Few data are available on long-term fatigue (LTF) and quality of life (QoL) among epithelial ovarian cancer survivors (EOCS). In this case–control study, we compared LTF, symptoms and several QoL ...domains in EOCS relapse-free ≥3years after first-line treatment and age-matched healthy women.
EOCS were recruited from 25 cooperative GINECO centers in France. Controls were randomly selected from the electoral rolls. All participants completed validated self-reported questionnaires: fatigue (FACIT-F), QoL (FACT-G/O), neurotoxicity (FACT-Ntx), anxiety/depression (HADS), sleep disturbance (ISI), and physical activity (IPAQ). Severe LTF (SLTF) was defined as a FACIT-F score <37/52. Univariate and multivariate logistic regressions were conducted to analyze SLTF and its influencing factors in EOCS.
A total of 318 EOCS and 318 controls were included. EOCS were 63-year-old on average, with FIGO stage I/II (50%), III/IV (48%); 99% had received platinum and taxane chemotherapy, with an average 6-year follow-up. There were no differences between the two groups in socio-demographic characteristics and global QoL. EOCS had poorer FACIT-F scores (40 versus 45, P<0.0001), lower functional well-being scores (18 versus 20, P=0.0002), poorer FACT-O scores (31 versus 34 P<0.0001), and poorer FACT-Ntx scores (35 versus 39, P<0.0001). They also reported more SLTF (26% versus 13%, P=0.0004), poorer sleep quality (63% versus 47%, P=0.0003), and more depression (22% versus 13%, P=0.01). Fewer than 20% of EOCS and controls exercised regularly. In multivariate analyses, EOCS with high levels of depression, neurotoxicity, and sleep disturbance had an increased risk of developing SLTF (P<0.01).
Compared with controls, EOCS presented similar QoL but persistent LTF, EOC-related symptoms, neurotoxicity, depression, and sleep disturbance. Depression, neuropathy, and sleep disturbance are the main conditions associated with severe LTF.

166 patients with relapse-free ≥3 years after the end of treatment attended a consultation with a gynecologist, including a questionnaire related to vasomotor symptoms (VMS) and sexuality, a clinical examination, a blood sample and an osteodensitometry. QoL, fatigue, insomnia and mood disorders were measured with validated questionnaires and correlated to VMS. VMS and QoL were assessed according to natural menopause (NM) or surgical menopause (SM).
Mean age at the survey was 62 21–83 years and stage III/IV (48%). Mean delay since the end of treatment was 6 years. Fifty-nine patients (36%) had SM. Half of patients reported VMS. Seventy-two percent of EOCS with SM had VMS compared to 41% with NM (P < .001). VMS were not associated with poor global QoL, fatigue, insomnia or mood disorders. Two-thirds of EOCS reported a decrease in libido. Patients with SM showed a greater decrease in libido than NM (P < .02). Fourteen percent of them had osteoporosis and 50% osteopenia. Among the 85 patients with VMS, 80 did not receive HRT after cancer treatment. At the time of the survey, only 7 (4%) patients were receiving hormone replacement therapy (HRT).
VMS and sexual disorders are frequently reported by EOCS, particularly among patients with SM. Most EOCS with menopausal symptoms could benefit from HRT to improve these symptoms.
•≥3 years after treatment, more than half of epithelial ovarian cancer survivors EOCS had vasomotor symptoms.•Two thirds of the EOCS reported a decrease in libido and, 1/3 sexually active EOCS complained of dyspareunia.•EOCS with surgical menopause reported more vasomotor symptoms and sexual disorders than those with natural menopause.•Very few patients received hormone replacement therapy after cancer treatment.
Le cancer de l’ovaire (CEO) est un cancer de mauvais pronostic, souvent diagnostiqué à un stade avancé. L’identification de marqueurs pronostiques et prédictifs précoces permettrait de personnaliser ...les traitements et la séquence thérapeutique. Le plasma contient une petite quantité d’ADN libre circulant (ADNlc). Une fraction de l’ADNlc peut être d’origine tumorale (ADN tumoral circulant–ADNtc) et identifiée par la détection de mutations génétiques spécifiques. La courte demi-vie de l’ADNtc en fait un marqueur dynamique de la charge tumorale. Peu d’études ont évalué l’intérêt de ce biomarqueur dans les CEO. L’objectif de cette étude était d’évaluer l’impact pronostique de la cinétique précoce de l’ADNtc en cours de chimiothérapie néoadjuvante (CNA) dans une cohorte de ces patientes atteintes d’un CEO avancé de l’étude CHIVA.
CHIVA est un essai de phase II randomisé, en double aveugle, contrôlé par placebo. Les patientes sont atteintes d’un CEO de stade FIGO IIIC-IV et ont été traitées par CNA combinée ou non au nintedanib±chirurgie d’intervalle. Un prélèvement sanguin a été réalisé au diagnostic et avant chaque cycle de CNA. L’ADNlc a été extrait du plasma au diagnostic (T0) et après un cycle de chimiothérapie (T1). La détection d’ADNtc a été réalisée par séquençage de nouvelle génération (NGS) à l’aide d’un panel validé couvrant plus de 500 mutations. Les données ont été analysées avec la méthode BPER (Base Position Error Rate) basée sur la quantification du nombre d’erreur retrouvé sur chacune des bases séquencées. Des statistiques descriptives ont été utilisées pour décrire les groupes. L’analyse multivariée a été réalisée à l’aide du modèle de régression logistique en incluant les variables connues et significatives lors de l’analyse univariée. La survie sans progression (SSP) et la survie globale (SG) ont été comparées entre les groupes en fonction de la cinétique de l’ADNtc. Une courbe de Kaplan Mayer a été utilisée pour exprimer graphiquement les différences de SSP et de SG, comparée par le test de Log-rank.
Parmi les 188 patientes incluses dans l’étude CHIVA, du plasma était disponible à T0 et T1 pour 159 patientes. L’ADNtc était détectable à T0 pour 100 patientes et n’était plus détectable à T1 pour 75 d’entre elles. L’ADNtc n’était pas détectable à T0 pour 59 patientes et restait indétectable à T1 pour 58 d’entre elles. Deux groupes ont été ainsi définis : ADNtc non détectable à T1 (n=133), ADNtc détectable à T1 (n=26). En analyse multivariée, la présence d’ADNtc à T1 était associée à un plus mauvais pronostic en SSP (HR 1,93; 95IC 1,21–3,07 ; p=0,006) et en SG (HR 1,97 ; 95IC 1,13–3,45 ; p=0,017). La SSP et la SG étaient diminuées en cas d’ADNtc détectable à T1 (p<0,0001 et p=0,00072 respectivement).
La décroissance précoce de l’ADNtc après un cycle de CNA est un facteur pronostique de la SSP et de la SG. Elle semble être un marqueur précoce de chimiosensibilité. D’autres études sont nécessaires pour préciser la place de l’ADNtc dans la prise en charges de ces patientes.
Therapeutic education in oncology is a recent phenomenon, and one which is likely to become more advertised and talked about in the months to come. We already understand the term well; however, all ...too often it is mixed in and combined with personal information. It was important for us to carefully go over the history of therapeutic education, the concept and its principles in relation to clearly identified chronic diseases, such as asthma, diabetes and renal failure. We also wanted to emphasise the different recommendations given by the French National Authority for Health (HAS) and the Hospital, Patient, Health and Territory Act promoting this concept in the in the field of oncology, and highlight that it is very beneficial and that it has already been introduced by oncology teams. All that is required is to structure and formalise the teaching and ongoing support of self-care or adjustment skills by the cancer patient, with the help of teams trained in ETP (therapeutic education). We have chosen to illustrate this concept through adjuvant breast cancer therapy (in the non-metastatic phase, because in this case the notion of chronic illness is obvious), as continuing with a physical activity or weight control become independent prognostic factors, which are just as important as the initial clinical or histological prognostic features.
Résumé: L’éducation thérapeutique en cancérologie est une attitude récente, elle deviendra vraisemblablement le pendant indispensable du dispositif d’annonce dans les mois à venir. Nous connaissons bien le terme mais bien souvent un amalgame est fait avec celui d’information personnalisée. Il nous a semblé important de reprendre de façon précise l’historique de l’éducation thérapeutique, son concept et ses principes à travers des maladies chroniques bien identifiées comme l’asthme, le diabète et l’insuffisance rénale. Nous avons également voulu insister sur les différentes recommandations que ce soit de la Haute Autorité de santé (HAS) ou de la loi Hôpital, patient, santé, territoire(HPST) pour promouvoir ce concept dans le domaine de la cancérologie, et souligner qu’il est très complémentaire de ce qui a déjà étémis en place par les équipes d’oncologie. Il nécessite seulement de structurer et de formaliser l’acquisition ou le maintien de compétences d’autosoins ou d’adaptation par le patient cancéreux, avec l’aide d’équipes formées à l’éducation thérapeutique du patient (ETP). Nous avons choisi d’illustrer ce concept à travers le cancer du sein en situation adjuvante (et non en phase métastatique car, dans ce cas, la notion de maladie chronique est évidente), car le maintien d’une activité physique ou le contrôle du poids deviennent des facteurs pronostiques indépendants, aussi importants que les caractéristiques histopronostiques ou cliniques initiales.
Inflammatory breast cancer (IBC) is a rare and aggressive disease requiring a multimodal treatment. We evaluated the benefit of adding docetaxel–5-fluorouracil (D-5FU) regimen after preoperative ...dose-intense (DI) epirubicin–cyclophosphamide (EC) and locoregional treatment in IBC patients.
PEGASE 07 was a national randomized phase III open-label study involving 14 hospitals in France. Women with nonmetastatic IBC were eligible and randomly assigned to receive either four cycles of DI EC (E 150 mg/m2 and C 4000 mg/m2 every 3 weeks with repeated hematopoietic stem cell support), then mastectomy with axillary lymph node dissection, and radiotherapy (arm A) or the same treatment followed by four cycles of D-5FU (D 85 mg/m2, day 1 and 5FU 750 mg/m2/day continuous infusion, days 1–5 every 3 weeks) administered postradiotherapy (arm B). Patients with hormone receptor-positive tumors received hormonal therapy. Disease-free survival (DFS) was the primary end point. Secondary end points included tolerance, pathological complete response (pCR) rate, and overall survival (OS).
Between January 2001 and May 2005, 174 patients were enrolled and treated (87 in each arm). Median follow-up was similar in both arms: 59.6 months 95% confidence interval (CI) 58.4–60.3 in arm A and 60.5 months (95% CI 58.3–61.4) in arm B. The estimated 5-year DFS rates were not different: 55% (95% CI 43.9–64.7) in arm A and 55.5% (95% CI 44.3–65.3) in arm B hazard ratio (HR) = 0.94 (0.61–1.48); P = 0.81. Identical results were observed for 5-year OS: 70.2% (95% CI 59.1–78.8) in arm A and 70% (95% CI 58.8–78.7) in arm B HR = 0.93 (0.55–1.60); P = 0.814. Following DI EC induction, in-breast and global (breast plus nodes) pCR were 28.9% and 20.1%, respectively. Estrogen receptor and pCR status were independently associated with survival.
The addition of D-5FU after preoperative DI EC and standard local therapy did not improve DFS in IBC.
