An approach to carboplatin dosing in children with bilateral nephrectomy using a renal function-based dosing formula with a glomerular filtration rate of zero was investigated in the current study. ...Carboplatin exposure was determined in a total of nine courses of chemotherapy in four patients with Wilms' tumour. Carboplatin exposures following initial dosing were less than 50% of the defined target area under the plasma concentration-time curve (AUC) in all four patients studied, with actual AUC values of between 31% and 45% of the target exposures. The use of real-time pharmacokinetic monitoring to guide dosing within a course of carboplatin treatment resulted in exposures within 15% of the target AUC in all patients. Using this information to guide dosing on additional courses of treatment in the same patient resulted in consistent exposures without the need for further monitoring or dose adjustment. These results indicate that real-time pharmacokinetic monitoring of carboplatin treatment plays a key role in ensuring that an appropriate exposure to carboplatin is achieved in children with bilateral nephrectomy. Carboplatin dosing based on patient body weight, or use of a fixed dose of carboplatin, would both be predicted to result in individual patients receiving unsatisfactory drug exposures. Further studies are warranted to further elucidate the relationship between non-renal clearance of carboplatin and patient body weight in this and other patient subpopulations where there remains concern about the optimal way to use this anticancer drug.

This paper describes the Polarization Spectroscopic Telescope Array (PolSTAR), a mission proposed to NASA's 2014 Small Explorer (SMEX) announcement of opportunity. PolSTAR measures the linear ...polarization of 3-50 keV (requirement; goal: 2.5-70 keV) X-rays probing the behavior of matter, radiation and the very fabric of spacetime under the extreme conditions close to the event horizons of black holes, as well as in and around magnetars and neutron stars. The PolSTAR design is based on the technology developed for the Nuclear Spectroscopic Telescope Array (NuSTAR) mission launched in June 2012. In particular, it uses the same X-ray optics, extendable telescope boom, optical bench, and CdZnTe detectors as NuSTAR. The mission has the sensitivity to measure ~1% linear polarization fractions for X-ray sources with fluxes down to ~5 mCrab. This paper describes the PolSTAR design as well as the science drivers and the potential science return.
An agricultural production function is estimated using a Chinese county-level dataset along with associated geographic information. County-specific output-input elasticities are computed using the ...geographically weighted regression (GWR) and mapped with the geographical information system (GIS). A comparison of the ordinary least squares and GWR estimates confirms that allowing spatial variation in the parameters improves model fit of the agricultural production function, and provides valuable insights into the relative importance of inputs in different regions. Moran's indices reveal the spatial dependence of output per unit of land, and four inputs across regions. Mappings of GWR estimates help to detect a few clusters of high output-input elasticities: for labour in the Northeast, North, Southwest and Northwest China, for land in the Central and Southwest, for mechanical power in the North, Northwest and coastal area of the East and South and for fertiliser in the East. The county-specific scale elasticities suggest constant return to scale is likely to hold in the Central, East, Southwest and South. Copyright (c) 2007 the author(s).
