To characterize the nature of CACNA1A mutations in episodic ataxia type 2 (EA2), to search for mutations in sporadic cases, and to delineate better the clinical spectrum.
EA2 is an autosomal dominant ...disorder characterized by recurrent acetazolamide-responsive attacks of cerebellar ataxia. The mutated gene, CACNA1A, located on chromosome 19, encodes the alpha1A subunit of a voltage-dependent calcium channel. So far, only three CACNA1A mutations have been identified-in two EA2 families and in one sporadic case. These three mutations disrupted the reading frame and led to truncated proteins. Interestingly, distinct types of CACNA1A mutations have been identified in familial hemiplegic migraine (missense mutations) and spinocerebellar ataxia type 6 (SCA-6) progressive cerebellar ataxia (expanded CAG repeats). However, except for SCA-6, these genotype-phenotype correlations relied on the analysis of very few families.
To characterize CACNA1A mutations, eight familial and seven sporadic EA2 patients were selected. All 47 exons of CACNA1A were screened by a combination of single-strand conformer polymorphism and sequencing analysis. In addition, the length of the CAG repeat has been determined in all patients.
Seven new mutations were detected in four multiple case families and three sporadic cases. Six of them lead most likely to truncated or aberrant proteins. CAG repeat sizes were in the normal range.
These data clearly establish the specificity of EA2 mutations compared with SCA-6 and familial hemiplegic migraine. Detailed clinical analysis of the mutation carriers showed the highly variable penetrance and expression of this disorder: Several of the carriers did not show any clinical symptom; others displayed atypical or permanent neurologic symptoms (such as recurrent, transient diplopia or severe, permanent, and isolated cerebellar ataxia).
Most patients with severe odontogenic infections are successfully treated with large spectrum probabilistic antibioc therapy, drainage of the collections and tooth treatment or extraction and are ...discharged home before antibiotic sensitivity results were available. The investigators hypothesized whether bacteriological sampling should be systematically performed in the management of patients with severe odontogenic infections. Methods: The investigators implemented a prospective observational study. The sample was composed of patients managed between January 2004 and December 2014 for severe odontogenic infection based on three criteria: hospital admission, intravenous antibiotic therapy, tooth extraction and collections drainage under general anesthesia. The predictor variable was the results of bacteriological sampling, culture and sensitivity. The outcome variable was antibiotic therapy adaptation according to antibiotic sensitivity results.
The sample was composed of 653 patients; 386 (59%) were male and 267 (41%) female, with a mean age of 37 years (range 18–88); 378 (58%) patients had been receiving oral antibiotics before admission to hospital, for a mean duration of 4.1 days (range 1 – 30). About 535 (81.9%) patients had swabs taken during surgery. Microorganisms were observed in 477 (89.1%) patients but in 377 (70.5%) they were polymorphic oropharyngeal flora. After culture, at least one antibiogram was obtained for 91 (17%) patients and the results led to antibiotic therapy being adapted in 23 (4.3%) patients.
The results suggest that bacteriological analysis had an impact on evolution in less than 5% of patients. Future studies will focus on the patients for whom the bacteriological analysis is essential.
Un auto-questionnaire réalisé chez 164 patients de la région Poitou-Charentes atteints de sclérose en plaques traitée depuis plus de trois mois par un immunomodulateur montre que la prévalence de ...l’auto-injection est élevée, représentant plus de 60 p. 100 des patients. Cette auto-injection est significativement plus importante dès lors que le patient bénéficie d’une éducation délivrée par une équipe spécialisée sur plusieurs séances avec un suivi téléphonique ultérieur ; et ce quel que soit le mode d’injection de l’immunomodulateur (sous-cutanée ou intramusculaire). Cet auto-questionnaire montre que l’élimination des déchets d’activité de soins à risque infectieux (aiguilles) reste un problème important, principalement chez les patients pratiquant l’auto-injection. Des efforts dans ce domaine doivent être réalisés par les pouvoirs publics.
One hundred and sixty four patients of the Poitou-Charentes area suffering from multiple sclerosis (MS) and treated with an immunomodulating agent for more than 3 months completed a self-administered questionnaire. More than 60p.cent of the patients performed self-injection. For both modes of injection studied (subcutaneous or intramuscular), self-injection was significantly more frequent among patients who were received training and followed via telephone assistance conducted by a nurse with specialized training in MS. Our study demonstrated that waste disposal (needles), especially among patients performing self-injections, remains an important problem. Efforts must be taken concerning this important healthcare issue.
The aim of this study was to evaluate the analgesic efficacy and the safety of the association, paracetamol, opium prepared and caffeine, in two different dosages as compared to the conventional ...analgesic tramadol hydrochloride, on acute postoperative dental pain.
We conducted a randomised, double-blind, multicentre, parallel-group clinical trial to test the efficacy and safety of single doses of two associations; paracetamol 500
mg, caffeine 50
mg, opium prepared 25, and paracetamol 500
mg, caffeine 50
mg, opium prepared 50
mg, as compared to tramadol hydrochloride 100
mg (called hereafter tramadol 100), and placebo, in the control of postoperative pain following the removal of 2 ipsilateral impacted third molars. The primary efficacy criterion was the sum of pain intensity differences as assessed every 30
minutes within 3
hours after the baseline assessment and administration of study treatment (SPID
0–3
h
).
Of the 232 randomised patients, 228 (98%) completed the study. Analysis of the primary efficacy criterion (SPID
0–3
h
) established: (i) the superiority of the 3 active study treatments
vs. placebo (

0.005); (ii) non-inferiority of paracetamol, caffeine, and opium 25
mg, and paracetamol, caffeine, and opium 50
mg
vs. tramadol.
Besides, both formulations of paracetamol, caffeine, and opium showed: (i) a faster onset of analgesic effect as compared to tramadol 100; (ii) a significantly stronger analgesic efficacy than tramadol 100, as measured 1
hour after the treatment intake; this superiority lasted all over the study duration for paracetamol, cafeine, and opium 50
mg but not for paracetamol, caffeine, and opium 25
mg. No unexpected safety concerns occurred, the two formulations of paracetamol, caffeine, and opium showed a good safety profile especially with paracetamol, caffeine, and opium 25
mg as compared to tramadol.
This study evidenced the non-inferiority of the paracetamol, caffeine, and opium 25
mg or 50
mg
vs. tramadol 100, and even though the strengths of the tested formulations were higher than that of the 2009, commercialised formulation of paracetamol, caffeine, and opium, efficacy was not offset by an alteration of the well recognised safety profile of the less strengthened formulation of the product being in use for decades.
L’objectif de cette étude était d’évaluer l’efficacité antalgique et la tolérance de deux nouvelles formulations, associant dans de nouveaux rapports de doses paracétamol, caféine et poudre d’opium dans le traitement de la douleur après avulsion dentaire.
Il s’agissait d’une étude multicentrique, randomisée, à quatre bras parallèles, menée en double aveugle
versus placebo dans le but de démontrer la non-infériorité de l’efficacité antalgique et la tolérance de deux formulations contenant 500
mg de paracétamol, 50
mg de caféine et 25 ou 50
mg de poudre d’opium en comparaison avec 100
mg de chlorhydrate de tramadol dans le traitement de la douleur après avulsion des deux troisièmes molaires incluses homolatérales en première séquence. Le critère principal d’efficacité antalgique a été la somme des différences d’intensité de la douleur (SDID

) entre le moment de la prise du traitement (T0) et celles notées chaque demi-heure, pendant 3
heures.
Au total 232 patients ont été randomisés dont 228 (98 %) ont terminé l’étude. Les trois traitements actifs ont été significativement plus efficaces que le placebo (

e heure après la prise du traitement pour paracétamol/caféine/25
mg poudre d’opium, a été maintenue pour paracétamol/caféine/50
mg poudre d’opium, sur toute la durée de l’étude. Le délai d’apparition de l’effet antalgique était plus court pour les deux associations (30
minutes) par rapport au groupe tramadol 100
mg (1
heure). Aucun évènement indésirable grave ou inattendu n’a été rapporté.
Cette étude a montré que deux formulations de paracétamol, caféine, et poudre d’opium avaient une efficacité antalgique au moins équivalente à celle de 100
mg de tramadol, un délai d’action plus rapide qu’avec tramadol et un meilleur profil de tolérance notamment avec paracétamol, caféine et 25
mg de poudre d’opium.
Le service d’Odontologie offre des soins à une population particulièrement précaire, vis-à-vis de laquelle il est recommandé de promouvoir les tests de dépistage des infections virales chroniques. ...Une première étude d’acceptation du test de dépistage ELISA chez des personnes consultant la permanence des soins et présentant une pathologie orale pouvant être évocatrice d’une infection VIH faisait état d’un taux d’acceptation de seulement 39 %. Qu’en est-il des TRODs ?
Étude prospective mono centrique observationnelle entre le 1er septembre 2016 et le 31 décembre 2016. Inclusion : personne majeure consultant en permanence de soins ayant un score EPICES>30 et/ou une/des lésion(s) orale(s) potentiellement révélatrice(s) du VIH. Non inclusion : personne séropositive connue, absence de consentement écrit. Recueil des variables sociodémographiques, cliniques et du TROD VIH par IDE formé. En cas de positivité : consultation en Maladies infectieuses pour confirmation et suivi ultérieur.
Parmi les 364 patients consécutifs vus en permanence des soins, 343 ont accepté le test EPICES (moyenne du score EPICES 35,66±21,97 ; 188 présentaient un test >30) et 22 patients présentaient des lésions évocatrices du VIH (gingivite ulcéronécrotique, herpes récidivants, aphtose récidivante, candidose). Un TROD VIH a été proposé à 163/210 patients (77 % des patients éligibles du fait de surcharge de travail et/ou manque de motivation), d’âge moyen 37,6±15,7 ans, dont 51 % d’hommes ; 103 l’ont accepté (acceptabilité de 62,5 %). Il n’y a pas de différence d’acception selon le sexe et le niveau de précarité, mais selon l’âge (la tranche 30–50 ans acceptant le moins avec 47,17 % ; p<0,01) et la présence de lésions évocatrices (meilleure acceptabilité avec 85,71 % ; p<0,001). Les causes de refus étaient : pas intéressé (20 %), test récent (20 %), proposition inadaptée refusée avec violence ou agressivité (18 %), situation matrimoniale stable (11 %), soins dentaires réalisés en urgence éprouvants (10 %), phobie des piqûres (7 %), autres (12 %). Un test s’est révélé positif, confirmé ensuite, chez un patient présentant une candidose et un herpès récidivant, ayant des CD4=20/mm3.
Le taux de précarité en permanence des soins du service d’Odontologie se confirme élevé. La proposition du test–non exhaustive ou proposée dans de bonnes conditions par manquement du personnel soignant – et l’acceptation de 62,5 % des patients–qui pourrait être améliorée car au moins 45 % des causes alléguées pour refuser le test ne l’étaient pas pour de bonnes raisons – renforcent la nécessité d’entretenir la motivation des soignants. La prévalence de l’infection VIH proche de 1 % montre tout l’intérêt du dépistage dans cette population, auquel en 2018, il faudra ajouter le TROD VHC.

In aggressive forms, tumor size and high recurrence risk need large ...surgical resections. In order to minimize surgical morbidity, especially in children, medical treatments acting on the tumor proliferation are currently being assessed: steroids (triamcinolone), anti-osteoclastic drugs (calcitonine, alendronate, denosumab), anti-angiogenic drugs (interferon α). However to date, there is no evidence for any superiority of medical over surgical treatment. Complete response is rarely obtained and additional surgery is often necessary to remove the tumor in case of tumor progression, to remove a remnant or to remodel bone. Moreover, these drugs have frequent local or systemic side effects such as osteonecrosis and growth deficiencies.
Le granulome central à cellule géante (GCCG) est une tumeur bénigne qui peut se présenter sous une forme non agressive ou sous une forme agressive. Dans la forme agressive, le risque élevé de ...récidive et la taille de la tumeur imposent des exérèses chirurgicales étendues. Pour limiter la morbidité liée à la chirurgie, notamment chez l’enfant, des molécules agissant directement sur le caractère prolifératif de la tumeur sont actuellement testées : anti-inflammatoires stéroïdiens (triamcinolone), anti-ostéoclastiques (calcitonine, alendronate, denosumab), anti-angiogéniques (interféron α). Les études publiées à ce jour ne permettent pas de démontrer la supériorité de ces traitements par rapport à la chirurgie. Leurs effets sur la régression de la tumeur sont souvent incomplets. En cas de réponse au traitement, une chirurgie complémentaire est souvent nécessaire pour réséquer le granulome en cas de poursuite évolutive ou pour cureter le reliquat tumoral et modeler l’os. Ces molécules s’accompagnent d’effets indésirables fréquents que ce soit localement (ostéonécrose) ou à distance (retentissement sur la croissance).
