Laryngocele is an air-filled dilatation of the laryngeal saccule that extends upward within the false vocal folds. Different etiologies lead to laryngocele congenital malformation, weakness of the ...laryngeal tissues and increased intralaryngeal pressure. Laryngocele may be a secondary iatrogenic complication following subtotal laryngectomy.
We report the case of a 61-year-old patient who presented an external laryngomucocele 8 years after a supracricoid partial laryngectomy with cricohyoidoepiglottopexy (SCPL-CHEP). We focus on the clinical aspects and therapeutic attitude, then discuss the physiopathological conditions that could generate this late complication.
Laryngocele after subtotal laryngectomy should be considered a late iatrogenic complication. Histological examination is necessary after surgical management of laryngocele, as the association with cancer is frequent.
Context and Aims: A recent upsurge of severe fungal forms of malignant otitis externa (MOE) has been recorded. Understanding the misleading clinical aspect of the disease allows earlier diagnosis, ...avoiding inadequate treatments that may complicate the adapted management. Our objective was to study the epidemiological and clinical aspects of the MOE in our department and to analyze some predictive factors of the fungal form of the disease. Materials and Methods: Medical data of 100 patients treated for MOE between 2004 and 2013 were retrospectively reviewed. Epidemiological findings were recorded. Clinical aspects and presentations were studied and noted. Analysis of some predictive factors of the fungal form of the disease was done. Results: Incidence of MOE was 10 new cases every year. The average age was 67.41 years, and the sex ratio was 0.92. All patients have diabetes. Otalgia was a constant symptom. Otorrhea was noted in 63% of cases. Hearing impairment and tinnitus were noted, respectively, in 23% and 18% of cases. Prior antibiotherapy was recorded in 33% of cases for a mean period of 21 days. Stenosis of the external auditory canal was observed in 92% of cases. Granulation tissue was present in 55% of patients. Twenty patients presented peripheral facial nerve palsy. Four others presented palsies of the VI, IX, and X nerves. Forty-one patients had mycological sampling. Twenty-six among them were diagnosed with fungal MOE. Identifying fungal agent took a mean delay of 40 days. Aspergillus and Candida were incriminated each one in half of the cases. Male gender and prior quinolone administration were predictive factors of fungal MOE with, respectively, P = 0.006 and P = 0.06. After multivariate analysis, only male gender was significantly correlated to fungal growth (P = 0.032). Conclusion: Management of fungal forms is challenging due to difficult and late diagnosis. An early mycological sampling is advisable with special attention to males and patients with prior quinolone administration.
Synovial cell sarcoma is a soft tissue tumor that occurs predominately in the lower limbs near the joints. Lesions of the head and neck are extremely rare. The larynx is the least frequent site with ...only about 20 cases reported in the literature. Treatment of these tumors is controversial and should follow the guidelines for other tumor sites. We report the case of a 37-year-old man with primary laryngeal synovial cell sarcoma, who was surgically managed by a narrow field laryngectomy.

First described in 1907, Gradenigo’s syndrome associates the following triad: acute otitis media as a causative infection of petrous apicitis, ipsilateral abducens nerve palsy and trigeminal ...neuralgia. This entity is becoming rare because of the wide use of antibiotics. The above definition can be extended as the fifth and sixth cranial nerves are intimately related near the apex of the petrous bone. Thus, any adjacent process may cause petrositis and palsies of these nerves.
Paranasal sinus adenocarcinomas are common. But primitive sphenoid bone localization is exceptional and manifest usually with a deep retro orbital headache.1
Gardenigo’s syndrome is a rare revelation mode of sphenoid tumors.
We report the case of a 55-years old female with a history of Budd Chiari syndrome, who presented with a typical Gradenigo’s Syndrome. The imaging showed an invasive process of sphenoid sinus. trans-sphenoidal biopsy concluded to adenocarcinoma. As the patient was inoperable, palliative radiotherapy was indicated.

The use of rapid prototyping tools such as MATLAB-Simulink and Xilinx System Generator becomes increasingly important because of time-to-market constraints. This paper presents a methodology for ...implementing real-time DSP applications on a reconfigurable logic platform using Xilinx System Generator (XSG) for Matlab. The methodology aims to improve the design verfication efficiency for such complex system. It presents architecture for Color Space Conversion (CSC) RGBTOYCbCr for video processing using Xilinx System Generator. The design was implemented targeting a Spartan3 device (3S200PQ208) then a Virtex II Pro (xc2vp7–6ff672). Obtained results are discussed and compared with an other architecture. The conversion method has been verified successfully with no visually perceptual errors in the transformed images.
Block-matching motion estimation plays an important role in video coding. Various simple and efficient block-matching algorithms (BMAs) are developed, like some popular methods, exhaustive search or ...full search (ES, FS respectively), three step search (TSS), new three step search (NTSS), diamond search (DS) etc. In this paper, we propose an improved for the fast motion adaptive rood pattern search (ARPS), which consists of two sequential search stages: Initial Search and Refined Local Search. The key idea of this method is above different motion characteristics of different sequence images, we adopt different Thresholds in the mean absolute difference (MAD) between the current and reference block. The proposed algorithm are compared which some BMAs by tested at several video sequences, the excellent performance is verified and it is suitable for videoconferencing sequences. The algorithm that are evaluated in this paper are implemented in video codec and provide competitive computational speedup with similar distortion performance as compared with the popular Diamond-based Search algorithm in the MPEG-4 Simple Profile.
