Prognostic factors for ambulatory oncology patients have been described, including Eastern Cooperative Oncology Group (ECOG), tumor stage and malnutrition. However, there is no firm evidence on which ...variables best predict mortality in hospitalized patients receiving active systemic treatment. Our main goal was to develop a predictive model for 90-day mortality upon admission.
Between 2020 and 2022, we prospectively collected data from three sites for cancer patients with hospitalizations. Those with metastatic disease receiving systemic therapy in the 6 months before unplanned admission were eligible to this study. The least absolute shrinkage and selection operator (LASSO) method was used to select the most relevant factors to predict 90-day mortality at admission. A multivariable logistic regression was fitted to create the PROgnostic Score for Hospitalized Cancer Patients (PROMISE) score. The score was developed in a single-center training cohort and externally validated.
Of 1,658 hospitalized patients, 1,009 met eligibility criteria. Baseline demographics, patient and disease characteristics were similar across cohorts. Lung cancer was the most common tumor type in both cohorts. Factors associated with higher 90-day mortality included worse ECOG, stable/progressive disease, low levels of albumin, increased absolute neutrophil count, and high lactate dehydrogenase. The c-index after bootstrap correction was 0.79 (95% CI, 0.75 – 0.82) and 0.74 (95% CI, 0.68 – 0.80) in the training and validation cohorts, respectively. A web tool (https://promise.vhio.net/) was developed to facilitate the clinical deployment of the model.
The PROMISE tool demonstrated high performance for identifying metastatic cancer patients who are alive 90 days after an unplanned hospitalization. This will facilitate healthcare providers with rational clinical decisions and care planning after discharge.

INTRODUÇÃO: O câncer de pulmão é a principal causa de morte relacionada a câncer no mundo. OBJETIVO: Determinar o tempo despendido com o diagnóstico, o estadiamento e o tratamento cirúrgico do câncer ...de pulmão em um hospital universitário. MÉTODO: Entre janeiro de 1990 e dezembro de 1998, 69 pacientes receberam diagnóstico de câncer de pulmão e foram tratados com cirurgia no Hospital das Clínicas de Porto Alegre. Os prontuários destes pacientes (53 homens e 16 mulheres) foram revisados, sendo coletados dados clínicos, histológicos, datas de consultas ambulatoriais, de internação hospitalar, de exames e de procedimentos, assim como o local de investigação. RESULTADOS: Quinze pacientes foram investigados em ambulatório (grupo I), 28 em ambulatório e durante internação (grupo II) e 26 durante internação (grupo III). A idade dos pacientes variou entre 43 e 79 anos (média de 61 ± 10 anos). Quarenta e três pacientes eram tabagistas atuais e 23 ex-tabagistas. O tempo médio do aparecimento dos sintomas até a primeira consulta foi de 110 dias e da primeira consulta até o diagnóstico foi de 33 dias. Vinte e cinco dias transcorreram entre o diagnóstico e a cirurgia. A média de tempo da primeira consulta até a cirurgia (tempo total) foi de 58 dias. A média do tempo total foi de 72 dias no grupo I, 72 dias no grupo II e 35 dias no grupo III (p < 0,01). Não houve diferença significativa no tempo total, considerando-se os diferentes estádios da doença (p = 0,16). CONCLUSÕES: Os resultados mostraram retardo na procura de atendimento médico e retardo adicional no tratamento do câncer de pulmão em pacientes cuja investigação foi ambulatorial.

Objective
To evaluate the factors associated with the need for insulin as a complementary treatment to metformin in pregnant women with gestational diabetes mellitus (GDM).
Methods
A ...case-control study was performed from April 2011 to February 2016 with pregnant women with GDM who needed complementary treatments besides diet and physical exercise. Those treated with metformin were compared with those who, in addition to metformin, also needed the combination with insulin. Maternal characteristics and glycemic control were evaluated. Multinomial logistic regression models were developed to evaluate the influence of different therapies on neonatal outcomes.
Results
A total of 475 pregnant women who needed pharmacological therapy were evaluated. Of these, 366 (77.05%) were submitted to single therapy with metformin, and 109 (22.94%) needed insulin as a complementary treatment. In the analysis of the odds ratio (OR), fasting glucose (FG) < 90 mg/dL reduced the odds of needing the combination (OR: 0.438 0.235–0.815;
p
= 0.009, as well as primiparity (OR: 0.280 0.111–0.704;
p
= 0.007. In obese pregnant women, an increased chance of needing the combination was observed (OR: 2,072 1,063–4,039;
p
= 0,032).
Conclusion
Obesity resulted in an increased chance of the mother needing insulin as a complementary treatment to metformin, while FG < 90 mg/dL and primiparity were protective factors.
Durante a gestação, a obesidade materna está associada ao desenvolvimento do Diabetes Mellitus Gestacional, doenças hipertensivas, intercorrências cardiovasculares, eventos tromboembólicos, ...cesarianas e complicações cirúrgicas durante o parto. Tendo em vista que a dieta materna, no período pré-natal, é de grande importância para o desenvolvimento fetal, pois determina o tipo de ácido graxo que se acumulará no tecido do feto, e que o transporte do ômega-3 se dá através da placenta, este trabalho tem como objetivo realizar uma revisão bibliográfica acerca dos efeitos do ômega-3 em gestantes obesas. Os resultados sobre os estudos analisados nos mostram que o ômega-3 é capaz de ajudar no controle do perfil lipídico materno e desenvolvimento fetal, conter reações inflamatórias, diminuir o risco de DMG e pré-eclâmpsia e possível redução da predisposição para doenças metabólicas na vida adulta. Portanto, o consumo de ômega-3, durante a gestação, é de grande importância, tanto para a mãe quanto para o concepto.
