Résumé Les tumeurs histiocytaires de l’orbite sont principalement représentées par les granulomes éosinophiles à cellules de Langerhans, caractérisés à l’imagerie par une ostéolyse à l’emporte-pièce ...et une infiltration intra-orbitaire et dont le traitement repose sur l’exérèse chirurgicale. La maladie de Rosai-Dorfman, plus rare, peut entraîner une atteinte orbitaire uni- ou bilatérale sans atteinte osseuse associée. Les autres granulomatoses restent exceptionnelles, mais certaines comme la maladie d’Erdheim-Chester ont un pronostic plus réservé.

There have been few studies of neurosurgeons’ involvement in decision-making in regard to treatment limitation after severe traumatic brain injury.Aim of the studyThe first aim of the study was to assess French neurosurgeons’ adherence to the law on patients’ rights and end of life which governs such decision-making. The second aim was to assess the prognostic and decision-making criteria applied by neurosurgeons.MethodsA declarative practice and opinion survey, using a self-administered questionnaire emailed to all practising neurosurgeons members of the French Society of Neurosurgery, was conducted from April to June 2016.ResultsOf the 197 neurosurgeons contacted, 62 filled in the questionnaire. The most frequently used prognostic criteria were clinical parameters, intracranial pressure, cerebral perfusion pressure, and imaging, with significant interindividual variation in their use. The main decision-making criteria were foreseeable disability, expected future quality of life, and age.ConclusionsNeurosurgeons showed good compliance with legal requirements, except in the matter of calling for the opinion of an external consultant. Furthermore, this survey confirmed variability in the use of prognosis predictors, and the need for further clinical research so as to achieve more-standardized practices to minimise the subjectivity in decision-making.

En France, la loi définit et interdit «l'obstination déraisonnable » et encadre la décision qui en découle de limiter, voire d’arrêter les thérapeutiques. Elle accorde aussi au patient le droit de désigner une personne de confiance et de formuler des directives anticipées à cet égard. Peu d’études ont été réalisées sur les décisions de limitation thérapeutique chez les traumatisés crâniens sévères, et encore moins concernant le rôle et l’implication des neurochirurgiens.
Le premier objectif de cette étude était d’évaluer l’observance de la loi n° 2005-370 du 22 avril 2005 relative aux droits des malades et à la fin de vie, qui encadre ces décisions. Le second objectif était d’estimer la fréquence des critères pronostiques et décisionnels utilisés par les neurochirurgiens.
Une enquête de pratique par questionnaire envoyé par courriel aux neurochirurgiens membres de la Société française de neurochirurgie a été menée en France d’avril à juin 2016.
Sur 197 neurochirurgiens contactés, 62 ont complété le questionnaire. Les discussions de limitation thérapeutique respectaient le principe de collégialité requis par la loi et incluaient le neurochirurgien référent du patient dans 100 % des cas. La personne de confiance et/ou la famille étaient toujours consultées, mais leur avis était pris en compte de façon variable. Les directives anticipées étaient le plus souvent inexistantes (68 %) 56 % ; 80 % (27 %) ou inappropriées (27 %) 16 ; 38. Les paramètres d’évaluation pronostique les plus utilisés étaient les paramètres cliniques, la pression intra-crânienne, la pression de perfusion cérébrale et l’imagerie cérébrale, avec toutefois une grande variabilité interindividuelle dans leur utilisation. Les critères décisionnels principaux étaient la qualité de vie et le handicap prévisibles, puis l’âge.
On observe globalement une bonne observance de la loi par les neurochirurgiens, sauf concernant l’appel au consultant externe. Cette étude confirme l’hétérogénéité dans l’utilisation des critères d’évaluation pronostique et la nécessité de poursuivre la recherche clinique pour parvenir à des pratiques standardisées permettant de limiter au maximum la part de subjectivité dans le processus décisionnel.
To perform a descriptive analysis of intracranial hemorrhages of patients treated with an antivitamin K (fluindione, acenocoumarol or warfarin) or a direct oral anticoagulant (dabigatran, rivaroxaban ...or apixaban) at the Nancy Regional University Hospital.
To perform a descriptive analysis of intracranial hemorrhages of patients treated with an antivitamin K (fluindione, acenocoumarol or warfarin) or a direct oral anticoagulant (dabigatran, rivaroxaban ...or apixaban) at the Nancy Regional University Hospital.
The study period was from January 2011 to December 2013 and the computerized data (Programme de Médicalisation des Systèmes d’Information) of our hospital was accessed to identify the patients. Clinical data were obtained from the patients’ files. Regional healthcare system was queried for reimbursement data.
Among the 157 identified cases of intracranial hemorrhage, 153 were related to antivitamin K, primarily fluindione (n=127), and only 4 to a direct oral anticoagulant (3 dabigatran and 1 rivaroxaban). During the same period, regional data indicated that 65,345 patients had had at least one reimbursement of antivitamin K and 20,983 patients one reimbursement of an oral direct anticoagulant. In our series, the most frequent intracranial hemorrhages were subdural hematoma (chronic in 65 cases, acute in 50 cases) and intraparenchymal hemorrhage (20 cases). The global mortality rate was 20.2% but varied with the site of hemorrhage. In multivariate analysis, the two risk factors of fatal outcome were coma on admission (OR 6.2; 95%CI: 2.6–15.0) and a history of previous intracranial hemorrhage (OR 13,4; 95% CI: 1,6–114,9).
During the 2011–2013 period, antivitamin K, especially fluindione, was the most frequently involved anticoagulants in intracranial hemorrhages with hospitalization at our Regional University Hospital. Coma on admission and a history of previous intracranial hemorrhage were the two main risk factors for fatal outcome.
Réaliser une analyse descriptive des cas d’hémorragie intracrânienne avec hospitalisation au CHRU de Nancy concernant des patients traités par antivitamine K (fluindione, warfarine ou acénocoumarol) ou anticoagulant oraux directs (dabigatran, rivaroxaban ou apixaban).
Sur la période de janvier 2011 à décembre 2013, identification des patients par une requête dans le Programme de médicalisation des systèmes d’information et recueil clinique à partir des dossiers médicaux.
Sur 157 cas d’hémorragie intracrânienne identifiés, 153 étaient observés chez des patients traités par antivitamine K, majoritairement la fluindione (80,9 %) et seulement 4 par anticoagulant oral direct. Sur la même période, dans la région, 65 345 personnes avaient eu au moins un remboursement d’antivitamine K et 20 983 au moins un remboursement d’anticoagulant oral direct. Les hémorragies intracrâniennes les plus fréquentes étaient les hématomes sous-duraux (chroniques : 65 cas, aigus : 50 cas) et les hémorragies intraparenchymateuses (20 cas). Les deux facteurs significativement associés à une évolution fatale en analyse multivariée étaient la présence d’un coma à l’admission (OR 6,2 ; IC95 % 2,6–15,0) et un antécédent d’accident vasculaire cérébral hémorragique (OR 13,4 ; IC95 % 1,6–114,9).
Sur la période 2011–2013, la fluindione restait le principal anticoagulant oral impliqué dans les hémorragies intracrâniennes avec hospitalisation au CHRU de Nancy. La présence d’un coma à l’admission et un antécédent d’hémorragie intracrânienne étaient les deux facteurs significativement associés à une évolution fatale.
Abstract Spontaneous intracranial hypotension (SIH) is a well-known, but under- or misdiagnosed, condition caused by cerebro-spinal fluid (CSF) leak due to idiopathic dural breach. Blind lumbar ...epidural blood patch (EBP) is an effective treatment in most cases but occasionally even targeted EBP fails to lead to improvement. In these cases, the CSF leak is usually repaired surgically, especially for large dural breaches (>5mm), once the site has been identified by imaging techniques (MR-Myelography/CT-Myelography/Isotopic Transit). We describe a less invasive percutaneous technique consisting of direct puncture into the epidural space with a 25G needle to access the injection site under CT-scan control. We report two cases with good technical and clinical outcome after 1- and 8-years of follow-up (clinical evaluation and brain imaging control by CT-scan). The technique we describe here is of high interest in refractory-SIH or for “serious form” of the disease, before considering surgical repair. Further prospective studies are required to provide general guidelines in treatment options for such patients with SIH.
Résumé Introduction Les hémorragies intracrâniennes au cours d’un traitement anticoagulant sont des accidents rares mais associés à une mortalité élevée. L’âge, l’hypertension artérielle, l’intensité ...de l’anticoagulation favorisent leur survenue. Les facteurs pronostiques sont mal définis pour ce qui concerne les hématomes sous-duraux. Le but de notre travail était de déterminer les facteurs de mauvais pronostic des hémorragies intracrâniennes associées à un traitement par anticoagulants oraux. Méthodes Nous avons étudié rétrospectivement 186 patients admis dans notre institution entre 2000 et 2006 pour une hémorragie intracrânienne lors d’un traitement par antivitamine K. L’analyse statistique a été conduite sur l’échantillon global et sur le sous-groupe des hématomes sous duraux. Le critère de jugement principal était binaire et portait sur l’évolution (guérison ou décès) croisé avec différentes variables. Résultats L’âge moyen de la population était de 70,6 ans. Cent treize patients présentaient un hématome sous dural. Dans la population globale, l’analyse multivariée retrouvait comme facteur individuel de mauvais pronostic l’âge et le coma à l’admission. En revanche, les céphalées étaient un facteur de bon pronostic. Dans le sous-groupe des patients ayant présenté un hématome sous dural, les mêmes facteurs de mauvais pronostic étaient retrouvés. Le caractère chronique de l’hématome serait un facteur de bon pronostic comme les céphalées. Conclusion Certains des facteurs pronostiques identifiés ont déjà été mis en évidence pour les hémorragies intracrâniennes en dehors d’un traitement anticoagulant. D’autres comme les céphalées n’avaient jamais été décrits. Notre étude permet de guider la prise en charge thérapeutique de ces patients.
