In a recent study, De Haart et al. (Arch Phys Med Rehabil 85:886-895, 2004) investigated the recovery of balance in stroke patients using traditional analyses of center-of-pressure (COP) trajectories ...to assess the effects of health status, rehabilitation, and task conditions like standing with eyes open or closed and standing while performing a cognitive dual task. To unravel the underlying control processes, we reanalyzed these data in terms of stochastic dynamics using more advanced analyses. Dimensionality, local stability, regularity, and scaling behavior of COP trajectories were determined and compared with shuffled and phase-randomized surrogate data. The presence of long-range correlations discarded the possibility that the COP trajectories were purely random. Compared to the healthy controls, the COP trajectories of the stroke patients were characterized by increased dimensionality and instability, but greater regularity in the frontal plane. These findings were taken to imply that the stroke patients actively (i.e., cognitively) coped with the stroke-induced impairment of posture, as reflected in the increased regularity and decreased local stability, by recruiting additional control processes (i.e., more degrees of freedom) and/or by tightening the present control structure while releasing non-essential degrees of freedom from postural control. In the course of rehabilitation, dimensionality stayed fairly constant, whereas local stability increased and regularity decreased. The progressively less regular COP trajectories were interpreted to indicate a reduction of cognitive involvement in postural control as recovery from stroke progressed. Consistent with this interpretation, the dual task condition resulted in less regular COP trajectories of greater dimensionality, reflecting a task-related decrease of active, cognitive contributions to postural control. In comparison with conventional posturography, our results show a clear surplus value of dynamical measures in studying postural control.
Meta-analyses have shown the positive effects of strategy instruction on student performance; however, little meta-analytical research has been conducted on its long-term effects. We examined the ...long-term effects of 48 metacognitive strategy instruction interventions on student academic performance. The results show a very small increase of the effect at long-term compared with the posttest effects. The instruction effect at posttest increased from Hedges' g = 0.50 to 0.63 at follow-up test. Moderator analyses showed that low SES students benefited the most at long-term. Furthermore, instructions including the cognitive strategy ‘rehearsal’ had lower long-term effects compared to interventions without this component. Other specific strategies (within categories metacognitive, cognitive, management, or motivational) did not moderate the overall positive long-term effect of metacognitive strategy instructions. Particular attributes of the intervention –subject domain, measurement instrument, duration, time between posttest and follow-up test, and cooperation – neither had an impact on the follow-up effect.

The aim of the present study was to answer the question whether adaptations to local perturbations are restricted to the perturbed limb or whether they induce a reorganization of all co-moving limbs. ...Specifically, we studied the adaptations in arm movements to mass perturbations in seven healthy adults during walking on a treadmill. Four different perturbation conditions were employed in random order (no perturbation, mass added to both wrists, to the right wrist, and to the right ankle). During each experimental condition ten different belt speeds (0.5, 1.0, 1.5, 2.0, 2.5, 3.0, 3.5, 4.0, 4.5, 5.0 km/h) were successively offered, while the arm movements and the electromyographic activity of the musculus deltoid posterior and anterior were measured. The results indicated that cadence was not affected by adding mass to the wrist or ankle. However, adding mass to a wrist not only resulted in an increase in muscle activity and a decrease of movement amplitude of the perturbed arm, but also in alterations in the non-perturbed arm. Notably, adding mass to one ankle induced adaptive changes in both arms, in that both muscle activity and arm movement increased. The present results indicate that during walking the loading of one of the limbs induces a general reorganization, involving all participating bodily segments, presumably to maintain balance while providing rhythm constancy.
Arctic ground squirrels (
Urocitellus parryii plesius
(Osgood, 1900); formerly
Spermophilus parryii plesius
Osgood, 1900) were studied in three distinct habitat types (boreal forest, low-elevation ...meadows, and alpine meadows) in the Kluane region of the southwest Yukon Territory, Canada, from 2008 to 2010 to determine if populations in these different habitats provide evidence for habitat-specific distribution and abundance. Abundance in the boreal forest has been shown to be synchronous with the cycle of snowshoe hares (
Lepus americanus
Erxleben, 1777) in the region owing to shared predators. We predicted that populations in the boreal forest would be low because of the current low phase in the cycle of snowshoe hares, and that in low-altitude meadows and alpine meadows, ground squirrels would be relatively abundant. Late-summer densities differed significantly between habitat types with 0.38 ± 0.13 squirrel/ha (mean ± 1 SE) in boreal-forest habitat, 1.25 ± 0.22 squirrel/ha in low-altitude-meadow habitat, and 5.7 ± 0.22 squirrels/ha in alpine-meadow habitat. In 2009, populations were extirpated from boreal-forest habitat, while densities in low-elevation meadows and alpine meadows were 1.6 ± 0.34 squirrel/ha and 6.1 ± 0.7 squirrels/ha, respectively. The current absence of squirrels from the boreal forest and the persistence of populations in low-elevation-meadow and alpine-meadow habitat suggest that source–sink dynamics may exist between boreal-forest and meadow habitat types.

In this meta-analysis the results of studies on learning strategy instruction focused on improving self-regulated learning were brought together to determine which specific strategies were the most effective in increasing academic performance. The meta-analysis included 58 studies in primary and secondary education on interventions aimed at improving cognitive, metacognitive, and management strategy skills, as well as motivational aspects and metacognitive knowledge. A total of 95 interventions and 180 effect sizes demonstrated substantial effects in the domains of writing (Hedges’ g=1.25), science (.73), mathematics (.66) and comprehensive reading (.36). These domains differed in terms of which strategies were the most effective in improving academic performance. However, metacognitive knowledge instruction appeared to be valuable in all of them. Furthermore, it was found that the effects were higher when self-developed tests were used than in the case of intervention-independent tests. Finally, no differential effects were observed for students with different ability levels. To conclude, the authors have listed some implications of their analysis for the educational practice and made some suggestions for further research.
Environmental variation in mountainous regions can impose major differences in demography and physiology on animal populations that occupy a large elevation range. This variation can be both ...predictable and unpredictable. In the south‐western Yukon, arctic ground squirrel (AGS) populations occur all the way from the forested valley bottoms to the alpine meadows and, in so doing, experience a wide range of predation risk, forage quality and exposure to weather variables. To assess how AGS integrate these factors and respond to them, we measured fecal glucocorticoid metabolites (FCM) as an index of the stress axis on four sites at different elevations. We first validated the enzyme immunoassay with a hormonal challenge protocol and a simulated predator stressor. AGS show a strong diurnal pattern in FCM levels, with peaks at mid‐day and a marked increase in response to adrenocorticotropic hormone and the simulated predator and a decline in response to dexamethasone. The lag time between the challenge and its reflection in the feces was 4–12 h. Using this method in our field studies, we found that FCM levels decreased as the active season progressed; however, specific patterns differed among sites. We hypothesized that the early season peak in FCM levels followed by the general decline was due to brief, intense early season breeding, followed by the necessity of AGS to increase mass in preparation for hibernation. Although we found no clear, single explanation for the different FCM patterns among sites, we hypothesized that differences in seasonal climate and adverse weather may be major factors affecting FCM levels. The environment was markedly different between years, with 2008 being colder and wetter than 2009 and this was associated with AGS in 2008 having much higher FCM levels in general than 2009. We found that population density and visibility may also contribute. In conclusion, AGS live in a mosaic of habitats and each population is faced with a variety of environmental stressors; how they cope and respond to these stressors may not depend on a single factor but the complete aggregate of these stressors.
To evaluate the contrasting dynamical and biomechanical interpretations of the 2:1 frequency coordination between arm and leg movements that occurs at low walking velocities and the 1:1 frequency ...coordination that occurs at higher walking velocities, the authors conducted an experiment in which they quantified the effect of walking velocity on the stability of the frequency and phase coordination between the individual limb movements. Spectral analyses revealed the presence of 2:1 frequency coordination as a consistent feature of the data in only 3 out of 8 participants at walking velocities ranging from 1.0 to 2.0 km/h, in spite of the fact that the eigenfrequencies of the arms were rather similar across participants. The degree of interlimb coupling, as indexed by weighted coherence and variability of relative phase, was lower for the arm movements and for ipsilateral and diagonal combinations of arm and leg movements than for the leg movements. Furthermore, the coupling between all pairs of limb movements was found to increase with walking velocity, whereas no clear signs were observed that the switches from 2:1 to 1:1 frequency coordination and vice versa were preceded by loss of stability. Therefore, neither a purely biomechanical nor a purely dynamical model is optimally suited to explain these results. Instead, an integrative model involving elements of both approaches seems to be required.
Up to 7% of term and late-preterm neonates in high-income countries receive antibiotics during the first 3 days of life because of suspected early-onset sepsis. The prevalence of culture-proven ...early-onset sepsis is 0·1% or less in high-income countries, suggesting substantial overtreatment. We assess whether procalcitonin-guided decision making for suspected early-onset sepsis can safely reduce the duration of antibiotic treatment.
We did this randomised controlled intervention trial in Dutch (n=11), Swiss (n=4), Canadian (n=2), and Czech (n=1) hospitals. Neonates of gestational age 34 weeks or older, with suspected early-onset sepsis requiring antibiotic treatment were stratified into four risk categories by their treating physicians and randomly assigned 1:1 using a computer-generated list stratified per centre to procalcitonin-guided decision making or standard care-based antibiotic treatment. Neonates who underwent surgery within the first week of life or had major congenital malformations that would have required hospital admission were excluded. Only principal investigators were masked for group assignment. Co-primary outcomes were non-inferiority for re-infection or death in the first month of life (margin 2·0%) and superiority for duration of antibiotic therapy. Intention-to-treat and per-protocol analyses were done. This trial was registered with ClinicalTrials.gov, number NCT00854932.
Between May 21, 2009, and Feb 14, 2015, we screened 2440 neonates with suspected early-onset sepsis. 622 infants were excluded due to lack of parental consent, 93 were ineligible for reasons unknown (68), congenital malformation (22), or surgery in the first week of life (3). 14 neonates were excluded as 100% data monitoring or retrieval was not feasible, and one neonate was excluded because their procalcitonin measurements could not be taken. 1710 neonates were enrolled and randomly assigned to either procalcitonin-guided therapy (n=866) or standard therapy (n=844). 1408 neonates underwent per-protocol analysis (745 in the procalcitonin group and 663 standard group). For the procalcitonin group, the duration of antibiotic therapy was reduced (intention to treat: 55·1 vs 65·0 h, p<0·0001; per protocol: 51·8 vs 64·0 h; p<0·0001). No sepsis-related deaths occurred, and 9 (<1%) of 1710 neonates had possible re-infection. The risk difference for non-inferiority was 0·1% (95% CI −4·6 to 4·8) in the intention-to-treat analysis (5 0·6% of 866 neonates in the procalcitonin group vs 4 0·5% of 844 neonates in the standard group) and 0·1% (−5·2 to 5·3) in the per-protocol analysis (5 0·7% of 745 neonates in the procalcitonin group vs 4 0·6% of 663 neonates in the standard group).
Procalcitonin-guided decision making was superior to standard care in reducing antibiotic therapy in neonates with suspected early-onset sepsis. Non-inferiority for re-infection or death could not be shown due to the low occurrence of re-infections and absence of study-related death.

Atopic dermatitis is often complicated by allergic contact dermatitis, although patch testing may reveal positive reactions of uncertain relevance. We report a case of a 35‐year‐old woman with ...recalcitrant atopic dermatitis, with a positive patch‐test reaction to Compositae mix (CM). Initially, sensitization appeared to be of past relevance only, due to use of calendula. However, it turned out that she followed a self‐devised diet consisting largely of food products of the Compositae family. On excluding these food products her skin condition improved quickly. This case report underscores the difficulty in determining the relevance of positive patch tests, and shows that thorough analysis of positive patch tests, by both patient and physician, may reveal unexpected or less common sources of contact allergens.
The authors investigated the effects of velocity (increasing from 0.5 to 5.0 km/hr in steps of 0.5 km/hr) and limb loading on the coordination between arm and leg movements during treadmill walking ...in 7 participants. Both the consistency of the individual limb movements and the stability of their coordination increased with increasing velocity; the frequency coordination between arm and leg movements was 2:1 at the lower velocities and 1:1 at the higher velocities. The mass manipulation affected the individual limb movements but not their coordination, indicating that a stable walking pattern was preserved. The results differed qualitatively from those obtained in studies on bimanual interlimb coordination, implying that the dynamical principles identified therein are not readily applicable to locomotion.
