Development of serologic tests to detect recent HIV-1 infection has generated worldwide interest in applying this approach to estimate incidence. We previously devised an IgG-capture BED-EIA (or ...BED-CEIA) that detects increasing levels of anti-HIV IgG following seroconversion to identify recent infection and to estimate incidence among persons infected with diverse HIV-1 subtypes worldwide. Injection drug users (IDUs; n = 1969) were screened in 1996 for participation in a prospective cohort study. Serum specimens from 594 IDUs were HIV-1 seropositive (30.2%) and were tested with the BED-CEIA. The proportion of recent infections and estimated incidence by different epidemiological risk factors were compared with incidence data measured from the prospective cohort. Of 594 HIV-1-seropositive specimens, 113 (19%) were identified as recent infections. Overall, the estimated annual incidence among persons screened was 17.3%/year (95% CI, 12.8-24.2%/year) compared with 9.0%/year (95% CI, 6.7-11.9%/year) measured from the prospective cohort during the same time period. Estimated incidence was higher among younger aged and unemployed IDUs as well as among those who injected more frequently, confirming previously reported risk factors from this prospective cohort. As persons screened from a cross-sectional sampling probably have higher risk for HIV than selected uninfected individuals who choose to participate and receive risk reduction counseling in a longitudinal cohort study, use of this or other serologic testing strategies to identify populations with high incidence (such as for HIV vaccine trials) may overestimate incidence measured from prospective cohorts.
The South Pole Telescope (SPT) is currently surveying 2500 deg2 of the southern sky to detect massive galaxy clusters out to the epoch of their formation using the Sunyaev-Zel'dovich (SZ) effect. ...This paper presents a catalog of the 26 most significant SZ cluster detections in the full survey region. The catalog includes 14 clusters which have been previously identified and 12 that are new discoveries. These clusters were identified in fields observed to two differing noise depths: 1500 deg2 at the final SPT survey depth of 18 Delta *mK arcmin at 150 GHz and 1000 deg2 at a depth of 54 Delta *mK arcmin. Clusters were selected on the basis of their SZ signal-to-noise ratio (S/N) in SPT maps, a quantity which has been demonstrated to correlate tightly with cluster mass. The S/N thresholds were chosen to achieve a comparable mass selection across survey fields of both depths. Cluster redshifts were obtained with optical and infrared imaging and spectroscopy from a variety of ground- and space-based facilities. The redshifts range from 0.098 <= z <= 1.132 with a median of z med = 0.40. The measured SZ S/N and redshifts lead to unbiased mass estimates ranging from 9.8 X 1014 M h --1 70 <= M 200( Delta *rmean) <= 3.1 X 1015 M h --1 70. Based on the SZ mass estimates, we find that none of the clusters are individually in significant tension with the Delta *LCDM cosmological model. We also test for evidence of non-Gaussianity based on the cluster sample and find the data show no preference for non-Gaussian perturbations.
Extended tailing of low bacterial concentrations following breakthrough at the Narrow Channel focus area was observed for 4 months. Bacterial attachment and detachment kinetics associated with ...breakthrough and extended tailing were determined by fitting a one‐dimensional transport model to the field breakthrough‐tailing data. Spatial variations in attachment rate coefficient (kƒ) were observed under forced gradient conditions (i.e., kƒ decreased as travel distance increased), possibly because of decreased bacterial adhesion with increased transport distance. When pore water velocity decreased by an order of magnitude at 9 days following injection, apparent bacterial attachment rate coefficients did not decrease with velocity as expected from filtration theory, but, instead, increased greatly for most of the wells. The coincidence of the increase in apparent attachment rate coefficient with the occurrence of protist blooms suggested that the loss of bacteria from the aqueous phase during the protist blooms was not governed by filtration but rather was governed by predation. Simulations were performed to examine the transport distances achieved with and without detachment, using attachment and detachment rate coefficients similar to those obtained in this field study. Simulations that included detachment showed that transport distances of bacteria may significantly increase because of detachment under the conditions examined.
Abstract Anterior cruciate ligament (ACL) injuries are common, and many of these patients go on to ACL reconstruction. At a later date, some may develop symptomatic osteoarthritis and require total ...knee arthroplasty (TKA). This raises the question: Does prior ACL reconstruction have a deleterious impact on the outcome of knee arthroplasty? Thirty-six cases of patients who underwent ACL reconstruction and then TKA at a later date were retrospectively reviewed. A cohort of patients without ACL injuries who underwent TKA for the diagnosis of primary osteoarthritis were selected to serve as controls. The results of this study demonstrate that previous ACL reconstruction does not have a negative impact on the outcome of future TKA with respect to range of motion, outcome scores, infection, or patella baja.
In order to diagnose colon cancer at an earlier, more localized stage, there is a need to develop diagnostic markers (genes) which can detect early patterns of gene expression in exfoliated ...colonocytes shed in the stool during routine screening for this disease. An RNA-based detection is more pertinent than either a DNA-based or a protein-based method as a screening procedure, but it has not been widely used as a cancer screen because of the difficulty of handling and stabilizing the RNA molecule. We describe a method that permits extraction of intact nondegraded total RNA from human colonocytes in stool and from normal and malignant colon tissues (which were employed for comparison with stool). Because it utilizes commercially available kits, this method is simpler than other published methods and does not require isolation of messenger (m)RNA, thereby reducing the chances of contaminating the preparations with degrading nucleases, and even a small amount of isolated total RNA can be adequately reverse transcribed, making high-quality copy (c) DNA. This is followed by PCR (either qualitative end point or semiquantitative real-time) using colon cancer-specific gene primers. By routinely and systematically being able to perform quantitative gene expression measurements on noninvasive samples, the goal of this pilot work is to lay the groundwork for conducting a large clinical study to identify groups of selected genes whose expression is consistently altered at an early stage in the neoplastic process. Such work will permit noninvasive monitoring of at-risk patients through the analysis of their stool samples. Correct diagnosis will allow for surgical and/or other interventions before the tumor is well established and, thus, should decrease mortality from this preventable disease.
Using data from the PdBI Arcsecond Whirlpool Survey (PAWS), we have generated the largest extragalactic giant molecular cloud (GMC) catalog to date, containing 1507 individual objects. GMCs in the ...inner M51 disk account for only 54% of the total super(12)CO(1-0) luminosity of the survey, but on average they exhibit physical properties similar to Galactic GMCs. We do not find a strong correlation between the GMC size and velocity dispersion, and a simple virial analysis suggests that ~30% of GMCs in M51 are unbound. We have analyzed the GMC properties within seven dynamically motivated galactic environments, finding that GMCs in the spiral arms and in the central region are brighter and have higher velocity dispersions than inter-arm clouds. Globally, the GMC mass distribution does not follow a simple power-law shape. Instead, we find that the shape of the mass distribution varies with galactic environment: the distribution is steeper in inter-arm region than in the spiral arms, and exhibits a sharp truncation at high masses for the nuclear bar region. We propose that the observed environmental variations in the GMC properties and mass distributions are a consequence of the combined action of large-scale dynamical processes and feedback from high-mass star formation. We describe some challenges of using existing GMC identification techniques for decomposing the super(12)CO(1-0) emission in molecule-rich environments, such as M51's inner disk.
We present measurements of the inclusive branching fractions for the decays D+-->Xe+ nu(e) and D0-->Xe+ nu(e), using 281 pb(-1) of data collected on the psi(3770) resonance with the CLEO-c detector. ...We find B(D0-->Xe+ nu(e)) = (6.46+/-0.17+/-0.13)% and B(D+-->Xe+ nu(e)) = (16.13+/-0.20+/-0.33)%. Using the known D meson lifetimes, we obtain the ratio Gamma(D+)sl/Gamma(D0)sl = 0.985+/-0.028+/-0.015, confirming isospin invariance at the level of 3%. The positron momentum spectra from D+ and D0 have consistent shapes.

Aim
To compare the frequency of H. suis, relative to H. pylori, in gastric biopsies of patients with idiopathic parkinsonism (IP) and controls from gastroenterology services.
Methods
DNA extracts, archived at a Helicobacter Reference Laboratory, from IP patient and gastroenterology service biopsies were examined anonymously for H. suis, using species‐specific RT‐PCR.
Results
Relative risk of having H. suis in 60 IP patients compared with 256 controls was 10 times greater than that of having H. pylori. In patients with IP and controls, respectively, frequencies of H. suis were 27 (exact binomial 95% C.I. 15, 38) and 2 (0, 3)%, and of H. pylori, 28 (17, 40) and 16 (12, 21)%. Excess of H. suis in IP held when only the antral or corporal biopsy was considered. Of 16 IP patients with H. suis, 11 were from 19 with proven H. pylori eradication, 3 from 17 pre‐H. pylori eradication, 2 from 24 H. pylori culture/PCR‐negative. Frequency was different between groups (P = 0.001), greatest where H. pylori had been eradicated. Even without known exposure to anti‐H. pylori therapy, H. suis was more frequent in IP patients (5/41) than in controls (1/155) (P = 0.002). Partial multilocus sequence typing confirmed that strains from IP patients (6) and control (1) differed from RT‐PCR standard strain.
Conclusions
Greater frequency of H. suis in idiopathic parkinsonism appears exaggerated following H. pylori eradication. Multilocus sequence testing comparison with porcine strains may clarify whether transmission is from pigs/porcine products or of human‐adapted, H. suis‐like, bacteria.
