Cardiovascular disease (CVD) is the primary cause of death and disability worldwide. Although age-standardized CVD mortality rates decreased globally by 14.5% between 2006 and 2016, the burden of CVD ...remains disproportionately higher in low- and middle-income countries compared to high-income countries. Even though proven, effective approaches based on multiple-drug intake aimed at the prevention and treatment of CVD are currently available, poor adherence, early discontinuation of treatment, and suboptimal daily execution of the prescribed therapeutic regimes give rise to shortfalls in drug exposure, leading to high variability in the responses to the prescribed medications. Wald and Law, in their landmark paper published in BMJ 2003, hypothesized that the use of a fixed-dose combination of statins, β-blockers, angiotensin receptor blockers, angiotensin-converting enzyme inhibitors, and aspirin (classic Polypill composition) may increase adherence and decrease CVD by up to 80% when prescribed as primary prevention or in substitution of traditional protocols. Since then, many clinical trials have tested this hypothesis, with comparable results. This review aims to describe the available clinical trials performed to assess the impact of fixed-dose combinations on adherence, cost-effectiveness, and the risk factors critical to the onset of CVD.

This paper presents a non-invasive fully automatic procedure to obtain highly accurate fish length estimation in adult Bluefin Tuna, based on a stereoscopic vision system and a deformable model of the fish ventral silhouette. The present work takes a geometric tuna model, which was previously developed by the same authors to discriminate fish in 2D images, and proposes new models to enhance the capabilities of the automatic procedure, from fish discrimination to accurate 3D length estimation. Fish length information is an important indicator of the health of wild fish stocks and for predicting biomass using length-weight relations. The proposal pays special attention to parts of the fish silhouette that have special relevance for accurate length estimation. The models have been designed to best fit the rear part of the fish, in particular the caudal peduncle, and a width parameter has been added to better fit the silhouette. Moreover, algorithms have been developed to extract snout tip and caudal peduncle features, allowing better initialization of model parameters. Snout Fork Length (SFL) measurements using the different models are extracted from images recorded with a stereoscopic vision system in a sea cage containing 312 adult Atlantic Bluefin Tuna. The automatic measurements are compared with two ground truths: one configured with semiautomatic measurements of favourable selected samples and one with real SFL measurements of the tuna stock collected at harvesting. Comparison with the semiautomatic measurements demonstrates that the combination of improved geometric models and feature extraction algorithms delivers good results in terms of fish length estimation error (up to 90% of the samples bounded in a 3% error margin) and number of automatic measurements (up to 950 samples out of 1000). When compared with real SFL measurements of the tuna stock, the system provides a high number of automatic detections (up to 6706 in a video of 135 min duration, i.e., 50 automatic measurements per minute of recording) and highly accurate length measurements, obtaining no statistically significant difference between automatic and real SFL frequency distributions. This procedure could be extended to other species to assess the size distribution of stocks, as discussed in the paper.
Dermatophyte infections are the most common fungal infections in humans; among them, tinea capitis (TC) – the most contagious fungal infection – is caused by anthropophilic, zoophilic and geophilic ...dermatophytes. The purpose of this systematic review was to determine the different aetiological variants involved in TC and the overall epidemiology of the causes of this infection in the last two decades. We searched the MEDLINE (PubMed) and Embase databases for articles published from July 2000 to August 2019 using the following search terms: ‘Tinea capitis’, ‘Africa’, ‘America’, ‘Asia’, ‘Europe’, ‘Oceania’, and the names of the countries on each continent. The flow of information through the different phases in this systematic review was depicted using a PRISMA flow diagram, which mapped the number of records identified, included and excluded, and the reasons for exclusion. Our findings indicate that the frequency of different aetiologic agents of TC in the reported studies varied globally, from 0.4–87.7% in Africa, 0.2–74.0% in North America, 0.0–91.2% in Eastern Asia, 0.0–69.0% in Eastern Europe and 2.9–86.4% in Oceania. Microsporum canis is the most frequent reported zoophilic agent worldwide, while Trichophyton violaceum and Trichophyton tonsurans are the predominant anthropophilic agents. Over time, the frequency of these latter fungal infections has increased globally, and these fungi have become the major species globally. Anthropophilic transmission – the most prevalent type of transmission – could be explained by two factors: (i) the socioeconomic status of affected countries and population groups with associated risk factors and (ii) movement of populations importing new causes of infection to areas where they had not been encountered previously. We observed that intercontinental migration and travel; globalization; environmental, climatic and ecological changes; and accelerated evolution of health technologies may influence the observed epidemiological changes and, consequently, contributed to the variations in the global status of TC.
In this study, we evaluated the effect of low and high molecular weight polycyclic aromatic hydrocarbons (PAHs), i.e., Phenanthrene, Pyrene and Benzoapyrene, on the radial growth and morphology of ...the PAH-degrading fungal strains Aspergillus nomius H7 and Trichoderma asperellum H15. The presence of PAHs in solid medium produced significant detrimental effects on the radial growth of A. nomius H7 at 4,000 and 6,000 mg L(-1) and changes in mycelium pigmentation, abundance and sporulation ability at 1,000-6,000 mg L(-1). In contrast, the radial growth of T. asperellum H15 was not affected at any of the doses tested, although sporulation was observed only up to 4,000 mg L(-1) and as with the H7 strain, some visible changes in sporulation patterns and mycelium pigmentation were observed. Our results suggest that fungal strains exposed to high doses of PAHs significantly vary in their growth rates and sporulation characteristics in response to the physiological and defense mechanisms that affect both pigment production and conidiation processes. This finding is relevant for obtaining a better understanding of fungal adaptation in PAH-polluted environments and for developing and implementing adequate strategies for the remediation of contaminated soils.
Tuberculosis (TB) is a chronic infectious disease in which prolonged, non-resolutive inflammation of the lung may lead to metabolic and neuroendocrine dysfunction. Previous studies have reported that ...individuals coursing pulmonary TB experience cognitive or behavioural changes; however, the pathogenic substrate of such manifestations have remained unknown. Here, using a mouse model of progressive pulmonary TB, we report that, even in the absence of brain infection, TB is associated with marked increased synthesis of both inflammatory and anti-inflammatory cytokines in discrete brain areas such as the hypothalamus, the hippocampal formation and cerebellum accompanied by substantial changes in the synthesis of neurotransmitters. Moreover, histopathological findings of neurodegeneration and neuronal death were found as infection progressed with activation of p38, JNK and reduction in the BDNF levels. Finally, we perform behavioural analysis in infected mice throughout the infection, and our data show that the cytokine and neurochemical changes were associated with a marked onset of cognitive impairment as well as depressive- and anxiety-like behaviour. Altogether, our results suggest that besides pulmonary damage, TB is accompanied by an extensive neuroinflammatory and neurodegenerative state which explains some of the behavioural abnormalities found in TB patients.
A degrading microbial consortium highly tolerant to three-, four- and five-ring polycyclic aromatic hydrocarbons (PAHs) was selected from 50 fungal and bacterial isolates obtained from crude ...oil-contaminated soils. Morphological and molecular studies indicated that isolated fungi belonged to genera Aspergillus, Penicillium, Fusarium, Trichoderma, Scedosporium, and Acremonium and bacteria to Pseudomonas, Klebsiella, Bacillus, Enterobacter, Streptomyces, Stenotrophomonas, Kocuria, and Delftia genera. Individual fungal and bacterial isolates were evaluated for their potential to tolerate high concentrations of different molecular weight PAHs, as phenantrene (Phe), pyrene (Pyr), and benzoapyrene (BaP) by surface plate assays, showing significant differences in extension rates for fungi and inhibition ratios for bacteria when both were exposed to 0–6,000 mg of PAHs per liter. Trichoderma asperellum H15, Aspergillus nomius H7, Aspergillus flavus H6, Pseudomonas aeruginosa B7, Klebsiella sp. B10, and Stenotrophomonas maltophilia B14 grew using PAHs as sole carbon source and presented a remarkably high tolerance to PAHs, up to 6,000 mg l⁻¹. The consortium composed of 12 fungal and bacterial PAH-tolerant isolates for the bioremediation of a PAH-contaminated soiled to a removal of 87.76 % Phe, 48.18 % Pyr, and 56.55 % BaP after 14 days. The degrading microbial consortium presented high potential for bioremediation and may be useful for the treatment of sites polluted with PAHs due to their elevated tolerance to high molecular weight (HMW) PAHs and their capacity to utilize them as energy source. This is the first study which evaluated the microbial tolerance to extreme concentrations of PAHs, resulting in a degrading consortium and highly tolerant consortium compared with those reported in other studies, where the concentrations tested are low.
The aims of our work were to determine normal aging rates for structural and functional arterial parameters in healthy children, adolescents, and young adults and to identify gender-related ...differences in these aging rates. Methods. 161 subjects (mean: 15 years (range: 4–28 years), 69 females) were studied. Subjects included had no congenital or chronic diseases, nor had they been previously exposed to traditional cardiovascular risk factors. Arterial parameters assessed were (1) central blood pressure (BP) and aortic pulse wave analysis, (2) arterial local (pressure-strain elastic modulus) and regional (pulse wave velocity, PWV) stiffness, and (3) arterial diameters and carotid intima-media thickness (CIMT). Simple linear regression models (age as the independent variable) were obtained for all the parameters and the resulting rates of change were compared between genders. Results. No gender-related differences were found in mean values of arterial structural and functional parameters in prepubertal ages (4–8 years), but they started to appear at ~15 years. Boys showed a greater rate of change for central systolic BP, central pulse pressure, CIMT, and carotid-femoral PWV. Conclusion. Gender-related differences in arterial characteristics of adults can be explained on the basis of different growing-related patterns between boys and girls, with no existing differences in prepubertal ages.
Tuberculosis (TB) is one of the ten leading causes of death worldwide. Patients with TB have been observed to suffer from depression and anxiety linked to social variables. Previous experiments found ...that the substantial pulmonary inflammation associated with TB causes neuroinflammation, neuronal death, and behavioral impairments in the absence of brain infection. Curcumin (CUR) is a natural product with antioxidant, anti-inflammatory and antibacterial activities. In this work, we evaluated the CUR effect on the growth control of mycobacteria in the lungs and the anti-inflammatory effect in the brain using a model of progressive pulmonary TB in BALB/c mice infected with drug-sensitive mycobacteria (strain H37Rv). The results have shown that CUR decreased lung bacilli load and pneumonia of infected animals. Finally, CUR significantly decreased neuroinflammation (expression of TNFα, IFNγ and IL12) and slightly increased the levels of nuclear factor erythroid 2-related to factor 2 (Nrf2) and the brain-derived neurotrophic factor (BDNF) levels, improving behavioral status. These results suggest that CUR has a bactericidal effect and can control pulmonary mycobacterial infection and reduce neuroinflammation. It seems that CUR has a promising potential as adjuvant therapy in TB treatment.
Tuberculosis (TB) is an important infectious disease and a public health problem. The organs most frequently affected by TB are the lungs; despite this, it has been reported that TB patients suffer ...from depression and anxiety, which have been attributed to social factors. In previous experimental work, we observed that the extensive pulmonary inflammation characteristic of TB with high cytokine production induces neuroinflammation, neuronal death and behavioral abnormalities in the absence of brain infection. The objective of the present work was to reduce this neuroinflammation and avoid the psycho-affective disorders showed during pulmonary TB. Glucocorticoids (GCs) are the first-line treatment for neuroinflammation; however, their systemic administration generates various side effects, mostly aggravating pulmonary TB due to immunosuppression of cellular immunity. Intranasal administration is a route that allows drugs to be released directly in the brain through the olfactory nerve, reducing their doses and side effects. In the present work, dexamethasone’s (DEX) intranasal administration was evaluated in TB BALB /c mice comparing three different doses (0.05, 0.25 and 2.5 mg/kg BW) on lung disease evolution, neuroinflammation and behavioral alterations. Low doses of dexamethasone significantly decreased neuroinflammation, improving behavioral status without aggravating lung disease.
