measurements of positrons ejected from the back of a gold target that was irradiated with an intense picosecond duration laser pulse reveal that the positrons are ...ejected in a collimated relativistic jet. The laser-positron energy conversion efficiency is ∼2×10{-4}. The jets have ∼20 degree angular divergence and the energy distributions are quasimonoenergetic with energy of 4 to 20 MeV and a beam temperature of ∼1 MeV. The sheath electric field on the surface of the target is shown to determine the positron energy. The positron angular and energy distribution is controlled by varying the sheath field, through the laser conditions and target geometry.
Abstract
We present a blind time-delay strong lensing (TDSL) cosmographic analysis of the doubly imaged quasar SDSS 1206+4332 . We combine the relative time delay between the quasar images, Hubble ...Space Telescope imaging, the Keck stellar velocity dispersion of the lensing galaxy, and wide-field photometric and spectroscopic data of the field to constrain two angular diameter distance relations. The combined analysis is performed by forward modelling the individual data sets through a Bayesian hierarchical framework, and it is kept blind until the very end to prevent experimenter bias. After unblinding, the inferred distances imply a Hubble constant H0 = 68.8$^{+5.4}_{-5.1}$ km s−1 Mpc−1, assuming a flat Λ cold dark matter cosmology with uniform prior on Ωm in 0.05, 0.5. The precision of our cosmographic measurement with the doubly imaged quasar SDSS 1206+4332 is comparable with those of quadruply imaged quasars and opens the path to perform on selected doubles the same analysis as anticipated for quads. Our analysis is based on a completely independent lensing code than our previous three H0LiCOW systems and the new measurement is fully consistent with those. We provide the analysis scripts paired with the publicly available software to facilitate independent analysis (footnote with link to www.h0licow.org). The consistency between blind measurements with independent codes provides an important sanity check on lens modelling systematics. By combining the likelihoods of the four systems under the same prior, we obtain H0 = 72.5$^{+2.1}_{-2.3}$ km s−1 Mpc−1. This measurement is independent of the distance ladder and other cosmological probes.
Fracture liaison services (FLS) have been demonstrated to improve outcomes following osteoporotic fracture. The aim of this systematic literature review (SLR) was to determine the characteristics of ...an FLS that lead to improved patient outcomes. We conducted a SLR, including articles published between 2000 and February 2017, using global (Medline, EMBASE, PubMed and Cochrane Library) and local databases. Studies including patients aged ≥ 50 years with osteoporotic fractures enrolled in an FLS were assessed. Information extracted from each article included key person coordinating the FLS (physician, nurse or other healthcare professional), setting (hospital vs community), intensity (single vs multiple), duration (long vs short term), fracture type and gender. A meta-analysis of randomised controlled trials was conducted based on the key person coordinating the FLS. Out of 7236 articles, 57 were considered to be high quality and identified for further analysis. The SLR identified several components which contributed to FLS success, including multidisciplinary involvement, driven by a dedicated case manager, regular assessment and follow-up, multifaceted interventions and patient education. Meta-analytic data confirm the effectiveness of an FLS following an osteoporotic fracture: approximate 27% increase in the likelihood of BMD testing and up to 21% increase in the likelihood of treatment initiation compared with usual care. The balance of evidence indicates that the multifaceted FLS and dedicated coordination are important success factors that contribute to effective FLS interventions which reduce fracture-related morbidity and mortality.

Patient characteristics, disease manifestations and causative pathogens were summarized descriptively. Categorical variables were assessed by chi-square test or Fischer's exact test, and continuous variables by the Wilcoxon–Mann–Whitney or Kruskal–Wallis test. Risk factors for the different clinical manifestations of mucormycosis were identified using multivariate logistic regression.
Initial database searches identified 3619 articles of which 600 (851 individual patient cases) were included in the final analysis. Diabetes mellitus was the commonest underlying condition (340/851, 40%) and was an independent risk for rhino-orbital-cerebral mucormycosis (odds ratio (OR) 2.49; 95% CI 1.77–3.54; p < 0.001). Underlying haematological malignancy was associated with disseminated infection (OR 3.86; 95% CI 1.78–8.37; p 0.001), whereas previous solid organ transplantation was associated with pulmonary (OR 3.19; 95% CI 1.50–6.82; p 0.003), gastrointestinal (OR 4.47; 95% CI 1.69–11.80; p 0.003), or disseminated (OR 4.20; 95% CI 1.68–10.46; p 0.002) mucormycosis. Eight genera (24 species) of Mucorales organisms were identified in 447/851 (53%) cases, of which Rhizopus spp. (213/447, 48%) was the most common. Compared with other genera, Rhizopus spp. was predominantly observed in patients with rhino-orbital-cerebral mucormycosis (75/213, 35% versus 34/234, 15%; p < 0.001). Death was reported in 389/851 (46%) patients. Mortality associated with Cunninghamella infections was significantly higher than those caused by other Mucorales (23/30, 71% versus 185/417, 44%; p < 0.001). However, Cunninghamella spp. were isolated primarily in patients with pulmonary (17/30, 57%) or disseminated disease (10/30, 33%).
Findings from the current review have helped ascertain the association between various manifestations of mucormycosis, their respective predisposing factors and causative organisms.

Aspirin increases the risk of gastrointestinal bleeding.
Aim
To investigate the risk of lower gastrointestinal bleeding (LGIB) in aspirin users.
Methods
Low‐dose (75‐325 mg daily) ...aspirin users and controls matched by age, gender and enrollment time in a 1:5 ratio were selected from 1 million randomly sampled subjects in the National Health Insurance Research Database of Taiwan. Cox proportional hazard regression models were developed to evaluate the predictors of LGIB with adjustments for age, gender, comorbidities including coronary artery disease, ischaemic stroke, diabetes, hypertension, chronic kidney disease, liver cirrhosis, chronic obstructive pulmonary disease, dyslipidemia, uncomplicated peptic ulcer disease, history of peptic ulcer bleeding, and concomitant use of clopidogrel, ticlopidine, warfarin, nonsteroidal anti‐inflammatory drugs (NSAIDs), cyclooxygenase‐2 inhibitors, steroids, proton pump inhibitors (PPIs), histamine‐2 receptor antagonists (H2RAs), nitrates, alendronate, selective serotonin reuptake inhibitors (SSRIs) and calcium channel blockers.
Results
A total of 53 805 aspirin users and 269 025 controls were included. Aspirin group had a higher incidence of LGIB within 1 year than control group (0.20% vs 0.06%, P<.0001). Aspirin (hazard ratio HR: 2.75, 95% confidence interval CI: 2.06‐3.65), NSAIDs (HR: 8.61, 95% CI: 3.28‐22.58), steroids (HR: 10.50, 95% CI: 1.98‐55.57), SSRIs (HR: 11.71, 95% CI: 1.40‐97.94), PPIs (HR: 8.47, 95% CI: 2.26‐31.71), and H2RAs (HR: 10.83, 95% CI: 2.98‐39.33) were significantly associated with LGIB.
Conclusions
The risk of LGIB was higher in low‐dose aspirin users than in aspirin nonusers in this nationwide cohort. Low‐dose aspirin, NSAIDs, steroids, SSRIs, PPIs and H2RAs were independent risk factors for LGIB.

Since the first observation of odd and even high-order harmonics generated from ZnO crystals in 2011, the dependence of the harmonic yields on the orientation of the laser polarization with respect ...to the crystal axis has never been properly interpreted. This failure has been traced to the lack of a correct account of the phase of the transition dipole moment between the valence band and the conduction band. Using a simple one-dimensional two-band model, here we demonstrate that the observed odd harmonics is directly related to the orientation dependence of the magnitude of the transition dipole, while even harmonics is directly related to the phase of the transition dipole. Our result points out the essential role of the complex transition dipole moment in understanding harmonic generation from solids that has long been overlooked so far.

However, when the vector-mesons are described by momentum-dependent bound-state amplitudes, VMD fails for heavy vector-mesons: it cannot be used reliably to estimate either a photon-to-vector-meson transition strength or the momentum dependence of those integrands that would arise in calculations of the different reaction amplitudes. Consequently, for processes involving heavy mesons, the veracity of both cross-section estimates and conclusions based on the VMD assumption should be reviewed, e.g., those relating to hidden-charm pentaquark production and the origin of the proton mass.
The BCL-2 inhibitor venetoclax combined with hypomethylating agents or low-dose cytarabine represents an important new therapy for older or unfit patients with acute myeloid leukemia (AML). We ...analyzed 81 patients receiving these venetoclax-based combinations to identify molecular correlates of durable remission, response followed by relapse (adaptive resistance), or refractory disease (primary resistance). High response rates and durable remissions were typically associated with NPM1 or IDH2 mutations, with prolonged molecular remissions prevalent for NPM1 mutations. Primary and adaptive resistance to venetoclax-based combinations was most commonly characterized by acquisition or enrichment of clones activating signaling pathways such as FLT3 or RAS or biallelically perturbing TP53. Single-cell studies highlighted the polyclonal nature of intratumoral resistance mechanisms in some cases. Among cases that were primary refractory, we identified heterogeneous and sometimes divergent interval changes in leukemic clones within a single cycle of therapy, highlighting the dynamic and rapid occurrence of therapeutic selection in AML. In functional studies, FLT3 internal tandem duplication gain or TP53 loss conferred cross-resistance to both venetoclax and cytotoxic-based therapies. Collectively, we highlight molecular determinants of outcome with clinical relevance to patients with AML receiving venetoclax-based combination therapies.

We report the discovery and constrain the physical conditions of the interstellar medium of the highest-redshift millimeter-selected dusty star-forming galaxy to date, SPT-S J031132−5823.4 (hereafter ...SPT0311−58), at . SPT0311−58 was discovered via its 1.4 mm thermal dust continuum emission in the South Pole Telescope (SPT)-SZ survey. The spectroscopic redshift was determined through an Atacama Large Millimeter/submillimeter Array 3 mm frequency scan that detected CO(6-5), CO(7-6), and (2-1), and subsequently was confirmed by detections of CO(3-2) with the Australia Telescope Compact Array and with APEX. We constrain the properties of the ISM in SPT0311−58 with a radiative transfer analysis of the dust continuum photometry and the CO and line emission. This allows us to determine the gas content without ad hoc assumptions about gas mass scaling factors. SPT0311−58 is extremely massive, with an intrinsic gas mass of . Its large mass and intense star formation is very rare for a source well into the epoch of reionization.
We use machine learning to identify in color images of high-redshift galaxies an astrophysical phenomenon predicted by cosmological simulations. This phenomenon, called the blue nugget (BN) phase, is ...the compact star-forming phase in the central regions of many growing galaxies that follows an earlier phase of gas compaction and is followed by a central quenching phase. We train a convolutional neural network (CNN) with mock "observed" images of simulated galaxies at three phases of evolution- pre-BN, BN, and post-BN-and demonstrate that the CNN successfully retrieves the three phases in other simulated galaxies. We show that BNs are identified by the CNN within a time window of ∼0.15 Hubble times. When the trained CNN is applied to observed galaxies from the CANDELS survey at z = 1-3, it successfully identifies galaxies at the three phases. We find that the observed BNs are preferentially found in galaxies at a characteristic stellar mass range, 109.2-10.3 M at all redshifts. This is consistent with the characteristic galaxy mass for BNs as detected in the simulations and is meaningful because it is revealed in the observations when the direct information concerning the total galaxy luminosity has been eliminated from the training set. This technique can be applied to the classification of other astrophysical phenomena for improved comparison of theory and observations in the era of large imaging surveys and cosmological simulations.
