We performed coronary artery bypass grafting on a 58-year-old man who only 9 months earlier had undergone right pneumonectomy for bronchial carcinoma. Although his preoperative pulmonary function had ...been poor, coronary artery bypass surgery was successful, and the patient was discharged on the 9th postoperative day Two years after surgery, he remained in New York Heart Association functional class I. We attribute this success to special management before, during, and after the operation. On the 32nd postoperative month, this patient died of multiple tumor metastases.
We report the evidence of a new \(\beta\) Cephei type variable located in the Southern open cluster \astrobj{NGC 6200}: \astrobj{ALS 3728}. Spectroscopic and photometric observations of NGC\,6200 ...stars were carried out at the South African Astronomical Observatory (SAAO) in 2011. It was found that \astrobj{ALS 3728} has a frequency of about 4.95 d\(^{-1}\) with highest amplitude. The star also shows a remarkable stillstand phase just before its light maxima which is a typical characteristic occurring among these type of stars. Furthermore, a mode identification is applied for the dominant frequency that is calculated from the Fourier analysis.
We present the results of a new investigation aimed to identify the
pulsational characteristics of the Southern $\beta$ Cephei star ALS 3721.
Spectroscopic and multicolour photometric data were ...acquired at the South
African Astronomical Observatory (SAAO), South Africa in 2011. Frequency
analysis showed that the oscillations of ALS 3721 could be attributed to the
two main frequencies with higher significance. Stellar parameters and projected
rotational velocity obtained by the spectra were also used to perform
photometric mode identification. In order to determine spherical harmonic
degrees ($l$), a principal method was followed by comparing the observed light
amplitude ratios in different passbands with those computed from non--adiabatic
pulsation models. In general case, therefore, the spherical harmonic degrees
corresponding of the frequencies were found in the expected $\beta$ Cephei
range.
ZusammenfassungWir beschreiben erstmals die Koinzidenz von Marfan-Syndrom und Ventrikelseptumaneurysma. Neben einer grenzwertig dilatierten Aorta waren bei der 35-jährigen Patientin keine weiteren ...kardiovaskulären Manifestationen bekannt. Erst eine transoesophageale Untersuchung erbrachte den Befund des großen Ventrikelseptumaneurysmas. Um eine Ruptur und das akute Auftreten eines großen Links-Rechts-Shunts auf Ventrikelebene sowie die Zunahme der Irritation der Trikuspidalklappe zu vermeiden, erfolgte die elektive Operation.Bei Patienten mit Marfan-Syndrom sollte die kardiologische Diagnostik auch die Suche nach intrakardialen Defekten einschließen.
This is the first description of a patient with Marfan syndrome and an aneurysm of the ventricular septum. Apart from a borderline dilatation of the ascending aorta, there were no cardiovascular ...manifestations of Marfan syndrome. A transesophageal echocardiographic examination showed a large aneurysm of the ventricular septum. To prevent the imminent rupture and the acute occurrence of a significant left-to-right-shunt on the ventricular level, as well as increasing irritation of tricuspid valve, an elective operation was performed. Besides the routine cardiological diagnostics, the search for intracardial defects is also necessary in patients with Marfan syndrome.PUBLICATION ABSTRACT
Abstract Background Graft coronary artery disease, a serious problem after orthotopic heart transplantation (OHT), has multifactorial etiologies with dyslipidemia as one of the major risk factors. In ...this study we examined lipid profiles and drug therapy of our patients before and after OHT. Methods Thirteen patients who underwent OHT at our center were enrolled in the study. We noted the patients’ clinical and demographic data and current medications as well as pre- and postoperative lipid values. Results The mean age of the study group was 32.0 ± 13.2 years with three women. Compared to the preoperative values, significant increases were detected in the mean levels of low-density lipoprotein (LDL) (81.3 ± 29.1 vs 103.5 ± 22.2 mg/dL; P = .03) and total cholesterol (142.0 ± 58.5 vs 184.0 ± 37.8 mg/dL; P = .02), while triglyceride (113.5 ± 67.3 vs 137.0 ± 69.9 mg/dL; P = .1) and high-density lipoprotein (42.7 ± 10.2 vs 48.7 ± 14.4 mg/dL; P = .2) levels did not change significantly at 2 to 3 months postoperatively. On follow-up eight patients were prescribed a statin (atorvastatin in all), one of whom was on ezetimibe in addition to statin and one, fenofibrate. The patients tolerated lipid-lowering agents well; no significant side effect was noted. Conclusion These findings demonstrated increased lipid values, mainly in total cholesterol and LDL levels, after OHT. Regarding the importance of dyslipidemia as a major atherosclerotic risk factor, we believe that statins in the absence of a contraindication should be part of the treatment protocol in patients with a transplanted heart.
Aims Beta-blockers (BBs) have been shown to improve survival and reduce the risk of re-infarction in patients following myocardial infarction. There are conflicting data about the effects of BB ...therapy on cardiac biomarkers after percutaneous coronary interventions (PCIs). The aim of the study was to investigate the effects of BB use on cardiac troponin-I (cTnI) levels in patients who had undergone elective PCI. Methods and results In this prospective study, 287 patients with coronary artery disease were included. Patients were randomized either to BB or control groups prior to the intervention. Blood samples for cTnI were obtained before and at 6, 24, and 36 h after the procedure. Of the 287 patients included, 143 received metoprolol succinate 100 mg/day, and 144 received no BB and served as the control group. Baseline clinical characteristics of both groups, except for history of coronary artery bypass graft surgery, were similar. We observed no significant difference in the elevation of cTnI levels between the two groups after PCI (BB group, 17 patients, 11.9%; control group, 10 patients, 6.9%; P=0.2). Conclusion Metoprolol succinate therapy seems to have no cardioprotective effect in limiting troponin-I rise after PCI.
