•Pixel- and object-based methods, and QuickBird imagery were used for mapping weeds.•Research was conducted at field-level (15 fields) and broad-level (entire image).•On both levels wild oat patches ...were accurately mapped in wheat fields.•Classification accuracies higher than 91% were obtained at both levels.•Weed maps showed potential to design a site-specific control strategy.
This paper compares of pixel- and object-based techniques for mapping wild oat weed patches in wheat fields using multi-spectral QuickBird satellite imagery for site-specific weed management. The research was conducted at two levels: (1) at the field level, on 11 and 15 individual infested wheat fields in 2006 and 2008, respectively, and (2) on a broader level, by analysing the entire 2006 and 2008 images. To evaluate the wild oat patches mapping at the field level, both pixel- and object-based image analyses were tested with six classification algorithms: Parallelepipeds (P), Mahalanobis Distance (MD), Maximum Likelihood (ML), Spectral Angle Mapper (SAM), Support Vector Machine (SVM) and Decision Tree (DT). The results showed that weed patches could be accurately detected with both analyses obtaining global accuracies between 80% and 99% for most of the fields. The MD and SVM classifiers were the most accurate for both the pixel- and object-based images from 2006 and 2008, respectively. In the broad-scale analysis, all of the wheat fields were identified in the imagery using a multiresolution hierarchical segmentation based on two scales. The first segmentation scale was classified using the MD and ML algorithms to discriminate wheat fields from other land uses. Accuracies greater than 85% were obtained for MD and 88% for ML for both imagery. A hierarchical analysis was then performed with the second segmentation scale, increasing the accuracies to 93% and 91% for 2006 and 2008 imagery, respectively. Finally, based on the most accurate results obtained in the field-level study, pixel-based classifications using the MD, ML and SVM algorithms were applied to the wheat fields identified. The results of these broad-level analyses showed that wild oat patches were accurately discriminated in all the wheat fields present in the entire images with accuracies greater than 91% for all the classifiers tested.
A group of eight Spanish lidars was formed in order to extend the European Aerosol Research Lidar Network-Advanced Sustainable Observation System (EARLINET-ASOS) project. This study presents ...intercomparisons at the hardware and software levels. Results of the system intercomparisons are based on range-square-corrected signals in cases where the lidars viewed the same atmospheres. Comparisons were also made for aerosol backscatter coefficients at 1064 nm (2 systems) and 532 nm (all systems), and for extinction coefficients at 532 nm (2 systems). In total, three field campaigns were carried out between 2006 and 2007. Comparisons were limited to the highest layer found before the free troposphere, i.e., either the atmospheric boundary layer or the aerosol layer just above it. Some groups did not pass the quality assurance criterion on the first attempt. Following modification and improvement to these systems, all systems met the quality criterion. The backscatter algorithm intercomparison consisted of processing lidar signal profiles simulated for two types of atmospheric conditions. Three stages with increasing knowledge of the input parameters were considered. The results showed that all algorithms work well when all inputs are known. They also showed the necessity to perform, when possible, additional measurements to attain better estimation of the lidar ratio, which is the most critical unknown in the elastic lidar inversion.
The objective of this work is to analyze the survival of infliximab, etanercept and adalimumab in patients who have switched among tumor necrosis factor (TNF) antagonists for the treatment of chronic ...arthritis. BIOBADASER is a national registry of patients with different forms of chronic arthritis who are treated with biologics. Using this registry, we have analyzed patient switching of TNF antagonists. The cumulative discontinuation rate was calculated using the actuarial method. The log-rank test was used to compare survival curves, and Cox regression models were used to assess independent factors associated with discontinuing medication. Between February 2000 and September 2004, 4,706 patients were registered in BIOBADASER, of whom 68% had rheumatoid arthritis, 11% ankylosing spondylitis, 10% psoriatic arthritis, and 11% other forms of chronic arthritis. One- and two-year drug survival rates of the TNF antagonist were 0.83 and 0.75, respectively. There were 488 patients treated with more than one TNF antagonist. In this situation, survival of the second TNF antagonist decreased to 0.68 and 0.60 at 1 and 2 years, respectively. Survival was better in patients replacing the first TNF antagonist because of adverse events (hazard ratio (HR) for discontinuation 0.55 (95% confidence interval (CI), 0.34-0.84)), and worse in patients older than 60 years (HR 1.10 (95% CI 0.97-2.49)) or who were treated with infliximab (HR 3.22 (95% CI 2.13-4.87)). In summary, in patients who require continuous therapy and have failed to respond to a TNF antagonist, replacement with a different TNF antagonist may be of use under certain situations. This issue will deserve continuous reassessment with the arrival of new medications.
Abstract
We describe a case report of a patient that was implanted with a posterior chamber phakic intraocular lens (Phakic Refractive Lens, PRL) for the correction of moderate myopia and who ...developed postoperatively a fixed mydriasis compatible with an Urrets-Zavalia Syndrome (UZS). Specifically, a sudden acute increase of IOP in the left eye was observed in the immediate postoperative period. After IOP stabilization, the refractive result was good, but a fixed and mydriatic pupil appeared. This condition led the patient to experience visual discomfort, halos, and glare associated with high levels of higher-order aberrations in spite of the good visual result. A tinted-contact lens was fitted in order to minimize those symptoms. The UZS should be considered as a possible complication after implantation of posterior chamber phakic intraocular lenses.
To evaluate the frequency and factors associated with cardiogenic shock (CS) in acute myocardial infarction (AMI) and unstable angina (UA) and percutaneous coronary intervention (PCI).
Spanish ...registry. The study period was June 1996 to December 2005. Follow-up was length of stay in an intensive care or coronary care unit (ICU/CCU). Multivariate studies evaluated factors associated with CS, mortality in CS, and PCI performance.
The study included 45.688 AMI patients and 17.277 UA patients. Cardiogenic shock occurred in 9.3% of patients with AMI and 1.79% of those with UA, frequencies that decreased over time. Variables associated with cardiogenic shock in AMI patients were female sex, age, type of infarction, diabetes, previous stroke, arrhythmia, previous angiography, complicated angina, and reinfarction. Hypertension and oral beta-blocking, ACE inhibitor, and hypolipidemic agents protected against CS. In UA, these variables were age, previous angina or AMI, right ventricular heart failure, arrhythmia. Beta-blocking agents were associated with a reduction in CS. Deaths from CS and AMI, respectively, were 62.8% and 38.7% in persons with UA. Doing PCIs has increased significantly; it is more prevalent in ex-smokers and those with right ventricular heart failure and mechanical ventilation; lower performance is associated with need for cardiopulmonary resuscitation; patients who die are older or have a history of AMI.
There has been a slight drop in the frequency of CS and its mortality. Factors associated with CS are similar to those associated with acute coronary syndromes. The frequency of PCI was low.

Background and objectives: by means of this update, the GARIN working group aims to define its position regarding the dietary treatment of patients with chronic kidney disease (CKD). In this area ...there are many aspects of uncertainty. Material and methods: bibliographical review and specific questions in advance were discussed and answered at a meeting in the form of conclusions. Results: the therapeutic action must be individualized and taking into account the degree of renal failure that the patient presents and their comorbidities. Regarding nutritional medical therapy, our group proposes three different levels of action, in which the recommendations of protein intake, fiber, fatty acids or potassium are different. In addition, we suggest using the phosphorus/protein ratio concept in adjusting the diet of the patient with CKD. We give recommendations regarding treatment in diabetes and artificial supplementation. Conclusions: these recommendations about dietary issues in patients with CKD can add value to clinical work.
